BAGGRUND:
Mere end 3 millioner personer i USA over 60 år har isoleret systolisk hypertension. De står over for en overdreven risiko (2-3 gange) for slagtilfælde, andre hjerte-kar-sygdomme og død. Befolkningsbaserede data viser, at prævalensen stiger fra cirka 8 procent i aldersgruppen 60-69 år til cirka 20 procent over 80 år. Baseret på tilgængelige data er der estimeret en årlig slagtilfælde på 2,0 procent i denne population. Det kliniske forsøg i fuld skala fulgte en pilotundersøgelse udført fra 1980 til 1983. Rekruttering i forsøget begyndte i marts 1985 og blev afsluttet i januar 1988. Opfølgningen sluttede i februar 1991. Dataanalysen fortsatte til oktober 1996.
DESIGN FORTÆLLING:
Et randomiseret, dobbeltblindt forsøg, hvor 2.365 forsøgspersoner blev tildelt aktiv behandling og 2.371 til placebo. For den aktive behandlingsgruppe blev der anvendt et trinvis plejeregime, som omfattede chlorthalidon 12,5 eller 25 mg/dag og efter behov tilsætning af atenolol 25 eller 50 mg/dag eller reserpin, 0,05 eller 0,10 mg/dag. Behandlingens mål var at reducere det systoliske blodtryk med mindst 20 mm Hg fra baseline og til under 160 mm Hg med minimale mængder af undersøgelsesmedicin. Det primære endepunkt var forekomsten af fatalt og ikke-fatalt slagtilfælde. Sekundære endepunkter var kardiovaskulær og koronar morbiditet og mortalitet, mortalitet af alle årsager og livskvalitetsmål.
