BAKGRUNN:
Omtrent to millioner amerikanere lider av hjertesvikt. Siden forekomsten av kongestiv hjertesvikt er kjent for å øke med alderen, vil forbedringer i gjennomsnittlig levealder forventes å øke omfanget av problemet i løpet av de neste tiårene. Mens fremskritt innen behandling av hypertensjon, koronararteriesykdom og kirurgisk behandling av medfødt og valvulær hjertesykdom har forlenget overlevelsen, utvikler mange av disse pasientene til slutt hjertesvikt senere i livet.
Behandlingen av kongestiv hjertesvikt har gjennomgått rask endring. Fram til tidlig på 1970-tallet var bærebjelken i behandlingen for pasienten med akutt og kronisk kongestiv hjertesvikt digitalis og diuretika. Bruken av digitalis hos pasienter med hjertesvikt sekundært til iskemisk hjertesykdom og hos de med alvorlig venstre ventrikkelsvikt av enten ikke-iskemisk eller iskemisk opprinnelse var imidlertid kontroversiell.
Kontrovers om de akutte og langsiktige effektene av digitalis førte til introduksjonen av en rekke alternative og supplerende terapeutiske tilnærminger for pasienten med hjertesvikt. På midten av 1980-tallet var et bredt spekter av vasodilatorer tilgjengelig. Nye, på den tiden, inotrope midler fant også anvendelser som alternativer eller supplementer til digitalisadministrasjon. Selv om alle disse legemidlene hadde vist seg å være til en viss fordel i behandlingen av pasienter med kronisk hjertesvikt, var det mangelfull informasjon om de langsiktige hemodynamiske effektene av noen av disse midlene og praktisk talt ingen informasjon om deres effekt på overlevelse.
Erkjennelsen av at pasienter med kongestiv hjertesvikt ofte har forhøyet perifer vaskulær motstand førte til innføring av vasodilatorterapi. Av vasodilatorene så angiotensin-konverterende enzym (ACE)-hemmere ut til å være de mest lovende fordi de hadde vist seg å motvirke noen av de viktigste uønskede hormonelle og vasokonstriktor-mekanismene, lindre symptomer, redusere hjertedilatasjon etter hjerteinfarkt og forbedre treningskapasiteten. og ejeksjonsfraksjon.
SOLVD ble startet i 1986 primært for å evaluere effekten av enalapril, en angiotensin-konverterende enzym (ACE)-hemmer. To hensyn påvirket utformingen av SOLVD: det var mulig at medikamentell behandling gitt til pasienter som allerede hadde manifest kongestiv hjertesvikt kanskje ikke var like fordelaktig som å starte behandling hos pasienter med asymptomatisk venstre ventrikkel dysfunksjon; Tidlig behandling av slike asymptomatiske pasienter kan være mindre fordelaktig, da pasienten kanskje ikke har målbar aktivering av renin-angiotensin-aksen.
Forespørselen om forslag ble utgitt i august 1984 med de første tildelingene i juli 1985. En annen forespørsel om forslag til ytterligere kliniske sentre ble utgitt i mars 1985.
DESIGN NARRATIV:
Begge studiene var randomiserte og dobbeltblinde. Før randomisering fikk alle pasienter enalapril og placebo i to uker for å muliggjøre tidlig oppdagelse av ikke-kompatible pasienter, de som ikke kunne tolerere stoffet og de med tidlige bivirkninger. Etter fullføring av pre-randomiseringsperioden ble deltakerne allokert til enalapril eller placebo ved å bruke en permutert blokk-randomisering innen hvert av de kliniske sentrene. Oppfølgingsbesøk ble planlagt i to og seks uker etter randomisering og deretter ved fire, åtte og tolv måneder. Deretter ble det planlagt klinikkbesøk med fire måneders mellomrom. Pasientene ble fulgt i minimum to år og maksimalt fem år. Primært utfall for hvert forsøk var dødelighet av alle årsaker. Sekundære endepunkter inkluderte kardiovaskulær dødelighet, plutselig død på grunn av forverret kongestiv hjertesvikt, sykehusinnleggelse for kongestiv hjertesvikt, hjerteinfarkt, hjerneslag, behov for hjertetransplantasjon og livskvalitet. Utbruddet av kongestiv hjertesvikt var et tilleggsresultat i forebyggingsforsøket. Syv delstudier ble utført blant undergrupper av deltakere og inkluderte: diastolisk funksjon; ekkokardiografi; trening; nevrohumoral; livskvalitet; radionuklid; plutselig død. Pasientrekruttering begynte i juli 1986. Rekrutteringen i behandlingsforsøket ble avsluttet seks måneder før skjema i februar 1989, og oppfølgingen ble avsluttet i februar 1991. Forebyggingsforsøket avsluttet rekrutteringen i april 1990 og fullførte oppfølgingen i juli 1991. Dataanalysen fortsatte gjennom november 1994.
Forsøket inkluderte også et register som inkluderte 6300 pasienter for å studere påvirkningen av en rekke pasientkarakteristikker på dødelighet. Atten av de kliniske sentrene deltok. Data ble samlet inn fra påfølgende pasienter med ejeksjonsfraksjon mindre enn 0,45 eller med radiologiske tegn på kongestiv hjertesvikt over en tolv måneders periode. Etter ett år ble pasientens vitale status konstatert per post og om nødvendig telefonoppfølging. Ett års oppfølging ble fullført på alle registerpasienter innen mai 1990. Langsiktig oppfølging ble gjort gjennom National Death Index.
