Pilottitutkimus telmisartaanista (Micardis) akuutin siirrännäisen vs. isäntätaudin ehkäisemiseksi allogeenisen hematopoieettisen kantasolusiirron jälkeen
Tämä on yhden keskuksen avoin, prospektiivinen tutkimus telmisartaanista akuutin GVHD:n ehkäisyyn noin 60 potilaalla, joille tehdään allogeeninen HCT hematologisen pahanlaatuisen kasvaimen hoitoon.

Yksityiskohtainen kuvaus
Tämä on yhden keskuksen avoin, prospektiivinen tutkimus telmisartaanista akuutin GVHD:n ehkäisyyn noin 60 potilaalla, joille tehdään allogeeninen HCT hematologisen pahanlaatuisen kasvaimen hoitoon.
Koehenkilöt saavat 160 mg Micardis-telmisartaania kerran päivässä alkaen 2 päivää ennen HCT:tä (päivä -2).
Kun potilas on kotiutettu HCT:n jälkeen, hoitoa jatketaan päivään +98 HCT:n jälkeen yhteensä 101 päivää.
Hoidon lopettamisen jälkeen päivänä +98 HCT:n jälkeen tai ennen sitä, koehenkilöitä seurataan enintään 6 kuukauden ajan primaaristen ja toissijaisten päätetapahtumien osalta.

Sisällyttämiskriteerit:
Diagnoosi:
- Akuutti myelooinen tai lymfoidinen leukemia remissiossa,
- myelodysplastinen oireyhtymä,
- Krooninen lymfaattinen leukemia,
- Non-Hodgkin-lymfooma,
- Hodgkin-lymfooma,
- Krooninen myelooinen leukemia kroonisessa tai kiihtyvässä vaiheessa,
- Myeloproliferatiivinen häiriö tai
- Multippeli myelooma
- Allogeeninen HSC-siirto sukulaiselta tai ei-sukulaiselta luovuttajalta, joka vastasi vähintään 7/8 HLA-A-, -B-, -C- ja DR-lokuksista ("8/8" tai "7/8")
Jolle tehdään allogeeninen HSC-siirto myeloablatiivisen TBI-, busulfaani- tai (ei-myeloablatiivisen) melfalaanipohjaisen siirtoa edeltävän hoito-ohjelman jälkeen. Elinsiirto-ohjelmat sisältävät jotakin seuraavista aineista, jotka annetaan yhdessä fludarabiinin tai syklofosfamidin kanssa:
- Busulfaani 130 mg/m2 iv vuorokaudessa x 2 (alennettu intensiteetti) tai 4 päivää
- TBI 150 cGy bid x8 annosta (1200 Gy)
- Melfalaani 140 mg/m2. (Vaikka melfalaani ei ole myeloablatiivinen hoito-ohjelma, se johtaa kliinisesti merkittävään mukosiittiin, ja tätä lääkettä saavat potilaat ovat erittäin kiinnostuneita näiden tietojen analysoinnissa.)
- Mies- tai naispotilas 18 vuotta tai vanhempi
- Karnofskyn suorituskykytila > 70 % siirtoa edeltävän käsittelyn aloitushetkellä
- Transplantaatiospesifinen rinnakkaissairauspistemäärä <5 siirtoa edeltävän käsittelyn aloitushetkellä
- Potilaat, jotka käyttävät verenpainelääkkeitä (mukaan lukien telmisartaani), ovat kelvollisia, mutta potilaan on lopetettava hoito vähintään 48 tuntia ennen ensimmäistä tutkimuslääkitysannosta
- Pystyy antamaan tietoisen suostumuksen ja allekirjoittanut tietoisen suostumuksen lomakkeen
Poissulkemiskriteerit:
- Kyvyttömyys antaa tietoon perustuva suostumus
- Potilaat, joilla on tunnettu sydämen vajaatoiminta, pitkälle edennyt munuaisten vajaatoiminta, joka vaatii munuaisten korvaushoitoa, tai maksan vajaatoiminta, vaikka nämä potilaat eivät todennäköisesti olisi oikeutettuja HCT:hen.
- Potilaat, jotka käyttävät ACE:n estäjiä, kaliumlisää tai spironolaktonia (tai mitä tahansa muuta kaliumia säästävää diureettia), jotka eivät voi lopettaa käyttöä ennen tutkimushoidon aloittamista TAI jotka tarvitsevat runsaasti kaliumia sisältävää ruokavaliota
- Potilas ei pysty lopettamaan nykyistä verenpainelääkitystä lääketieteellisistä tai muista syistä kahteen päivään ennen telmisartaanin aloittamista
- Krooninen oireenmukainen hypotensio, tilavuuden väheneminen.
