MÅL: I. Sammenlign lokoregional kontroll, sykdomsfri overlevelse og total overlevelse hos pasienter med karsinom i prostata som anses å ha høy risiko for tilbakefall og får langtidsadjuvant zoladex (ZDX) vs. ingen adjuvant behandling etter neoadjuvant ZDX/ flutamid og strålebehandling. II. Sammenlign seksuell funksjon hos pasienter behandlet med disse to regimene.
OVERSIKT: Randomisert studie. Arm I: Neoadjuvant antiandrogenterapi og frigjørende faktor agonistterapi pluss strålebehandling. Flutamid, FLUT, NSC-147834; og Zoladex, ZDX, NSC-606864; pluss ekstern strålebestråling ved bruk av megaspenningsutstyr med energier på minst 4 MV (minst 6 MV foretrekkes). Arm II: Neoadjuvant antiandrogenterapi og frigjørende faktor agonistterapi pluss strålebehandling etterfulgt av adjuvant frigjørende faktor agonistterapi. FLUT; og ZDX; pluss ekstern strålebestråling ved bruk av utstyr som i arm I; etterfulgt av ZDX.
PROSJERT PÅSLUTNING: 1508 pasienter vil påløpe over 2 år.

SYKDOMS KARAKTERISTIKA: Histologisk bekreftet, lokalt avansert adenokarsinom i prostata, inkludert: voluminøse primære svulster begrenset til prostata (klinisk stadium T2c) Primære svulster som strekker seg utover kapselen (klinisk stadium T3-4) Ingen vanlig involvering av iliaca eller para-aorta nodal lymfeknuteinvolvering under det vanlige iliaca-nivået tillatt. Positive noder på bildestudier må biopsieres av FNA eller kirurgisk prøvetaking PSA ikke mer enn 150 (obligatorisk) Ingen fjernmetastaser
