LINFOTARGAM : traitement de première intention par chimiothérapie à haute dose plus rituximab (R-CHOP/14) et thérapie antirétrovirale hautement active (HAART) chez les patients atteints de lymphome diffus à grandes cellules B (DLBCL) et d'infection par le virus de l'immunodéficience humaine (VIH)
Objectif principal:
Pour évaluer l'applicabilité du traitement :
- Évaluer la toxicité du traitement selon les Common Terminology Criteria (CTC) version 3.0 du National Cancer Institute (NCI).
- Évaluer les infections opportunistes et non opportunistes après 6 cycles de traitement par rituximab plus cyclophosphamide, doxorubicine, vincristine et prednisone (R-CHOP) administrés tous les 14 jours et traitement antirétroviral hautement actif (HAART) chez des patients atteints de lymphome diffus à grandes cellules B ( DLBCL) et infection par le VIH.
- Évaluer l'adhésion au traitement avec 6 cycles de R-CHOP compte tenu des délais d'administration des cycles et des réductions des doses de chimiothérapie (dose planifiée administrée dans le délai prévu).
Objectifs secondaires :
Pour évaluer l'efficacité du traitement chez les patients atteints de LDGCB et infectés par le VIH après 6 cycles de traitement par R-CHOP administré tous les 14 jours (R-CHOP/14) :
- Pour déterminer la réponse globale et la remise complète de l'impôt.
- Pour évaluer la durée de la réponse.
- Évaluer la probabilité de survie sans événement à 5 ans.
- Évaluer la probabilité de survie globale dans 5 ans.
- Identifier les facteurs prédictifs de réponse après 6 cycles de traitement par R-CHOP administré tous les 14 jours chez des patients atteints de LDGCB et infectés par le VIH.
- Évaluer l'impact de l'association thérapeutique R-CHOP et HAART sur les paramètres de l'infection par le VIH (charge virale VIH et numération des lymphocytes CD4+).

Critère d'intégration:
- Patients infectés par le VIH diagnostiqués avec un DLBCL à n'importe quel stade (I-IV selon la classification d'Ann Arbor) non traités auparavant pour le lymphome.
- Patients atteints de lymphome diffus à grandes cellules B CD20 positif
- De 18 à 70 ans
- Tout score de l'index pronostique international. (Il s'applique également aux patients atteints d'un lymphome non hodgkinien [LNH] infectés par le VIH.)
- Statut de performance ECOG 0 à 3
- Consentement éclairé écrit
- Nombre absolu de neutrophiles > 1,5 x 10^9/L.
- Absence de néoplasie synchronique ou non synchronique à l'exception des tumeurs cutanées non mélaniques ou du carcinome cervical in situ.
- Numération des lymphocytes CD4+ > 100/µL
Critère d'exclusion:
- Patients atteints de lymphome diffus à grandes cellules B précédemment traités.
- Patients atteints d'un lymphome primitif du système nerveux central.
- Patients atteints de LNH de Burkitt ou de type Burkitt.
- Numération des lymphocytes CD4+ < 100/µL
- Infections opportunistes ou autres néoplasies liées au SIDA en activité.
- Toxicomanie active.
- Femmes enceintes ou allaitantes ou adultes en âge de procréer qui n'utilisent pas de méthode contraceptive efficace.
- Patients présentant une altération grave de la fonction rénale (créatinine > 2,5 x limite supérieure de la normale [LSN]) ou hépatique [bilirubine, ALT ou AST > 2,5 x LSN], sauf si les investigateurs soupçonnent qu'elles sont causées par la maladie.
- Insuffisance cardiaque avec fraction d'éjection < 40%
- Patients atteints de maladies psychiatriques graves pouvant interférer avec leur capacité à comprendre l'étude (y compris l'alcoolisme ou la toxicomanie active).
- ECOG > 3
- Patients présentant une hypersensibilité connue aux protéines murines ou à tout autre composant des médicaments à l'étude.
