Questo è uno studio prospettico monocentrico, in aperto, sul telmisartan per la prevenzione della GVHD acuta in circa 60 soggetti sottoposti a HCT allogenico per il trattamento di una neoplasia ematologica.

Questo è uno studio prospettico monocentrico, in aperto, sul telmisartan per la prevenzione della GVHD acuta in circa 60 soggetti sottoposti a HCT allogenico per il trattamento di una neoplasia ematologica.
I soggetti riceveranno 160 mg di telmisartan a marchio Micardis una volta al giorno, a partire da 2 giorni prima dell'HCT (giorno -2).
Una volta che il paziente è stato dimesso dopo l'HCT, il trattamento continuerà fino al Giorno +98 post-HCT per un totale di 101 giorni.
Dopo l'interruzione del trattamento entro il giorno +98 post-HCT, i soggetti saranno seguiti fino a 6 mesi per gli endpoint primari e secondari.

Criterio di inclusione:
Diagnosi di:
- Leucemia acuta mieloide o linfoide in remissione,
- Sindrome mielodisplasica,
- leucemia linfoide cronica,
- Linfoma non Hodgkin,
- linfoma di Hodgkin,
- Leucemia mieloide cronica in fase cronica o accelerata,
- Disturbo mieloproliferativo, o
- Mieloma multiplo
- Sottoporsi a trapianto allogenico di CSE da un donatore correlato o non correlato almeno 7 su 8 dei loci HLA-A, -B, -C e DR (corrispondenza "8/8" o "7/8")
Sottoposto a trapianto allogenico di HSC dopo un regime di condizionamento pre-trapianto a base di TBI mieloablativo, busulfan o (non mieloablativo) melfalan. I regimi per il trapianto includeranno uno dei seguenti agenti, somministrato in combinazione con fludarabina o ciclofosfamide:
- Busulfan 130 mg/m2 iv al giorno x 2 (intensità ridotta) o 4 giorni
- TBI 150 cGy bid x8 dosi (1200 Gy)
- Melfalan 140 mg/m2. (Sebbene il melfalan non sia un regime mieloablativo, provoca una mucosite clinicamente significativa e i pazienti che ricevono questo farmaco saranno di notevole interesse nell'analisi di questi dati.)
- Paziente maschio o femmina di età pari o superiore a 18 anni
- Karnofsky performance status > 70% al momento dell'inizio del condizionamento pre-trapianto
- Punteggio di comorbilità specifico del trapianto <5 al momento dell'inizio del condizionamento pre-trapianto
- I pazienti che assumono farmaci antipertensivi (incluso telmisartan) sono idonei ma il paziente deve interrompere il trattamento almeno 48 ore prima della prima dose del farmaco in studio
- In grado di fornire il consenso informato e di aver firmato il modulo di consenso informato
Criteri di esclusione:
- Impossibilità di fornire il consenso informato
- Soggetti con insufficienza cardiaca nota, insufficienza renale avanzata che richiede terapia renale sostitutiva o insufficienza epatica sebbene questi pazienti molto probabilmente non sarebbero idonei per HCT.
- Soggetti che assumono ACE-inibitori, integratori di potassio o spironolattone (o qualsiasi altro diuretico risparmiatore di potassio) che non possono interrompere l'uso prima dell'inizio del trattamento in studio OPPURE che richiedono una dieta ad alto contenuto di potassio
- Paziente incapace di interrompere l'attuale terapia antipertensiva per motivi medici o di altro tipo per due giorni prima di iniziare il trattamento con telmisartan
- Ipotensione cronica sintomatica, deplezione di volume.
