OBJECTIFS : I. Comparer le contrôle locorégional, la survie sans maladie et la survie globale chez les patients atteints d'un carcinome de la prostate qui sont considérés à haut risque de rechute et reçoivent du zoladex adjuvant à long terme (ZDX) par rapport à l'absence de traitement adjuvant après néoadjuvant ZDX/ flutamide et radiothérapie. II. Comparez la fonction sexuelle chez les patients traités avec ces deux régimes.
APERÇU : Étude randomisée. Groupe I : Thérapie antiandrogène néoadjuvante et thérapie par agonistes des facteurs de libération plus radiothérapie. Flutamide, FLUT, NSC-147834 ; et Zoladex, ZDX, NSC-606864; ainsi que l'irradiation par faisceau externe utilisant des équipements à mégatension avec des énergies d'au moins 4 MV (au moins 6 MV de préférence). Bras II : thérapie antiandrogène néoadjuvante et thérapie par agoniste des facteurs de libération plus radiothérapie suivie d'une thérapie adjuvante par agonistes des facteurs de libération. FLUT ; et ZDX ; plus irradiation par faisceau externe à l'aide d'un équipement comme dans le bras I ; suivi de ZDX.
RECUL PROJETÉ : 1 508 patients seront comptabilisés sur 2 ans.

CARACTÉRISTIQUES DE LA MALADIE : Adénocarcinome de la prostate localement avancé confirmé histologiquement, y compris : Tumeurs primaires volumineuses confinées à la prostate (stade clinique T2c) Tumeurs primaires s'étendant au-delà de la capsule (stade clinique T3-4) Aucune atteinte commune des ganglions iliaques ou para-aortiques Régionale atteinte des ganglions lymphatiques en dessous du niveau iliaque commun autorisée Les ganglions positifs sur les études d'imagerie doivent être biopsiés par FNA ou prélèvement chirurgical PSA pas plus de 150 (obligatoire) Pas de métastases à distance
