Gerandomiseerde fase 2-studie waarin pathologische reacties op metastasen van colorectale kanker worden vergeleken na preoperatieve behandeling Bevacizumab combineren met FOLFOX of FOLFIRI
De studie is opgezet om de pathologische tumorrespons op gereseceerde colorectale kankermetastasen na preoperatieve behandeling met bevacizumab gecombineerd met FOLFOX- of FOLFIRI-regime in een prospectief cohort te analyseren en om deze respons te correleren met de uitkomst van de patiënt.

Dit is een fase II, open-label, gerandomiseerde studie bij patiënten met een bevestigde diagnose van resectabel gemetastaseerd colorectaal adenocarcinoom, die niet eerder chemotherapie hebben gekregen voor hun gemetastaseerde ziekte.
De studie is opgezet om pathologische reacties die zijn waargenomen na preoperatieve chemotherapie bevacizumab te vergelijken met FOLFOX of FOLFIRI.

Beschrijving
Inclusiecriteria:
- 1. Vrouwelijke of mannelijke patiënten met ten minste 18 jaar op het moment dat de geïnformeerde toestemming wordt ondertekend
- 2. ECOG (Eastern Cooperative Oncology Group) prestatiestatus 0 of 1
- 3.Histologisch of cytologisch bevestigde diagnose van adenocarcinoom van colon of endeldarm, met of zonder primaire tumor in situ. Wildtype of gemuteerde KRAS-tumorstatus.
- 4.Patiënten moeten een resectabele metastatische ziekte vertonen waarvoor de beslissing van preoperatieve chemotherapie wordt overwogen. Resectabiliteit kan, indien geïndiceerd, in één of meerdere fasen worden gepland. Zoals algemeen wordt erkend, betekent resectabiliteit de chirurgische verwijdering (+/- radiofrequente ablatie) van alle detecteerbare (lever)laesies met tumorvrije marges en verenigbaar met een adequate leverreserve. In de praktijk blijven de bilaterale tumorlocatie, het aantal en de locatie van laesies en onvoldoende leverreserve de belangrijkste beslissingsfactoren.
- 5. Gedeeltelijke en kleine resectie van gemetastaseerde ziekte is toegestaan binnen 3 maanden vóór opname als de patiënt nooit chemotherapie heeft gekregen voor mCRC.
- 6.Extra hepatische metastatische locatie is beperkt tot 1 site. Extra-hepatische locatie moet gemakkelijk reseceerbaar zijn in een operatie in één fase.
- 7.Patiënten mogen adjuvante chemotherapie of (neo-) adjuvante chemo-radiotherapie van het bekken hebben gekregen, op voorwaarde dat de laatste dosis chemotherapie ten minste 6 maanden vóór inclusie is toegediend (12 maanden voor oxaliplatine). Eerdere radiotherapie van het bekken is geen uitsluitingscriterium.
- 8. Voldoende hematologische, nier- en leverfunctie als volgt: Hematologische neutrofielen > 1,5 x 109/l Bloedplaatjes > 100 x 109/l Niercreatinine < 1,5 x ULN (bovengrens van normaal) Hepatisch bilirubine < 1,5 x ULN ASAT (aspartaataminotransferase) , ALT (Alanine Aminotransferase) < 5 x ULN Phos Alc. < 5 x ULN
- 9.Proteïnurie <2+ (urineonderzoek met peilstok) of =1g/24uur.
- 10. Geen voorgeschiedenis van myocardinfarct en/of beroerte binnen 6 maanden voorafgaand aan randomisatie. Geen ongecontroleerde hypertensie (gedefinieerd als systolische bloeddruk >150 mmHg en/of diastolische bloeddruk >100 mmHg), of voorgeschiedenis van hypertensieve crisis of hypertensieve encefalopathie.
- 11. Vrouwelijke patiënten moeten postmenopauzaal zijn, onvruchtbaar (chirurgisch of door bestraling of chemisch geïnduceerd), of als ze seksueel actief zijn, moeten ze een aanvaardbare anticonceptiemethode gebruiken.
- 12. Mannelijke patiënten moeten chirurgisch steriel zijn of, als ze seksueel actief zijn en een premenopauzale partner hebben, een acceptabele anticonceptiemethode gebruiken.
- 13. Levensverwachting van minimaal 3 maanden zonder enige actieve behandeling.
Uitsluitingscriteria:
- 1. Niet-reseceerbare mCRC (gemetastaseerde colorectale kanker) (indien resecabiliteit onzeker of onwaarschijnlijk blijft na 3 maanden chemotherapie, wordt de patiënt uitgesloten van deelname aan de studie).
- 2. Voorafgaande chemotherapie of systemische therapie voor mCRC. Adjuvante chemotherapie voor colorectale kanker is geen uitsluitingscriterium, mits deze meer dan 6 maanden voorafgaand aan inclusie is voltooid. Chemotherapie op basis van oxaliplatine moet meer dan 1 jaar voorafgaand aan opname voltooid zijn.
- 3. Eerder gebruik van bevacizumab, aflibercept (of andere anti-VEGF (vasculaire endotheliale groeifactor) therapie).
- 4. Eerdere bestraling in de bovenbuik.
- 5. Bewijs van ascites, cirrose, portale hypertensie, betrokkenheid van een hoofdportaal veneuze tumor of trombose zoals bepaald door klinische of radiologische beoordeling.
- 6. Voorafgaande grote leverresectie: resterende lever < 50% van het initiële levervolume.
- 7. Niet-kwaadaardige ziekte die de patiënt ongeschikt zou maken voor behandeling volgens dit protocol.
- 8. Gelijktijdige metastasen van het centrale zenuwstelsel
- 9.Perifere neuropathie ≥ graad 2.
- 10.Interstitiële longziekte
- 11. Zwanger of borstvoeding.
- 12. De patiënt heeft eerdere of gelijktijdige maligniteiten, behalve: invasieve maligniteiten in remissie gedurende meer dan 5 jaar en niet-melanoom huidkanker of carcinoma in situ van de cervix.
