ARRIÈRE-PLAN:
Environ deux millions d'Américains souffrent d'insuffisance cardiaque. Étant donné que la prévalence de l'insuffisance cardiaque congestive est connue pour augmenter avec l'âge, l'amélioration de l'espérance de vie moyenne devrait augmenter l'ampleur du problème au cours des prochaines décennies. Alors que les progrès dans le traitement de l'hypertension, des maladies coronariennes et du traitement chirurgical des cardiopathies congénitales et valvulaires ont prolongé la survie, nombre de ces patients finissent par développer une insuffisance cardiaque plus tard dans la vie.
Le traitement de l'insuffisance cardiaque congestive a subi des changements rapides. Jusqu'au début des années 1970, le pilier du traitement des patients atteints d'insuffisance cardiaque congestive aiguë et chronique était la digitaline et les diurétiques. L'utilisation de la digitaline chez les patients atteints d'insuffisance cardiaque secondaire à une cardiopathie ischémique et chez ceux atteints d'insuffisance ventriculaire gauche sévère d'origine non ischémique ou ischémique était cependant controversée.
La controverse sur les effets aigus et à long terme de la digitaline a conduit à l'introduction d'une variété d'approches thérapeutiques alternatives et complémentaires pour le patient souffrant d'insuffisance cardiaque. Au milieu des années 1980, un large éventail de vasodilatateurs était disponible. Nouveaux, à cette époque, les agents inotropes, ont également trouvé des applications comme alternatives ou compléments à l'administration digitale. Bien que tous ces médicaments se soient avérés bénéfiques dans la prise en charge des patients atteints d'insuffisance cardiaque chronique, il n'y avait pas suffisamment d'informations sur les effets hémodynamiques à long terme de certains de ces agents et pratiquement aucune information concernant leur effet sur la survie.
La reconnaissance que les patients atteints d'insuffisance cardiaque congestive ont souvent une résistance vasculaire périphérique élevée a conduit à l'introduction de la thérapie vasodilatatrice. Parmi les vasodilatateurs, les inhibiteurs de l'enzyme de conversion de l'angiotensine (ECA) semblaient être les plus prometteurs car il avait été démontré qu'ils neutralisaient certains des principaux mécanismes hormonaux et vasoconstricteurs indésirables, soulageaient les symptômes, diminuaient la dilatation cardiaque après un infarctus du myocarde et amélioraient la capacité d'exercice. et la fraction d'éjection.
SOLVD a été lancé en 1986 principalement pour évaluer les effets de l'énalapril, un inhibiteur de l'enzyme de conversion de l'angiotensine (ECA). Deux considérations ont influencé la conception de SOLVD : il était possible que le traitement médicamenteux administré aux patients qui avaient déjà une insuffisance cardiaque congestive manifeste ne soit pas aussi bénéfique que l'initiation du traitement chez les patients asymptomatiques atteints de dysfonction ventriculaire gauche ; un traitement précoce de ces patients asymptomatiques pourrait être moins bénéfique, car le patient pourrait ne pas avoir d'activation mesurable de l'axe rénine-angiotensine.
La demande de propositions a été publiée en août 1984 et les premiers prix ont été attribués en juillet 1985. Une deuxième demande de propositions pour des centres cliniques supplémentaires a été publiée en mars 1985.
NARRATIF DE CONCEPTION :
Les deux essais étaient randomisés et en double aveugle. Avant la randomisation, tous les patients ont reçu de l'énalapril et un placebo pendant deux semaines pour permettre une détection précoce des patients non conformes, de ceux qui ne tolèrent pas le médicament et de ceux qui présentent des effets secondaires précoces. Après la fin de la période de pré-randomisation, les participants ont été affectés à l'énalapril ou au placebo en utilisant une randomisation en blocs permutés dans chacun des centres cliniques. Des visites de suivi ont été programmées pendant deux et six semaines après la randomisation, puis à quatre, huit et douze mois. Par la suite, des visites à la clinique ont été programmées à des intervalles de quatre mois. Les patients ont été suivis pendant un minimum de deux ans et un maximum de cinq ans. Le critère de jugement principal pour chaque essai était la mortalité toutes causes confondues. Les critères d'évaluation secondaires comprenaient la mortalité cardiovasculaire, la mort subite due à l'aggravation de l'insuffisance cardiaque congestive, l'hospitalisation pour insuffisance cardiaque congestive, l'infarctus du myocarde, l'accident vasculaire cérébral, le besoin de transplantation cardiaque et la qualité de vie. L'apparition d'une insuffisance cardiaque congestive était un résultat supplémentaire dans l'essai de prévention. Sept sous-études ont été menées parmi des sous-ensembles de participants et comprenaient : la fonction diastolique ; échocardiographie; exercer; neurohumoral; qualité de vie; radionucléide; mort subite. Le recrutement des patients a commencé en juillet 1986. Le recrutement dans l'essai de traitement s'est terminé six mois plus tôt que prévu en février 1989 et le suivi s'est terminé en février 1991. L'essai de prévention a mis fin au recrutement en avril 1990 et a terminé le suivi en juillet 1991. L'analyse des données s'est poursuivie jusqu'en novembre 1994.
L'essai comprenait également un registre qui a recruté 6 300 patients pour étudier l'influence d'un certain nombre de caractéristiques des patients sur la mortalité. Dix-huit des centres cliniques ont participé. Les données ont été recueillies auprès de patients consécutifs avec une fraction d'éjection inférieure à 0,45 ou avec des signes radiologiques d'insuffisance cardiaque congestive sur une période de douze mois. A un an, le statut vital du patient était renseigné par courrier et, si nécessaire, par un suivi téléphonique. Un suivi d'un an a été effectué sur tous les patients du registre en mai 1990. Un suivi à plus long terme a été effectué grâce à l'Index national des décès.
