BAKGRUNN:
Mer enn 3 millioner personer i USA over 60 år har isolert systolisk hypertensjon. De står overfor en overrisiko (2-3 ganger) for hjerneslag, andre hjerte- og karsykdommer og død. Befolkningsbaserte data viser at prevalensen stiger fra cirka 8 prosent i aldersgruppen 60-69 år til cirka 20 prosent over 80 år. Basert på tilgjengelige data er det anslått en årlig slagrate på 2,0 prosent i denne populasjonen. Den fullskala kliniske studien fulgte en pilotstudie utført fra 1980 til 1983. Rekrutteringen i rettssaken begynte i mars 1985 og ble avsluttet i januar 1988. Oppfølgingen ble avsluttet i februar 1991. Dataanalysen fortsatte gjennom oktober 1996.
DESIGN NARRATIV:
En randomisert, dobbeltblind studie der 2365 personer ble tildelt aktiv behandling og 2371 til placebo. For den aktive behandlingsgruppen ble det brukt et trinnvis behandlingsregime som inkluderte klortalidon 12,5 eller 25 mg/dag, og etter behov, tillegg av atenolol 25 eller 50 mg/dag eller reserpin, 0,05 eller 0,10 mg/dag. Behandlingsmålet var å redusere systolisk blodtrykk med minst 20 mm Hg fra baseline og til under 160 mm Hg med minimale mengder studiemedisin. Det primære endepunktet var forekomsten av dødelig og ikke-dødelig hjerneslag. Sekundære endepunkter var kardiovaskulær og koronar morbiditet og dødelighet, dødelighet av alle årsaker og livskvalitetsmål.
