- 1. インフォームドコンセント署名時に18歳以上の女性または男性の患者
- 2.ECOG (Eastern Cooperative Oncology Group) パフォーマンスステータス 0 または 1
- 3. 上皮内原発腫瘍の有無にかかわらず、結腸または直腸の腺癌の組織学的または細胞学的診断が確認されている。 野生型または変異型 KRAS 腫瘍の状態。
- 4.患者は術前化学療法の決定が考慮される切除可能な転移疾患を呈していなければならない。 必要に応じて、切除可能性を 1 段階または複数段階で計画することができます。 一般に認められているように、切除可能とは、辺縁に腫瘍がなく、十分な肝予備能を備えた、検出可能なすべての(肝臓)病変の外科的除去(+/- 高周波アブレーション)を意味します。 実際には、両側の腫瘍の位置、病変の数と位置、および不十分な肝予備能が依然として主な決定要因となります。
- 5.患者がmCRCに対する化学療法を受けたことがない場合、転移性疾患の部分的および軽度の切除は、包含前3か月以内に許可されます。
- 6.肝臓以外の転移部位は1部位に限定されます。 肝臓以外の部位は、一段階の手術で簡単に切除可能でなければなりません。
- 7. 患者は、骨盤に対する補助化学療法または(新規)補助化学放射線療法を受けている可能性があります。ただし、化学療法の最後の用量が、組み入れの少なくとも6か月前(オキサリプラチンの場合は12か月)前に投与されている必要があります。 骨盤への以前の放射線治療は除外基準ではありません。

- 12.男性患者は外科的に不妊であるか、性的に活動的で閉経前のパートナーがいる場合は許容可能な避妊方法を使用している必要があります。
- 13.積極的な治療を行わない場合の余命は少なくとも3か月。
除外基準:
- 1.切除不能なmCRC(転移性結腸直腸がん)(3か月の化学療法後も切除可能か不確実または可能性が低い場合、患者は試験から除外される)。
- 2.mCRCに対する以前の化学療法または全身療法。 結腸直腸がんに対する補助化学療法は、対象となる前に 6 か月以上前に完了していれば除外基準にはなりません。 オキサリプラチンベースの化学療法は、参加の 1 年以上前に完了する必要があります。
- 3.ベバシズマブ、アフリベルセプト(または他の抗VEGF(血管内皮増殖因子)療法)の以前の利用。
- 4.上腹部に照射された以前の放射線治療。
- 5.臨床的または放射線学的評価によって決定される、腹水、肝硬変、門脈圧亢進症、主要な門脈腫瘍の関与または血栓症の証拠。
- 6.以前の大規模な肝臓切除術: 残存肝臓が最初の肝臓容積の 50% 未満。
- 7. 患者をこのプロトコールによる治療に不適当にする非悪性疾患。
- 8.中枢神経系転移の併発
- 9.グレード2以上の末梢神経障害。
- 10.間質性肺疾患
- 11.妊娠中または授乳中。
- 12.患者は、以下を除く悪性腫瘍を以前または併発している:5年以上寛解している浸潤性悪性腫瘍、および非黒色腫皮膚癌または子宮頸部上皮内癌。
