HÁTTÉR:
Az Egyesült Államokban több mint 3 millió 60 év feletti embernek van izolált szisztolés magas vérnyomása. Nagyobb (2-3-szoros) kockázattal néznek szembe a stroke, más szív- és érrendszeri betegségek és a halálozás kockázatával. A népességen alapuló adatok azt mutatják, hogy a prevalencia a 60-69 éves korcsoport körülbelül 8 százalékáról körülbelül 20 százalékra növekszik a 80 év feletti korosztályban. A rendelkezésre álló adatok alapján 2,0 százalékos éves stroke-arányt becsültek ebben a populációban. A teljes körű klinikai vizsgálat egy 1980 és 1983 között végzett kísérleti vizsgálatot követett. A tárgyaláson a toborzás 1985 márciusában kezdődött és 1988 januárjában fejeződött be. A nyomon követés 1991 februárjában ért véget. Az adatok elemzése 1996 októberéig folytatódott.
TERVEZÉSI NARRATÍV:
Egy randomizált, kettős vak vizsgálat, amelyben 2365 alanyt jelöltek ki aktív kezelésre és 2371 alanyt placebóra. Az aktív kezelési csoportnál lépcsőzetes gondozási rendet alkalmaztunk, amely 12,5 vagy 25 mg/nap klórtalidont, és szükség szerint napi 25 vagy 50 mg atenolol vagy 0,05 vagy 0,10 mg/nap rezerpin hozzáadását tartalmazta. A kezelés célja az volt, hogy a szisztolés vérnyomást a kiindulási értékhez képest legalább 20 Hgmm-rel és 160 Hgmm alá csökkentsék minimális mennyiségű vizsgálati gyógyszerrel. Az elsődleges végpont a halálos és nem halálos kimenetelű stroke incidenciája volt. A másodlagos végpontok a kardiovaszkuláris és koszorúér-megbetegedések és mortalitások, a minden ok miatti mortalitás és az életminőség mérései voltak.
