UM ENSAIO DE FASE III DO USO DE SUPRESSÃO ANDROGÊNICA TOTAL DE LONGO PRAZO APÓS CITOREDUÇÃO HORMONAL NEOADJUVANTE E RADIOTERAPIA EM CARCINOMA DE PRÓSTATA LOCALMENTE AVANÇADO

OBJETIVOS: I. Comparar o controle locorregional, a sobrevida livre de doença e a sobrevida global em pacientes com carcinoma da próstata considerados de alto risco de recidiva e que recebem adjuvante de longo prazo com zoladex (ZDX) versus nenhum tratamento adjuvante após ZDX/neoadjuvante flutamida e radioterapia. II. Compare a função sexual em pacientes tratados com esses dois regimes.
DESCRIÇÃO: Estudo randomizado. Braço I: Terapia Antiandrogênica Neoadjuvante e Terapia com Agonistas do Fator de Liberação mais Radioterapia. Flutamida, FLUT, NSC-147834; e Zoladex, ZDX, NSC-606864; mais irradiação de feixe externo usando equipamento de megavoltagem com energias de pelo menos 4 MV (pelo menos 6 MV de preferência). Braço II: Terapia Antiandrogênica Neoadjuvante e Terapia com Agonistas do Fator de Liberação mais Radioterapia seguida de Terapia com Agonistas do Fator de Liberação Adjuvante. FLUTA; e ZDX; mais irradiação de feixe externo usando equipamento como no Braço I; seguido por ZDX.
ACRESCIMENTO PROJETADO: 1.508 pacientes serão acumulados ao longo de 2 anos.

CARACTERÍSTICAS DA DOENÇA: Adenocarcinoma da próstata localmente avançado, histologicamente confirmado, incluindo: Tumores primários volumosos confinados à próstata (Estágio clínico T2c) Tumores primários que se estendem além da cápsula (Estágio clínico T3-4) Sem envolvimento nodal ilíaco ou para-aórtico comum Regional envolvimento do linfonodo abaixo do nível ilíaco comum permitido linfonodos positivos em estudos de imagem devem ser biopsiados por FNA ou amostragem cirúrgica PSA não superior a 150 (obrigatório) Sem metástases distantes
