Verstellbare Prothesenschäfte für Veteranen mit Amputation der unteren Extremitäten: Patientenbefragungsergebnisse und Innendruckmessungen im Schaft
Jedes Jahr schließen sich etwa 1500 Veteranen den 623.000 Amerikanern an, die mit einer Amputation der unteren Gliedmaßen leben. Viele dieser Veteranen wählen den mühsamen Weg der Rehabilitation, um gehfähig zu bleiben, ein Prozess, der die Verschreibung einer Prothese für die unteren Gliedmaßen beinhaltet. Viel hängt von der Passgenauigkeit ihrer Prothese ab. Eine gute Passform fühlt sich bequem an und ermöglicht eine Vielzahl von Gehaktivitäten. Eine schlechte Passform führt zu Unbehagen, oft begleitet von chronischen Hautproblemen, und Aktivitäten des täglichen Lebens werden eingeschränkt.
Konventionelle Prothesenschäfte werden von einem geschickten Orthopädietechniker maßgefertigt, sorgfältig geformt, um auf den Stumpf des Veteranen zu passen, und gute Passformen können normalerweise erreicht werden. Diese Schäfte sind starre, festgeformte Strukturen mit der Robustheit eines Bulldozers, gebaut für anhaltende, schwere Belastungen. Leider kann sich die Form des Stumpfes einer Person im Laufe der Zeit ändern, so dass eine gute Passform schließlich schlecht wird. Eine Schätzung deutet darauf hin, dass alle drei bis vier Jahre ein neuer Unterschenkelschaft benötigt wird. Die Kosten für den Ersatz von Schäften, die nicht mehr passen, sind nicht unerheblich. Die Medicare-Ausgaben für den Ersatz bestehender Unterschenkelschäfte und zugehöriger Komponenten wurden 2017 auf 50 Millionen US-Dollar geschätzt. Wichtig ist, dass Medicare auf nur 20 % des Marktes geschätzt wird, was darauf hindeutet, dass die geschätzten Ausgaben für die gesamte Bevölkerung mit Amputationen der unteren Gliedmaßen wahrscheinlich erheblich höher sind. Eine gut sitzende Prothese, die ihre Passform über die Zeit beibehält, würde Veteranen gut dienen.
Eine mögliche Lösung, die eine gute Passform länger erhalten könnte, ist ein einstellbarer Prothesenschaft. Obwohl nur wenige Beweise für die Verschreibungspraxis vorliegen, haben die Centers for Medicare and Medicaid Services (CMS) kürzlich die Erstattung für einstellbare Prothesenschäfte genehmigt, was auf einen dringenden Bedarf für ein solches Produkt hindeutet.
Um das Potenzial für schädliche Auswirkungen und die Nutzung einstellbarer Prothesenschäfte durch Veteranen zu untersuchen, zielt diese Forschung darauf ab, den Druck zu bestimmen, der während Aktivitäten unterschiedlicher Intensität durch einen einstellbaren Prothesenschaft auf den Stumpf ausgeübt wird, sowie die Aktivitäten, die Anpassungen auslösen.
Ein laborbasierter Versuch mit menschlichen Probanden wird unter Verwendung eines maßgefertigten Sensors durchgeführt, der in der einstellbaren Platte eines studienbereitgestellten einstellbaren Prothesenschaftes platziert wird. Die Teilnehmer werden sich zwei Wochen lang im Feld an den einstellbaren Prothesenschaft gewöhnen und dann ins Labor zurückkehren, um sieben Aktivitäten durchzuführen. Der Druck innerhalb der Prothese wird während dieser Aktivitäten gemessen. Die Teilnehmer werden auch berichten, wie eng ihr Schaft nach jeder Aktivität empfunden wird.

