FUNDO:
Mais de 3 milhões de pessoas nos Estados Unidos com mais de 60 anos têm hipertensão sistólica isolada. Eles enfrentam um risco excessivo (2-3 vezes) de acidente vascular cerebral, outras doenças cardiovasculares e morte. Dados baseados na população mostram que a prevalência sobe de aproximadamente 8% na faixa etária de 60-69 anos para aproximadamente 20% acima dos 80 anos. Com base nos dados disponíveis, uma taxa anual de AVC de 2,0% foi estimada nessa população. O ensaio clínico em grande escala seguiu um estudo piloto conduzido de 1980 a 1983. O recrutamento no julgamento começou em março de 1985 e foi concluído em janeiro de 1988. O acompanhamento terminou em fevereiro de 1991. A análise dos dados continuou até outubro de 1996.
NARRATIVA DO DESENHO:
Um estudo randomizado, duplo-cego no qual 2.365 indivíduos foram designados para tratamento ativo e 2.371 para placebo. Para o grupo de tratamento ativo, foi utilizado um esquema de tratamento escalonado que incluía clortalidona 12,5 ou 25 mg/dia e, conforme necessário, adição de atenolol 25 ou 50 mg/dia ou reserpina, 0,05 ou 0,10 mg/dia. O objetivo do tratamento era reduzir a pressão arterial sistólica em pelo menos 20 mm Hg da linha de base e abaixo de 160 mm Hg com quantidades mínimas da medicação do estudo. O endpoint primário foi a incidência de AVC fatal e não fatal. Os desfechos secundários foram morbidade e mortalidade cardiovascular e coronariana, mortalidade por todas as causas e medidas de qualidade de vida.
