ACHTERGROND:
Meer dan 3 miljoen personen in de Verenigde Staten ouder dan 60 jaar hebben geïsoleerde systolische hypertensie. Ze lopen een verhoogd risico (2-3 maal) op een beroerte, andere hart- en vaatziekten en overlijden. Uit populatiegegevens blijkt dat de prevalentie stijgt van ongeveer 8 procent in de leeftijdsgroep 60-69 jaar tot ongeveer 20 procent boven de 80 jaar. Op basis van beschikbare gegevens wordt in deze populatie een jaarlijks percentage beroertes van 2,0 procent geschat. De grootschalige klinische proef volgde op een pilootstudie die werd uitgevoerd van 1980 tot 1983. De rekrutering in het proces begon in maart 1985 en eindigde in januari 1988. Follow-up eindigde in februari 1991. De gegevensanalyse ging door tot en met oktober 1996.
ONTWERP VERHAAL:
Een gerandomiseerde, dubbelblinde studie waarin 2.365 proefpersonen een actieve behandeling kregen en 2.371 een placebo. Voor de actieve behandelingsgroep werd een getrapt zorgregime gebruikt dat chloortalidon 12,5 of 25 mg/dag omvatte, en indien nodig toevoeging van atenolol 25 of 50 mg/dag of reserpine, 0,05 of 0,10 mg/dag. Het doel van de behandeling was om de systolische bloeddruk te verlagen met ten minste 20 mm Hg ten opzichte van de uitgangswaarde en tot minder dan 160 mm Hg met minimale hoeveelheden onderzoeksmedicatie. Het primaire eindpunt was de incidentie van fatale en niet-fatale beroerte. Secundaire eindpunten waren cardiovasculaire en coronaire morbiditeit en mortaliteit, mortaliteit door alle oorzaken en metingen van kwaliteit van leven.
