LINFOTARGAM: eerstelijnsbehandeling met dosisdichte chemotherapie plus rituximab (R-CHOP/14) en zeer actieve antiretrovirale therapie (HAART) bij patiënten met diffuus grootcellig B-cellymfoom (DLBCL) en infectie met het humaan immunodeficiëntievirus (HIV)
Hoofddoel:
Om de toepasbaarheid van de behandeling te beoordelen:
- Om de behandelingstoxiciteit te evalueren volgens de Common Terminology Criteria (CTC) versie 3.0 van het National Cancer Institute (NCI).
- Evaluatie van opportunistische en niet-opportunistische infecties na 6 behandelingscycli met rituximab plus cyclofosfamide, doxorubicine, vincristine en prednison (R-CHOP) elke 14 dagen toegediend en zeer actieve antiretrovirale therapie (HAART) bij patiënten met diffuus grootcellig B-cellymfoom ( DLBCL) en hiv-infectie.
- Om de therapietrouw te evalueren met 6 cycli R-CHOP, rekening houdend met de vertragingen in de toediening van de cycli en de verlagingen van de doses chemotherapie (geplande dosis toegediend op voorspelde termijn).
Secundaire doelstellingen:
Om de werkzaamheid van de behandeling te evalueren bij patiënten met DLBCL en HIV-infectie na 6 behandelingscycli met R-CHOP elke 14 dagen toegediend (R-CHOP/14):
- Om de globale respons te bepalen en de kwijtscheldingsbelasting te voltooien.
- Om de duur van het antwoord te evalueren.
- Om de waarschijnlijkheid van gebeurtenisvrije overleving in 5 jaar te evalueren.
- Om de waarschijnlijkheid van wereldwijde overleving in 5 jaar te evalueren.
- Om voorspellende responsfactoren te identificeren na 6 behandelingscycli met R-CHOP elke 14 dagen toegediend bij patiënten met DLBCL en HIV-infectie.
- Evalueren van de impact van de therapeutische combinatie van R-CHOP en HAART op de parameters van de hiv-infectie (hiv-virusbelasting en aantal CD4+-lymfocyten).

Beschrijving
Inclusiecriteria:
- Patiënten met een HIV-infectie met de diagnose DLBCL in elk stadium (I-IV volgens de Ann Arbor-classificatie) die niet eerder zijn behandeld voor het lymfoom.
- Patiënten met CD20-positief diffuus grootcellig B-cellymfoom
- Leeftijd van 18 tot 70 jaar
- Elke score van International Prognostic Index. (Het is ook van toepassing bij patiënten met non-Hodgkin-lymfoom [NHL] die zijn geïnfecteerd met hiv.)
- ECOG-prestatiestatus 0 tot 3
- Schriftelijke geïnformeerde toestemming
- Absoluut aantal neutrofielen > 1,5 x 10^9/L.
- Afwezigheid van synchrone of niet-synchrone neoplasie, met uitzondering van niet-melanome huidtumoren of in situ cervicaal carcinoom.
- Aantal CD4+-lymfocyten > 100/µL
Uitsluitingscriteria:
- Eerder behandelde patiënten met diffuus grootcellig B-cellymfoom.
- Patiënten met primair lymfoom van het centrale zenuwstelsel.
- Patiënten met Burkitt of Burkitt-achtige NHL.
- Aantal CD4+-lymfocyten < 100/µL
- Opportunistische infecties of andere AIDS-gerelateerde neoplasieën in activiteit.
- Actieve drugsverslaving.
- Zwangere of zogende vrouwen of volwassenen in de vruchtbare leeftijd die geen effectieve anticonceptiemethode gebruiken.
- Patiënten met een ernstig veranderde nierfunctie (creatinine > 2,5 x bovengrens van normaal [ULN]) of hepatisch [bilirubine, ALAT of ASAT > 2,5 x ULN], behalve als de onderzoekers vermoeden dat ze door de ziekte worden veroorzaakt.
- Hartinsufficiëntie met ejectiefractie < 40%
- Patiënten met ernstige psychiatrische aandoeningen die hun vermogen om het onderzoek te begrijpen kunnen verstoren (waaronder alcoholisme of actieve drugsverslaving).
- ECOG > 3
- Patiënten met een bekende overgevoeligheid voor muriene eiwitten of een ander bestanddeel van de onderzoeksgeneesmiddelen.
