Estudo Piloto de Telmisartan (Micardis) para a Prevenção de Doença Aguda do Enxerto vs. Hospedeiro Pós Transplante Alogênico de Células Tronco Hematopoiéticas
Este é um estudo prospectivo de centro único, aberto, de telmisartan para a prevenção de DECH aguda em aproximadamente 60 indivíduos submetidos a TCH alogênico para tratamento de malignidade hematológica.

Este é um estudo prospectivo de centro único, aberto, de telmisartan para a prevenção de DECH aguda em aproximadamente 60 indivíduos submetidos a TCH alogênico para tratamento de malignidade hematológica.
Os indivíduos receberão 160 mg de telmisartan da marca Micardis uma vez ao dia, começando 2 dias antes do HCT (dia -2).
Uma vez que o paciente receba alta pós-HCT, o tratamento continuará até o Dia +98 pós-HCT por um total de 101 dias.
Após a descontinuação do tratamento no ou antes do dia +98 pós-HCT, os indivíduos serão acompanhados por até 6 meses para desfechos primários e secundários.

Critério de inclusão:
Diagnóstico de:
- Leucemia mielóide ou linfóide aguda em remissão,
- Síndrome mielodisplásica,
- Leucemia linfoide crônica,
- linfoma não-Hodgkin,
- Linfoma de Hodgkin,
- Leucemia mielóide crônica em fase crônica ou acelerada,
- Distúrbio mieloproliferativo ou
- Mieloma múltiplo
- Submetidos a transplante alogênico de HSC de um doador aparentado ou não aparentado compatível com pelo menos 7 de 8 dos loci HLA-A, -B, -C e DR (compatibilidade "8/8" ou "7/8")
Submetidos a transplante alogênico de HSC após um regime de condicionamento pré-transplante baseado em TCE mieloablativo, bussulfano ou melfalano (não mieloablativo). Os regimes para transplante incluirão um dos seguintes agentes, administrados em conjunto com fludarabina ou ciclofosfamida:
- Bussulfano 130 mg/m2 iv diariamente x 2 (intensidade reduzida) ou 4 dias
- TBI 150 cGy bid x8 doses (1200 Gy)
- Melfalano 140 mg/m2. (Embora o melfalano não seja um regime mieloablativo, resulta em mucosite clinicamente significativa e os pacientes que recebem esta medicação serão de interesse considerável na análise desses dados.)
- Paciente do sexo masculino ou feminino com idade igual ou superior a 18 anos
- Performance status de Karnofsky > 70% no momento do início do condicionamento pré-transplante
- Pontuação de comorbidade específica do transplante <5 no momento do início do condicionamento pré-transplante
- Os pacientes que tomam medicamentos anti-hipertensivos (incluindo telmisartan) são elegíveis, mas o paciente deve descontinuar o tratamento pelo menos 48 horas antes da primeira dose do medicamento do estudo
- Capaz de dar consentimento informado e ter assinado o formulário de consentimento informado
Critério de exclusão:
- Incapacidade de fornecer consentimento informado
- Indivíduos com insuficiência cardíaca conhecida, insuficiência renal avançada que requer terapia de substituição renal ou insuficiência hepática, embora esses pacientes provavelmente não sejam elegíveis para HCT.
- Indivíduos que tomam inibidores da ECA, suplementos de potássio ou espironolactona (ou qualquer outro diurético poupador de potássio) que não podem interromper o uso antes do início do tratamento do estudo OU que requerem uma dieta rica em potássio
- Paciente incapaz de descontinuar a medicação atual para hipertensão por motivos médicos ou outros dois dias antes de iniciar o telmisartan
- Hipotensão crônica sintomática, depleção de volume.
