Más de 3 millones de personas en los Estados Unidos mayores de 60 años tienen hipertensión sistólica aislada. Se enfrentan a un riesgo excesivo (2-3 veces) de accidente cerebrovascular, otras enfermedades cardiovasculares y muerte. Los datos basados en la población muestran que la prevalencia aumenta de aproximadamente el 8 % en el grupo de edad de 60 a 69 años a aproximadamente el 20 % en personas mayores de 80 años. Según los datos disponibles, se ha estimado una tasa anual de accidentes cerebrovasculares del 2,0 % en esta población. El ensayo clínico a gran escala siguió a un estudio piloto realizado entre 1980 y 1983. El reclutamiento en el ensayo comenzó en marzo de 1985 y finalizó en enero de 1988. El seguimiento finalizó en febrero de 1991. El análisis de datos continuó hasta octubre de 1996.
NARRATIVA DE DISEÑO:
Un ensayo aleatorizado, doble ciego en el que 2365 sujetos fueron asignados al tratamiento activo y 2371 al placebo. Para el grupo de tratamiento activo, se utilizó un régimen escalonado que incluía clortalidona 12,5 o 25 mg/día y, según fuera necesario, atenolol 25 o 50 mg/día o reserpina, 0,05 o 0,10 mg/día. El objetivo del tratamiento era reducir la presión arterial sistólica en al menos 20 mm Hg desde el inicio y por debajo de 160 mm Hg con cantidades mínimas de medicación del estudio. El punto final primario fue la incidencia de accidentes cerebrovasculares fatales y no fatales. Los criterios de valoración secundarios fueron la morbilidad y mortalidad cardiovascular y coronaria, la mortalidad por todas las causas y las medidas de calidad de vida.
