Beschreibung
Einschlusskriterien:
- Die Diagnose eines HCC wurde entweder histologisch oder klinisch gemäß den AASLD-Kriterien für Patienten mit Leberzirrhose bestätigt. Für Teilnehmer ohne Leberzirrhose ist eine histologische Sicherung zwingend erforderlich.
- HCC, das nach Meinung der an der Betreuung des Teilnehmers beteiligten Chirurgen und Onkologen oder Hepatologen für eine chirurgische R0-Resektion mit kurativer Absicht geeignet ist. Geeignet sind Patienten mit resektablem HCC innerhalb oder außerhalb der Milan-Kriterien (ohne extrahepatische Ausbreitung oder makrovaskuläre Invasion).
- Messbare Krankheit (mindestens eine Zielläsion) gemäß RECIST v1.1, wie vom Prüfer bestimmt
- Leistungsstatus der Eastern Cooperative Oncology Group (ECOG) von 0 oder 1 innerhalb von 7 Tagen vor der Randomisierung
- Child-Pugh-Klasse A innerhalb von 7 Tagen vor der Randomisierung
- Negativer HIV-Test beim Screening
- Keine vorherige lokoregionäre oder systemische Behandlung von HCC
- Ausreichende hämatologische und Endorganfunktion
- Dokumentierter virologischer Status der Hepatitis
- Für Frauen im gebärfähigen Alter: Zustimmung zur Abstinenz (Verzicht auf heterosexuellen Geschlechtsverkehr) oder zur Empfängnisverhütung
- Für Männer: Einverständnis zur Abstinenz (Verzicht auf heterosexuellen Geschlechtsverkehr) bzw. zur Empfängnisverhütung und Einverständnis zum Verzicht auf Samenspenden
Allgemeine Ausschlusskriterien:
- Vorliegen einer extrahepatischen Erkrankung oder einer makrovaskulären Invasion
- Bekanntes fibrolamellares HCC, sarkomatoides HCC, gemischtes Cholangiokarzinom und HCC oder andere seltene Varianten von HCC
- Vorgeschichte einer hepatischen Enzephalopathie, wenn diese innerhalb eines Jahres vor Beginn der Studienbehandlung klinisch signifikant war
- Mäßiger oder schwerer Aszites
- Aktive Koinfektion mit HBV und HCV
- Aktive Koinfektion mit HBV und einer Hepatitis-D-Virusinfektion
- Vorherige Behandlung mit CD137-Agonisten oder Immun-Checkpoint-Inhibitoren, einschließlich therapeutischer Anti-CTLA-4-, Anti-PD-1- und Anti-PD-L1-Antikörper
- Behandlung mit einer Prüftherapie innerhalb von 28 Tagen vor Beginn der Studienbehandlung
- Unbehandelte oder unvollständig behandelte Ösophagus- und/oder Magenvarizen, die bluten oder bei denen ein hohes Blutungsrisiko besteht
- Ein früheres Blutungsereignis aufgrund von Ösophagus- und/oder Magenvarizen innerhalb von 6 Monaten vor Beginn der Studienbehandlung
- Unzureichend kontrollierter Bluthochdruck
- Vorgeschichte einer hypertensiven Krise oder einer hypertensiven Enzephalopathie
- Signifikante Gefäßerkrankung innerhalb von 6 Monaten vor Beginn der Studienbehandlung
- Hämoptyse in der Anamnese innerhalb eines Monats vor Beginn der Studienbehandlung
- Anzeichen einer Blutungsdiathese oder einer erheblichen Koagulopathie
- Aktuelle oder kürzliche (<= 10 Tage vor Beginn der Studienbehandlung) Anwendung oraler oder parenteraler Antikoagulanzien oder Thrombolytika in voller Dosis zu therapeutischen Zwecken
- Vorgeschichte von Bauch- oder tracheoösophagealen Fisteln, Magen-Darm-Perforationen oder intraabdominalen Abszessen innerhalb von 6 Monaten vor Beginn der Studienbehandlung
- Anamnese einer Darmobstruktion und/oder klinische Anzeichen oder Symptome einer GI-Obstruktion
- Schwere, nicht heilende oder dehiszierende Wunde, aktives Geschwür oder unbehandelter Knochenbruch
- Grad >= Proteinurie
- Größerer chirurgischer Eingriff, offene Biopsie oder schwerwiegende traumatische Verletzung oder Bauchoperation, Eingriffe oder traumatische Verletzungen oder Erwartung der Notwendigkeit eines größeren chirurgischen Eingriffs außer einer möglicherweise kurativen Leberresektion
- Chronische tägliche Behandlung mit einem nichtsteroidalen Antiphlogistikum (NSAID)
- Schwerwiegende Infektion, die orale oder intravenöse Antibiotika und/oder einen Krankenhausaufenthalt erfordert

MPR rate was defined as the percentage of participants who had achieved MPR and was estimated for each treatment cohort in the efficacy-evaluable population.

Intrahepatic recurrence was defined by the appearance of one or more intrahepatic lesions with a longest diameter of > 1 cm and a typical vascular pattern of HCC on dynamic imaging (i.e., hypervascularization in the arterial phase with washout in the portal venous or late venous phase).
Extrahepatic recurrence was assessed by RECIST v1.1 as the appearance of new, measurable malignant lesions outside the liver.
Data for participants who did not have documented recurrence of disease or death were censored at the day of the last tumor assessment for participants.
Kaplan-Meier (K-M) method was used to estimate median RFS.
