Pilotstudie van telmisartan (Micardis) voor de preventie van acute graft-versus-hostziekte na allogene hematopoëtische stamceltransplantatie
Dit is een single-center, open-label, prospectieve studie van telmisartan voor de preventie van acute GVHD bij ongeveer 60 proefpersonen die allogene HCT ondergaan voor de behandeling van een hematologische maligniteit.

Dit is een single-center, open-label, prospectieve studie van telmisartan voor de preventie van acute GVHD bij ongeveer 60 proefpersonen die allogene HCT ondergaan voor de behandeling van een hematologische maligniteit.
Proefpersonen krijgen eenmaal daags 160 mg telmisartan van het merk Micardis, te beginnen 2 dagen voorafgaand aan HCT (dag -2).
Zodra de patiënt na HCT is ontslagen, wordt de behandeling voortgezet tot dag +98 post-HCT gedurende in totaal 101 dagen.
Na stopzetting van de behandeling op of vóór dag +98 post-HCT, zullen proefpersonen gedurende maximaal 6 maanden worden gevolgd voor primaire en secundaire eindpunten.

Inclusiecriteria:
Diagnose van:
- Acute myeloïde of lymfoïde leukemie in remissie,
- Myelodysplastisch syndroom,
- Chronische lymfoïde leukemie,
- Non-Hodgkin lymfoom,
- Hodgkin-lymfoom,
- Chronische myeloïde leukemie in chronische of acceleratiefase,
- Myeloproliferatieve aandoening, of
- Multipel myeloom
- Allogene HSC-transplantatie ondergaan van een verwante of niet-verwante donor die overeenkomt met ten minste 7 van de 8 van de HLA-A-, -B-, -C- en DR-loci ("8/8" of "7/8" match)
Allogene HSC-transplantatie ondergaan na een myeloablatief TBI-, busulfan- of (niet-myeloablatief) pre-transplantatie conditioneringsregime op basis van melfalan. Regimes voor transplantatie omvatten een van de volgende middelen, gegeven in combinatie met fludarabine of cyclofosfamide:
- Busulfan 130 mg/m2 iv dagelijks x 2 (verminderde intensiteit) of 4 dagen
- TBI 150 cGy bid x8 doses (1200 Gy)
- Melfalan 140 mg/m2. (Hoewel melfalan geen myeloablatief regime is, resulteert het in klinisch significante mucositis en patiënten die dit medicijn krijgen, zullen van groot belang zijn bij de analyse van deze gegevens.)
- Mannelijke of vrouwelijke patiënt van 18 jaar of ouder
- Prestatiestatus Karnofsky> 70% op het moment van aanvang van pre-transplantatieconditionering
- Transplantatiespecifieke co-morbiditeitsscore van <5 op het moment van aanvang van pre-transplantatieconditionering
- Patiënten die antihypertensiva gebruiken (waaronder telmisartan) komen in aanmerking, maar de patiënt moet de behandeling ten minste 48 uur vóór de eerste dosis studiemedicatie stopzetten
- In staat om geïnformeerde toestemming te geven en het formulier voor geïnformeerde toestemming te hebben ondertekend
Uitsluitingscriteria:
- Onvermogen om geïnformeerde toestemming te geven
- Proefpersonen met bekend hartfalen, gevorderde nierfunctiestoornis die niervervangende therapie nodig heeft, of leverfalen, hoewel deze patiënten hoogstwaarschijnlijk niet in aanmerking komen voor HCT.
- Proefpersonen die ACE-remmers, kaliumsupplementen of spironolacton (of andere kaliumsparende diuretica) gebruiken en die het gebruik niet kunnen stopzetten voordat de studiebehandeling wordt gestart OF die een kaliumrijk dieet nodig hebben
- Patiënt niet in staat om de huidige hypertensiemedicatie te stoppen om medische of andere redenen gedurende twee dagen voorafgaand aan het starten met telmisartan
- Chronische symptomatische hypotensie, volumedepletie.
