ACHTERGROND:
Ongeveer twee miljoen Amerikanen lijden aan hartfalen. Aangezien bekend is dat de prevalentie van congestief hartfalen toeneemt met de leeftijd, wordt verwacht dat verbeteringen in de gemiddelde levensverwachting de omvang van het probleem in de komende decennia zullen vergroten. Hoewel vooruitgang in de behandeling van hypertensie, coronaire hartziekte en chirurgische behandeling van aangeboren en hartklepaandoeningen de overleving hebben verlengd, ontwikkelen veel van deze patiënten uiteindelijk hartfalen op latere leeftijd.
De therapie van congestief hartfalen heeft snelle veranderingen ondergaan. Tot het begin van de jaren zeventig waren digitalis en diuretica de steunpilaren van de therapie voor de patiënt met acuut en chronisch congestief hartfalen. Het gebruik van digitalis bij patiënten met hartfalen secundair aan ischemische hartziekte en bij patiënten met ernstig linkerventrikelfalen van niet-ischemische of ischemische oorsprong was echter controversieel.
Controverse over de acute en langetermijneffecten van digitalis leidde tot de introductie van een verscheidenheid aan alternatieve en aanvullende therapeutische benaderingen voor de patiënt met hartfalen. Halverwege de jaren tachtig was er een breed spectrum aan vaatverwijders beschikbaar. Nieuwe, destijds inotrope middelen, vonden ook toepassingen als alternatief voor of aanvulling op digitalistoediening. Hoewel van al deze geneesmiddelen is aangetoond dat ze enig nut hebben bij de behandeling van patiënten met chronisch hartfalen, was er onvoldoende informatie over de hemodynamische effecten op lange termijn van sommige van deze middelen en praktisch geen informatie over hun effect op de overleving.
De erkenning dat patiënten met congestief hartfalen vaak een verhoogde perifere vasculaire weerstand hebben, leidde tot de introductie van vaatverwijdende therapie. Van de vasodilatatoren leken de angiotensine-converting enzyme (ACE)-remmers de meest veelbelovende, omdat was aangetoond dat ze enkele van de belangrijkste ongunstige hormonale en vasoconstrictieve mechanismen tegengaan, symptomen verlichten, hartdilatatie na een myocardinfarct verminderen en de inspanningscapaciteit verbeteren. en ejectiefractie.
SOLVD werd in 1986 opgericht, voornamelijk om de effecten van enalapril, een angiotensine-converterend enzym (ACE)-remmer, te evalueren. Twee overwegingen waren van invloed op het ontwerp van SOLVD: het was mogelijk dat medicamenteuze behandeling van patiënten die al manifest congestief hartfalen hadden niet zo gunstig zou zijn als het starten van een behandeling bij patiënten met asymptomatische linkerventrikeldisfunctie; vroege behandeling van dergelijke asymptomatische patiënten kan minder gunstig zijn, aangezien de patiënt mogelijk geen meetbare activering van de renine-angiotensine-as heeft.
De Request for Proposals werd uitgebracht in augustus 1984 en de eerste prijzen werden uitgereikt in juli 1985. Een tweede Request for Proposals voor aanvullende klinische centra werd uitgebracht in maart 1985.
ONTWERP VERHAAL:
Beide onderzoeken waren gerandomiseerd en dubbelblind. Voorafgaand aan de randomisatie kregen alle patiënten gedurende twee weken enalapril en placebo om vroegtijdige opsporing mogelijk te maken van patiënten die niet aan de regels voldeden, patiënten die het geneesmiddel niet konden verdragen en patiënten met vroegtijdige bijwerkingen. Na voltooiing van de pre-randomisatieperiode werden de deelnemers toegewezen aan enalapril of placebo met behulp van een gepermuteerde blokrandomisatie binnen elk van de klinische centra. Follow-upbezoeken waren gepland gedurende twee en zes weken na randomisatie en vervolgens na vier, acht en twaalf maanden. Daarna werden polikliniekbezoeken gepland met tussenpozen van vier maanden. Patiënten werden minimaal twee jaar en maximaal vijf jaar gevolgd. Primaire uitkomst voor elke proef was sterfte door alle oorzaken. Secundaire eindpunten waren cardiovasculaire mortaliteit, plotseling overlijden als gevolg van verergering van congestief hartfalen, ziekenhuisopname wegens congestief hartfalen, myocardinfarct, beroerte, behoefte aan harttransplantatie en kwaliteit van leven. Het ontstaan van congestief hartfalen was een bijkomende uitkomst in de Preventiestudie. Er werden zeven deelonderzoeken uitgevoerd onder subgroepen van deelnemers, waaronder: diastolische functie; echocardiografie; oefening; neurohumoraal; kwaliteit van het leven; radionuclide; plotselinge dood. De werving van patiënten begon in juli 1986. De rekrutering voor de Treatment Trial eindigde zes maanden eerder dan gepland in februari 1989 en de follow-up eindigde in februari 1991. De preventieproef beëindigde de rekrutering in april 1990 en voltooide de follow-up in juli 1991. De gegevensanalyse ging door tot november 1994.
De proef omvatte ook een register waarin 6.300 patiënten werden ingeschreven om de invloed van een aantal patiëntkenmerken op de mortaliteit te bestuderen. Achttien van de klinische centra namen deel. Er werden gegevens verzameld van opeenvolgende patiënten met een ejectiefractie van minder dan 0,45 of met radiologisch bewijs van congestief hartfalen gedurende een periode van twaalf maanden. Na een jaar werd de vitale toestand van patiënte per post en eventueel telefonische nacontrole vastgesteld. In mei 1990 was de follow-up van een jaar voltooid bij alle geregistreerde patiënten. Follow-up op langere termijn vond plaats via de National Death Index.
