Étude randomisée de phase 2 comparant les réponses pathologiques sur les métastases du cancer colorectal après un traitement préopératoire associant le bevacizumab au FOLFOX ou au FOLFIRI
L'étude est conçue pour analyser la réponse tumorale pathologique sur les métastases de cancer colorectal réséquées après un traitement préopératoire avec le bevacizumab combiné avec le régime FOLFOX ou FOLFIRI dans une cohorte prospective et pour corréler cette réponse avec les résultats du patient.

Il s'agit d'une étude de phase II, ouverte, randomisée chez des patients avec un diagnostic confirmé d'adénocarcinome colorectal métastatique résécable, qui n'ont pas reçu de chimiothérapie antérieure pour leur maladie métastatique.
L'étude est conçue pour comparer les réponses pathologiques observées après une chimiothérapie préopératoire bevacizumab avec FOLFOX ou FOLFIRI.

Critère d'intégration:
- 1. Patients de sexe féminin ou masculin âgés d'au moins 18 ans au moment de la signature du consentement éclairé
- 2. Statut de performance ECOG (Eastern Cooperative Oncology Group) 0 ou 1
- 3. Diagnostic histologique ou cytologique confirmé d'adénocarcinome du côlon ou du rectum, avec ou sans tumeur primitive in situ. Statut de la tumeur KRAS de type sauvage ou muté.
- 4.Les patients doivent présenter une maladie métastatique résécable pour laquelle la décision d'une chimiothérapie préopératoire est envisagée. La résécabilité peut être planifiée en un ou plusieurs temps si indiqué. Comme communément admis, la résécabilité signifie la clairance chirurgicale (+/- ablation par radiofréquence) de toutes les lésions détectables (foie) avec des marges sans tumeur et compatibles avec une réserve hépatique adéquate. Pratiquement, la localisation tumorale bilatérale, le nombre et la localisation des lésions, et une réserve hépatique insuffisante restent les principaux facteurs décisionnels.
- 5. La résection partielle et mineure de la maladie métastatique est autorisée dans les 3 mois précédant l'inclusion si le patient n'a jamais reçu de chimiothérapie pour le mCRC.
- 6. L'emplacement métastatique extra-hépatique est limité à 1 site. La localisation extra-hépatique doit être facilement résécable en un temps chirurgical.
- 7.Les patients peuvent avoir reçu une chimiothérapie adjuvante ou une (néo-)chimio-radiothérapie adjuvante du bassin, à condition que la dernière dose de chimiothérapie ait été administrée au moins 6 mois avant l'inclusion (12 mois pour l'oxaliplatine). Une radiothérapie antérieure du bassin n'est pas un critère d'exclusion.
- 8.Fonction hématologique, rénale et hépatique adéquate comme suit : Neutrophiles hématologiques > 1,5 x 109/L Plaquettes > 100 x 109/L Créatinine rénale < 1,5 x LSN (limite supérieure de la normale) Bilirubine hépatique < 1,5 x LSN AST (Aspartate aminotransférase) , ALT (Alanine Aminotransférase) < 5 x LSN Phos Alc. < 5 x LSN
- 9.Protéinurie <2+ (analyse d'urine sur bandelette) ou =1g/24hour.
- 10.Aucun antécédent d'infarctus du myocarde et/ou d'AVC dans les 6 mois précédant la randomisation. Pas d'hypertension non contrôlée (définie comme une pression artérielle systolique > 150 mmHg et/ou une pression artérielle diastolique > 100 mmHg), ou antécédent de crise hypertensive ou d'encéphalopathie hypertensive.
- 11.Les patientes doivent être ménopausées, stériles (chirurgicalement, radio-induites ou induites par des produits chimiques) ou sexuellement actives en utilisant une méthode de contraception acceptable.
- 12.Les patients masculins doivent être chirurgicalement stériles ou s'ils sont sexuellement actifs et avoir un partenaire pré-ménopausique doivent utiliser une méthode de contraception acceptable.
- 13.Espérance de vie d'au moins 3 mois sans aucun traitement actif.
Critère d'exclusion:
- 1. CCRm non résécable (cancer colorectal métastatique) (si la résécabilité reste incertaine ou improbable après 3 mois de chimiothérapie, le patient est exclu de l'essai).
- 2. Chimiothérapie antérieure ou thérapie systémique pour le mCRC. La chimiothérapie adjuvante du cancer colorectal n'est pas un critère d'exclusion à condition qu'elle ait été réalisée plus de 6 mois avant l'inclusion. La chimiothérapie à base d'oxaliplatine doit être terminée depuis plus d'un an avant l'inclusion.
- 3. Utilisation antérieure du bevacizumab, de l'aflibercept (ou d'un autre traitement anti-VEGF (facteur de croissance de l'endothélium vasculaire)).
- 4. Radiothérapie antérieure administrée au haut de l'abdomen.
- 5. Preuve d'ascite, de cirrhose, d'hypertension portale, d'atteinte tumorale de la veine porte principale ou de thrombose, déterminée par une évaluation clinique ou radiologique.
- 6.Résection hépatique majeure antérieure : foie résiduel < 50 % du volume initial du foie.
- 7. Maladie non maligne qui rendrait le patient inapte au traitement selon ce protocole.
- 8.Métastases concomitantes du système nerveux central
- 9. Neuropathie périphérique ≥ grade 2.
- 10.Pneumopathie interstitielle
- 11. Enceinte ou allaitante.
- 12.La patiente a des affections malignes antérieures ou concomitantes, à l'exception : des affections malignes invasives en rémission depuis plus de 5 ans et d'un cancer de la peau non mélanique ou d'un carcinome in situ du col de l'utérus.

La chirurgie des métastases du cancer colorectal sera poursuivie après 3 à 6 cycles de chimiothérapie (folfox ou folfiri) + bevacizumab.
Bévacizumab 5 mg/kg dans 100 ml de NaCl 0,9 % en perfusion IV 3 à 5 cycles.
Pas de bevacizumab pour le cycle ultime.
