ARRIÈRE-PLAN:
Plus de 3 millions de personnes aux États-Unis de plus de 60 ans ont une hypertension systolique isolée. Ils font face à un risque excessif (2 à 3 fois) d'accident vasculaire cérébral, d'autres maladies cardiovasculaires et de décès. Les données basées sur la population montrent que la prévalence passe d'environ 8 % dans le groupe d'âge des 60 à 69 ans à environ 20 % chez les 80 ans. Sur la base des données disponibles, un taux d'AVC annuel de 2,0 % a été estimé dans cette population. L'essai clinique à grande échelle faisait suite à une étude pilote menée de 1980 à 1983. Le recrutement pour l'essai a commencé en mars 1985 et s'est terminé en janvier 1988. Le suivi s'est terminé en février 1991. L'analyse des données s'est poursuivie jusqu'en octobre 1996.
NARRATIF DE CONCEPTION :
Un essai randomisé en double aveugle dans lequel 2 365 sujets ont été assignés au traitement actif et 2 371 au placebo. Pour le groupe de traitement actif, un régime de soins par étapes a été utilisé qui comprenait de la chlorthalidone 12,5 ou 25 mg/jour et, au besoin, l'ajout d'aténolol 25 ou 50 mg/jour ou de réserpine, 0,05 ou 0,10 mg/jour. L'objectif du traitement était de réduire la pression artérielle systolique d'au moins 20 mm Hg par rapport au départ et à moins de 160 mm Hg avec des quantités minimales de médicaments à l'étude. Le critère d'évaluation principal était l'incidence des accidents vasculaires cérébraux mortels et non mortels. Les critères d'évaluation secondaires étaient la morbidité et la mortalité cardiovasculaires et coronariennes, la mortalité toutes causes confondues et les mesures de la qualité de vie.
