Soquetes Ajustáveis para Veteranos com Amputação de Membros Inferiores: Resultados Reportados pelos Pacientes e Pressões Intra-soquete
Anualmente, cerca de 1500 Veteranos juntam-se aos 623.000 americanos que vivem com uma amputação de membro inferior. Muitos destes Veteranos escolhem o árduo caminho da reabilitação para permanecerem ambulatórios, um processo que inclui a prescrição de uma prótese de membro inferior. Muito depende do bom ajuste da sua prótese. Um bom ajuste é confortável e permite uma variedade de atividades ambulatórias. Um mau ajuste resulta em desconforto, muitas vezes acompanhado por problemas crónicos de pele, e as atividades da vida diária são limitadas.
As cavidades prostéticas convencionais são construídas à medida por um prostesista qualificado, cuidadosamente moldadas para se ajustarem ao membro residual do Veterano, e geralmente é possível obter um bom ajuste. Estas cavidades são estruturas rígidas de forma fixa com a robustez de uma escavadora, construídas para ação sustentada e de alta resistência. Infelizmente, a forma do membro residual de um indivíduo pode mudar ao longo do tempo, de modo que um bom ajuste acaba por se tornar mau. Uma estimativa sugere que uma nova cavidade abaixo do joelho é necessária a cada três anos e meio. A despesa de substituir cavidades que já não se ajustam não é insignificante. Os gastos do Medicare com a substituição de cavidades abaixo do joelho existentes e componentes associados foram estimados em 50 milhões de dólares em 2017. Importante, estima-se que o Medicare represente apenas 20% do mercado, sugerindo que a despesa estimada para toda a população com amputações de membros inferiores é provavelmente significativamente maior. Uma solução potencial que poderia fazer com que um bom ajuste durasse mais tempo é uma cavidade prostética ajustável. Embora existam poucas evidências para apoiar a prática de prescrição, os Centros de Serviços Medicare e Medicaid (CMS) autorizaram recentemente o reembolso para cavidades prostéticas ajustáveis, sugerindo uma necessidade premente de tal produto.
Para investigar o potencial de efeitos deletérios e como os Veteranos poderiam usar cavidades prostéticas ajustáveis, o objetivo desta investigação é determinar a pressão aplicada ao membro residual por uma cavidade prostética ajustável durante atividades de intensidade variável e as atividades que induzem ajustes.
Será realizado um experimento de laboratório com sujeitos humanos utilizando um sensor personalizado colocado no interior do painel ajustável de uma cavidade prostética ajustável fornecida pelo estudo. Os participantes irão habituar-se à cavidade prostética ajustável durante duas semanas no campo, depois regressarão ao laboratório para realizar sete atividades. A pressão no interior da prótese será medida durante estas atividades. Os participantes também irão relatar quão apertada a sua cavidade lhes parece após cada atividade.

As tomadas de próteses que permitem a mobilidade ambulatória de Veteranos com amputação do membro inferior são tipicamente elementos estruturais rígidos, excessivamente fortes e de forma fixa que atuam como a interface entre o corpo e o ambiente. A tomada protésica de um Veterano é personalizada; destina-se a caber a uma pessoa e apenas a uma pessoa.
As alterações no encaixe após o fornecimento de uma prótese são mais comuns do que não. As alterações relacionadas com a massa corporal, atrofia muscular ou aptidão física pessoal podem ocorrer ao longo de meses, enquanto as alterações devido a dieta ou complicações diabéticas podem ocorrer ao longo de horas. Se o desajuste na forma entre o membro residual e a tomada for pequeno, o Veterano pode gerir a diferença colocando ou retirando meias do membro residual. A abordagem não é ideal, uma vez que alguns doentes estão mais inclinados a adotar a prática do que outros. Se o desajuste na forma for grande, é geralmente necessária uma nova tomada protésica. Seja pequeno ou grande, a falha em gerir as alterações pode afetar negativamente a sua utilização da prótese e a saúde do seu membro residual.
Apesar das suas limitações, a tomada protésica de forma fixa tem sido o padrão de cuidados durante décadas. No entanto, um evento potencialmente significativo ocorreu em abril de 2024, quando os Centros de Serviços Medicare e Medicaid (CMS) autorizaram o reembolso para tomadas de próteses ajustáveis. Embora a nova autorização permita o pagamento pelo seu fornecimento, há poucas evidências para ajudar os doentes, prestadores e pagadores a compreender os potenciais resultados da prescrição de uma tomada protésica ajustável.
O objetivo desta investigação é descobrir como os Veteranos utilizam tomadas de próteses ajustáveis, medindo a pressão aplicada ao membro residual por uma tomada protésica ajustável durante atividades de intensidade variável, e identificar quais atividades podem induzir um ajuste. Os investigadores recrutarão Veteranos ambulatórios com amputações transtibiais unilaterais (n = 25) e equipá-los-ão com uma tomada protésica ajustável.
Os participantes usarão uma tomada protésica ajustável instrumentada após usarem uma tomada protésica ajustável duplicada não instrumentada durante duas semanas. Os participantes serão autorizados a caminhar durante 5 a 10 minutos enquanto o prostetista de investigação verifica o encaixe e o alinhamento da prótese do estudo. As medições de pressão serão então recolhidas enquanto sentados, de pé e a caminhar. As atividades de pé e caminhada serão realizadas em ordem aleatória, intercaladas com descansos sentados de 1 minuto, onde o participante libertará a pressão do painel ajustável. Os ensaios de caminhada serão realizados numa passadeira.

- Caminhar 15% mais rápido do que a velocidade autosselecionada 1 min
- Caminhar a velocidade autosselecionada 10 min. Imediatamente antes do início de cada atividade, será dito ao participante qual é a próxima atividade e será solicitado que ajuste a sua tomada em conformidade. Os participantes também serão livres de fazer ajustes durante uma atividade. Nesta situação, a duração da atividade será prolongada de modo a que restem 1 minuto de atividade na nova configuração de ajuste. No final de cada atividade, a sensação de aperto será avaliada usando uma pergunta de inquérito. Especificamente, após cada atividade, os investigadores pedirão a cada participante que classifique a sensação de aperto da sua tomada protésica ajustável no momento numa escala de 0 a 10.
Em geral, os investigadores hipotetizam que a pressão aplicada ao membro residual dependerá da atividade, assim como a sensação de aperto.

Critérios de Inclusão:
- Veteranos com amputação do membro inferior
- ter uma amputação transtibial unilateral
- usar uma prótese durante pelo menos 4 horas por dia, por auto-relato
- pelo menos um ano pós-amputação
- ter uma prótese modular que inclua uma fixação de encaixe de quatro parafusos
- conseguir caminhar numa passadeira
Critérios de Exclusão:
- ter um ajuste e suspensão inadequados com a sua prótese atual, e um bom ajuste não pode ser alcançado com os recursos clínicos disponíveis
- ter irritação cutânea ou lesão atuais no membro residual, osteoartrite, lesão ou dor que interfira com a capacidade de caminhar

Experimental: Encaixe protésico ajustável
Os participantes usarão uma tomada protética ajustável.
|
Os investigadores fabricarão e montarão uma prótese ajustável para cada participante, cujo formato é uma réplica da sua prótese atual, conforme prescrita.
Adicionaremos três painéis ajustáveis à prótese, que estarão localizados sobre a região medial e lateral proximal da tíbia e sobre a região posterior do gastrocnémio.
A posição dos painéis será ajustável para dentro e para fora através de um mecanismo de cabo.
