Dette er et internationalt, multicenter, parallelgruppe, hændelsesdrevet, randomiseret, dobbeltblindt, placebokontrolleret studie hos patienter med kronisk hjertesvigt med reduceret ejektionsfraktion (HFrEF), der evaluerer effekten af dapagliflozin versus placebo givet én gang dagligt i Ud over baggrundsbehandling med regional standard for pleje, til forebyggelse af kardiovaskulær (CV) død eller reduktion af hjertesvigt (HF) hændelser.

Inklusionskriterier:
- Levering af underskrevet informeret samtykke forud for undersøgelsesspecifikke procedurer
- Mand eller kvinde, i alderen ≥18 år
- Etableret dokumenteret diagnose af symptomatisk HFrEF (NYHA funktionsklasse II-IV), som har været til stede i mindst 2 måneder
- LVEF≤40 %
- Forhøjede NT-proBNP niveauer
- Patienter bør modtage baggrundsstandardbehandling for HFrEF og behandles i henhold til lokalt anerkendte retningslinjer
- eGFR ≥30 ml/min/1,73 m^2 (CKD-EPI-formel) ved tilmelding (besøg 1)
Ekskluderingskriterier:
- Modtagelse af behandling med en SGLT2-hæmmer inden for 8 uger før optagelse eller tidligere intolerance af en SGLT2-hæmmer
- Type 1 diabetes mellitus
- Symptomatisk hypotension eller systolisk blodtryk <95 mmHg ved 2 ud af 3 målinger enten ved besøg 1 eller besøg 2
- Aktuel akut dekompenseret HF eller indlæggelse på grund af dekompenseret HF <4 uger før indskrivning
- MI, ustabil angina, slagtilfælde eller forbigående iskæmisk anfald inden for 12 uger før indskrivning
- Koronar revaskularisering (perkutan koronar intervention eller koronar bypasstransplantation) eller klapreparation/udskiftning inden for 12 uger før indskrivning eller planlagt at gennemgå nogen af disse operationer efter randomisering
- Implantation af en CRT inden for 12 uger før tilmelding eller hensigt om at implantere en CRT-enhed
- Tidligere hjertetransplantation eller implantation af en ventrikulær hjælpeanordning eller lignende anordning, eller implantation forventet efter randomisering
- HF på grund af restriktiv kardiomyopati, aktiv myocarditis, konstriktiv pericarditis, hypertrofisk (obstruktiv) kardiomyopati eller ukorrigeret primær klapsygdom
- Symptomatisk bradykardi eller anden eller tredje grads hjerteblok uden pacemaker
- Alvorlig (eGFR <30 ml/min/1,73 m^2 af CKD-EPI), ustabil eller hurtigt fremadskridende nyresygdom på tidspunktet for randomisering
