Eine prospektive Phase-I- und -II-Studie mit Ramucirumab + Pembrolizumab bei Patienten mit rezidivierendem/metastatischem Kopf-Hals-Plattenepithelkarzinom

Einschlusskriterien:

  • Unheilbares HNSCC, definiert als RM-Erkrankung, die nicht durch Operation und/oder Strahlentherapie geheilt werden kann, oder Patient mit HNSCC, der abfällt oder für eine kurative Therapie nicht geeignet ist

  • Krankheitsbewertung:

    • In Phase I auswertbare oder messbare Erkrankung.
    • In Phase II messbare Erkrankung gemäß RECIST 1.1
  • Vorbehandlung:

    • Für Phase I eine beliebige Anzahl von Vortherapielinien für RM-HNSCC.
    • Für Phase II keine vorherige systemische Therapie für RM-HNSCC.
  • Mindestens 18 Jahre alt.

    • Protein-Kreatinin-Verhältnis im Urin (UPC) ≤ 1; wenn UPC ≥ 1, dann muss ein 24-Stunden-Urin-Protein bewertet werden; Patienten müssen einen Proteinwert im 24-Stunden-Urin von < 1 g haben, um in Frage zu kommen
    • INR ≤ 1,5 x ULN (≤ 3,0 x ULN bei Antikoagulation) und PTT ≤ 1,5 x ULN (
  • Frauen im gebärfähigen Alter und Männer müssen damit einverstanden sein, ab 14 Tage vor der ersten Ramucirumab-Dosis, während des Dosierungszeitraums und für mindestens 28 Tage danach eine angemessene Empfängnisverhütung (hormonelle oder Barrieremethode zur Empfängnisverhütung, Abstinenz) anzuwenden.
  • Unterschriebenes, vom IRB genehmigtes schriftliches Einwilligungsdokument.

Ausschlusskriterien:

  • Phase II: vorheriger PD-1-Inhibitor zur Behandlung von unheilbarem HNSCC. Für Phase I ist eine vorherige PD-1-Hemmer-Therapie im unheilbaren Setting zulässig.
  • Bestrahlung, Chemotherapie, zielgerichtete oder Prüftherapie innerhalb von 14 Tagen nach Behandlungsbeginn.
  • Größere Operation, Vorhandensein eines nicht heilenden, nicht bösartigen Geschwürs innerhalb von 14 Tagen nach Behandlungsbeginn; Vorgeschichte signifikanter Blutungen an der Tumorstelle innerhalb von 14 Tagen nach Studieneinwilligung.
  • Vorgeschichte anderer bösartiger Erkrankungen ≤ 1 Jahr mit Ausnahme von vollständig reseziertem Hautkarzinom oder anderen Krebsarten mit geringem Rezidivrisiko.
  • Zirrhose auf einem Niveau von Child-Pugh B (oder schlimmer), Zirrhose jeglichen Grades mit einer Vorgeschichte von hepatischer Enzephalopathie oder klinisch bedeutsamem Aszites (von einer Zirrhose, die Diuretika oder Parazentese erfordert).
  • Erhalt anderer Ermittlungsagenten.
  • Anhaltende Toxizität zurückzuführen auf eine vorherige Krebstherapie, die > Grad 1 ist, außer Alopezie, Anämie, Müdigkeit oder Hautausschlag.
  • Aktive Metastasen des Zentralnervensystems: definiert als derzeitige Strahlentherapie gegen metastasierende ZNS-Erkrankung. Nach Abschluss der Strahlentherapie sind Patienten mit ZNS-Erkrankungen berechtigt, wenn sie alle anderen Kriterien für die Aufnahme erfüllen.

  • Hat eine aktive Autoimmunerkrankung (z. rheumatoide Arthritis, Lupus, Sjögren-Syndrom), das in den letzten 6 Monaten eine intravenöse oder subkutane systemische Behandlung erforderte (ausgenommen Rituxin). Substitutionstherapie (d.h.
