Estudio piloto de telmisartán (Micardis) para la prevención de la enfermedad aguda de injerto contra huésped posterior al trasplante alogénico de células madre hematopoyéticas
Este es un estudio prospectivo abierto, de un solo centro, de telmisartán para la prevención de la EICH aguda en aproximadamente 60 sujetos que se sometieron a un TCH alogénico para el tratamiento de una neoplasia maligna hematológica.

Este es un estudio prospectivo abierto, de un solo centro, de telmisartán para la prevención de la EICH aguda en aproximadamente 60 sujetos que se sometieron a un TCH alogénico para el tratamiento de una neoplasia maligna hematológica.
Los sujetos recibirán 160 mg de telmisartán de la marca Micardis una vez al día, comenzando 2 días antes del HCT (día -2).
Una vez que el paciente recibe el alta después del TCH, el tratamiento continuará hasta el Día +98 después del TCH por un total de 101 días.
Después de la interrupción del tratamiento el día +98 después del TCH o antes, se realizará un seguimiento de los sujetos durante un máximo de 6 meses para los criterios de valoración primarios y secundarios.

Criterios de inclusión:
Diagnóstico de:
- Leucemia mieloide o linfoide aguda en remisión,
- Síndrome mielodisplásico,
- leucemia linfoide crónica,
- No linfoma de Hodgkin,
- Linfoma de Hodgkin,
- Leucemia mieloide crónica en fase crónica o acelerada,
- Trastorno mieloproliferativo, o
- Mieloma múltiple
- Someterse a un trasplante alogénico de HSC de un donante emparentado o no emparentado con al menos 7 de 8 de los loci HLA-A, -B, -C y DR (coincidencia "8/8" o "7/8")
Someterse a un trasplante alogénico de HSC después de un régimen de acondicionamiento previo al trasplante basado en TBI mieloablativo, busulfano o (no mieloablativo) melfalán. Los regímenes para el trasplante incluirán uno de los siguientes agentes, administrado junto con fludarabina o ciclofosfamida:
- Busulfán 130 mg/m2 iv diario x 2 (intensidad reducida) o 4 días
- TBI 150 cGy bid x8 dosis (1200 Gy)
- Melfalán 140 mg/m2. (Aunque el melfalán no es un régimen mieloablativo, produce mucositis clínicamente significativa y los pacientes que reciben este medicamento tendrán un interés considerable en el análisis de estos datos).
- Paciente masculino o femenino de 18 años o más
- Estado funcional de Karnofsky > 70 % en el momento del inicio del acondicionamiento previo al trasplante
- Puntuación de comorbilidad específica del trasplante de <5 en el momento del inicio del acondicionamiento previo al trasplante
- Los pacientes que toman medicamentos antihipertensivos (incluido telmisartán) son elegibles, pero el paciente debe interrumpir el tratamiento al menos 48 horas antes de la primera dosis del medicamento del estudio.
- Capaz de dar consentimiento informado y haber firmado el formulario de consentimiento informado
Criterio de exclusión:
- Incapacidad para proporcionar consentimiento informado
- Sujetos con insuficiencia cardíaca conocida, insuficiencia renal avanzada que requiera terapia de reemplazo renal o insuficiencia hepática, aunque es muy probable que estos pacientes no sean elegibles para un TCH.
- Sujetos que toman inhibidores de la ECA, suplementos de potasio o espironolactona (o cualquier otro diurético ahorrador de potasio) que no pueden interrumpir su uso antes de iniciar el tratamiento del estudio O que requieren una dieta rica en potasio
- Paciente incapaz de suspender la medicación actual para la hipertensión por razones médicas o de otro tipo durante dos días antes de comenzar con telmisartán
- Hipotensión sintomática crónica, depleción de volumen.