Prothesenschäfte, die die Gehfähigkeit von Veteranen mit Amputationen der unteren Extremitäten ermöglichen, sind in der Regel starre, übermäßig starke Strukturelemente mit fester Form, die als Schnittstelle zwischen Körper und Umwelt dienen. Der Prothesenschaft eines Veteranen ist maßgeschneidert; er soll nur einer einzigen Person passen.
Änderungen der Passform nach der Versorgung mit einer Prothese sind eher die Regel als die Ausnahme. Änderungen im Zusammenhang mit Körpermasse, Muskelatrophie oder persönlicher Fitness können über Monate hinweg auftreten, während Änderungen aufgrund von Ernährung oder diabetischen Komplikationen innerhalb von Stunden auftreten können. Wenn die Formabweichung zwischen dem Stumpf und dem Schaft gering ist, kann der Veteran den Unterschied durch das An- oder Ausziehen von Stumpfstrümpfen ausgleichen. Dieser Ansatz ist nicht ideal, da einige Patienten eher geneigt sind, diese Praxis anzunehmen als andere. Ist die Formabweichung groß, ist in der Regel ein neuer Prothesenschaft erforderlich. Egal ob klein oder groß, das Versäumnis, Änderungen zu bewältigen, kann sich negativ auf die Nutzung der Prothese und die Gesundheit des Stumpfes auswirken.
Trotz seiner Einschränkungen ist der Prothesenschaft mit fester Form seit Jahrzehnten der Standard der Versorgung. Ein potenziell bedeutendes Ereignis fand jedoch im April 2024 statt, als die Centers for Medicare and Medicaid Services (CMS) die Erstattung für einstellbare Prothesenschäfte genehmigten. Während die neue Genehmigung die Bezahlung für deren Bereitstellung ermöglicht, gibt es nur wenige Belege, die Patienten, Anbietern und Kostenträgern helfen, die potenziellen Ergebnisse der Verschreibung eines einstellbaren Prothesenschaftes zu verstehen.
Ziel dieser Forschung ist es herauszufinden, wie Veteranen einstellbare Prothesenschäfte nutzen, indem der Druck gemessen wird, den ein einstellbarer Prothesenschaft während Aktivitäten unterschiedlicher Intensität auf den Stumpf ausübt, und zu identifizieren, welche Aktivitäten möglicherweise eine Anpassung auslösen. Die Untersucher werden gehfähige Veteranen mit einseitigen transtibialen Amputationen (n = 25) rekrutieren und sie mit einem einstellbaren Prothesenschaft ausstatten.
Die Teilnehmer werden nach zweiwöchigem Tragen eines nicht instrumentierten Duplikats des einstellbaren Prothesenschaftes einen instrumentierten einstellbaren Prothesenschaft tragen. Den Teilnehmern wird erlaubt, 5 bis 10 Minuten umherzugehen, während der Forschungsprothetiker die Passform und Ausrichtung der Studienprothese überprüft. Anschließend werden Druckmessungen im Sitzen, Stehen und Gehen durchgeführt. Die Steh- und Gehaktivitäten werden in zufälliger Reihenfolge durchgeführt, unterbrochen von 1-minütigen Sitzpausen, in denen der Teilnehmer den Druck der einstellbaren Paneele löst. Die Gehversuche werden auf einem Laufband durchgeführt.

- Gehen 15 % schneller als selbstgewählte Geschwindigkeit 1 min
- Gehen mit selbstgewählter Geschwindigkeit 10 min. Unmittelbar vor Beginn jeder Aktivität wird dem Teilnehmer mitgeteilt, welche Aktivität als nächstes kommt, und er wird aufgefordert, seinen Schaft entsprechend anzupassen. Teilnehmer können während einer Aktivität auch freiwillig Anpassungen vornehmen. In diesem Fall wird die Dauer der Aktivität so verlängert, dass 1 Minute Aktivität bei der neuen Einstellung verbleibt. Am Ende jeder Aktivität wird die Enge mithilfe einer Umfragefrage bewertet. Konkret werden die Untersucher nach jeder Aktivität jeden Teilnehmer bitten, die Enge seines einstellbaren Prothesenschaftes im Moment auf einer Skala von 0 bis 10 zu bewerten.
Im Allgemeinen nehmen die Untersucher an, dass der auf den Stumpf ausgeübte Druck von der Aktivität abhängt, ebenso wie die Enge.

- Veteranen mit Amputation der unteren Gliedmaßen
- haben eine einseitige transtibiale Amputation
- tragen nach eigenen Angaben mindestens 4 Stunden täglich eine Prothese
- mindestens ein Jahr nach der Amputation
- haben eine modulare Prothese mit Vier-Bolzen-Schaftbefestigung
- können auf einem Laufband gehen
Ausschlusskriterien:
- haben eine unpassende Anpassung und Aufhängung ihrer aktuellen Prothese, die mit klinischen Mitteln nicht korrigiert werden kann
- haben aktuelle Hautreizungen oder Verletzungen des Stumpfes, Arthrose, Verletzungen oder Schmerzen, die die Gehfähigkeit beeinträchtigen

Experimental: Einstellbare Prothesenhülse
Teilnehmer tragen einen verstellbaren Prothesenschaft.
|
Die Untersucher werden für jeden Teilnehmer einen verstellbaren Prothesenschaft herstellen und zusammenbauen, dessen Form eine exakte Kopie ihres bestehenden, verordneten Prothesenschafts ist.
Wir werden drei verstellbare Platten am Prothesenschaft anbringen, die sich über dem medialen und lateralen proximalen Tibiabereich sowie dem hinteren Gastrocnemiusbereich befinden.
Die Position der Platten kann mithilfe eines Kabelmechanismus ein- und ausgestellt werden.
