Ziele: Das primäre Ziel der APIXBRAIN-HF-Studie ist die Erforschung der Auswirkungen von Apixaban auf den Schutz des Gehirns bei Patienten mit Sinusrhythmus und Herzinsuffizienz

- Sekundärer Endpunkt 1) Die durch MMSE-2 bewertete Veränderung der kognitiven Funktion. 2) Individuelle Variable des primären Endpunkts
- Bewertung der Sicherheitsendpunkte 1) Zerebrale Mikroblutungen in der Bildgebung

Beschreibung
Einschlusskriterien:
- Patienten im Alter von über 19 Jahren
- Patienten mit Sinusrhythmus
Neu diagnostizierte Herzinsuffizienz oder verschlimmerte Herzinsuffizienzsymptome
- Dyspnoe (≥ NYHA II)
- Plasma-BNP ≥ 200 pg/ml oder NT-proBNP ≥ 800 pg/ml
Systolische LV-Dysfunktion in der Echokardiographie innerhalb von 3 Monaten nach Einschreibung
1) LVEF ≤ 40 %
- Diejenigen mit CHA2DS2-VASc ≥ 3
- Modifizierter Rankin-Score ≤ 4
Ausschlusskriterien:
- Patienten, die bereits eine Therapie mit Warfarin oder Thrombozytenaggregationshemmern abonniert haben oder eine klare Indikation für eine Therapie mit Warfarin oder Thrombozytenaggregationshemmern haben
- Bei hohem Blutungsrisiko
- Patienten mit Vorhofflimmern
- Geschätzte glomeruläre Filtrationsrate (CKD-EPI-Formel) < 15 ml/min/1,73 m2)
- Kürzlicher Schlaganfall oder Gehirnblutung (innerhalb von 3 Monaten)
- Patienten, bei denen ein Myokardinfarkt diagnostiziert wurde oder die bei der Einschreibung eine PCI/CABG planen
- Herzinsuffizienz im Endstadium mit einer Lebenserwartung von ≤ 6 Monaten
- Patienten mit Bettlägerigkeit (Modified Rankin Score ≥ 5)
- Patienten mit Leberfunktionsstörungen (AST, ALT > 2-fache der oberen Normalgrenzen oder Gesamtbilirubin > 1,5 der oberen Normalgrenzen)
- Bei Schwangerschaft oder Stillzeit
- Patienten, die mit der Anwendung einer medizinisch akzeptablen Empfängnisverhütung während des klinischen Studienzeitraums nicht einverstanden sind
- Patienten mit Kontraindikation für Apixaban
