Более 3 миллионов человек в США старше 60 лет имеют изолированную систолическую гипертензию. Они сталкиваются с повышенным риском (в 2-3 раза) инсульта, других сердечно-сосудистых заболеваний и смерти. Популяционные данные показывают, что распространенность увеличивается примерно с 8 процентов в возрастной группе 60–69 лет до примерно 20 процентов в возрасте старше 80 лет. На основании имеющихся данных ежегодная частота инсультов в этой популяции оценивается в 2,0 процента. Полномасштабное клиническое испытание последовало за пилотным исследованием, проводившимся с 1980 по 1983 год. Набор в суд начался в марте 1985 года и закончился в январе 1988 года. Последующее наблюдение закончилось в феврале 1991 года. Анализ данных продолжался до октября 1996 года.
ОПИСАНИЕ ДИЗАЙНА:
Рандомизированное двойное слепое исследование, в котором 2365 пациентов получали активное лечение, а 2371 — плацебо. Для группы активного лечения использовали ступенчатую схему лечения, которая включала хлорталидон 12,5 или 25 мг/день и, при необходимости, добавление атенолола 25 или 50 мг/день или резерпина 0,05 или 0,10 мг/день. Целью лечения было снижение систолического артериального давления по крайней мере на 20 мм рт. ст. по сравнению с исходным уровнем и до уровня ниже 160 мм рт. ст. при минимальном количестве исследуемого препарата. Первичной конечной точкой была частота фатальных и нефатальных инсультов. Вторичными конечными точками были сердечно-сосудистые и коронарные заболевания и смертность, смертность от всех причин и показатели качества жизни.
