Prédisposition génétique de l'hôte à la grippe aviaire A/H5N1
Prédisposition génétique de l'hôte à la grippe aviaire A/H5N1 au Vietnam, en Thaïlande, en Indonésie et à Singapour
L'identification et la caractérisation des locus de susceptibilité à l'infection par le H5N1 chez l'homme pourraient avoir de profondes implications. La détection des facteurs génétiques de l'hôte peut faire la lumière sur les principales interactions pathogènes entre le H5N1 et les cellules humaines, aidant à identifier les caractéristiques virales déterminant le potentiel pandémique. De plus, l'identification et la vérification des loci de susceptibilité seraient suivies d'études fonctionnelles qui pourraient ouvrir la voie à de nouvelles options thérapeutiques et préventives.
L'objectif de cette étude est d'étudier si les facteurs génétiques de l'hôte sont associés à la susceptibilité à la grippe H5N1

Description détaillée
Il s'agit principalement d'un exercice de sélection d'hypothèses et bien que des suppositions basées sur la plausibilité biologique soient possibles, une analyse complète de la susceptibilité génétique ne peut être réalisée que par une approche du génome entier. Par conséquent, nous testerons la liaison génétique et l'association en utilisant un grand nombre de polymorphismes de nucléotides uniques (SNP) couvrant l'ensemble du génome.
Les chercheurs principaux ont accès aux informations sur tous les cas de H5N1 confirmés en laboratoire survivants et décédés. Ces cas formeront la base de l'étude. L'épidémiologiste tentera d'établir un contact téléphonique avec le patient récupéré ou, si le patient est décédé, le parent le plus proche du patient décédé. L'étude proposée sera expliquée et l'autorisation sera demandée à l'épidémiologiste et à une infirmière de l'étude de visiter le ménage pour expliquer l'étude plus en détail, offrir une possibilité de questions et, si le sujet accepte, d'obtenir un consentement éclairé pour la participation. Si l'épidémiologiste a des difficultés à entrer en contact avec le cas ou sa famille, les autorités publiques locales seront sollicitées pour aider à la prise de contact. Les membres éligibles de la famille des cas de H5N1 seront identifiés par des entretiens en face à face avec les cas confirmés survivants ou des membres proches de la famille des cas décédés.

- les cas symptomatiques avec une infection H5N1 confirmée en laboratoire pourront être inclus.
- La priorité de recrutement sera donnée aux cas issus de familles multicas afin de maximiser la probabilité d'inclure des cas d'étiologie génétique.

Critère d'intégration:
- La personne avait une maladie cliniquement compatible (définie comme des symptômes respiratoires et une radiographie pulmonaire anormale ou une encéphalite), ET
- ARN de la grippe A/H5 identifié dans un échantillon clinique par transcription inverse-amplification en chaîne par polymérase [RT-PCR], OU
- Influenza A/H5 cultivée à partir d'un échantillon clinique, OU
- Titres élevés (1:80 ou plus) d'anticorps anti-H5 par microneutralisation dans des échantillons convalescents de patients atteints d'une maladie compatible mais pas de RT-PCR ou négative, ET
- Le patient a survécu ou s'il est décédé, un échantillon biologique archivé est disponible pour une éventuelle extraction d'ADN, ET
- Un consentement valide et écrit est obtenu.
- Les parents biologiques et les frères et sœurs biologiques de tous les cas et tout autre membre de la famille reliant deux cas, ET
- Un consentement valide et écrit est obtenu.
