Description détaillée
Un coronavirus humain (HCoV-19) a provoqué la nouvelle épidémie de maladie à coronavirus (COVID-19) dans le monde. Les symptômes courants de la COVID-19 comprennent la fièvre, la toux et l'essoufflement. La majorité des cas entraînent des symptômes bénins, mais certains peuvent évoluer vers une pneumonie et une défaillance multiviscérale. Selon la gravité, il est divisé en léger, normal, grave et gravement malade, ce qui est associé à l'admission et à la mortalité aux soins intensifs. À l'heure actuelle, le traitement standard des patients atteints de COVD-19 est l'oxygénothérapie, la ventilation mécanique et les médicaments pour maintenir la pression artérielle. À ce jour, aucune thérapie antivirale spécifique n'est disponible pour les patients atteints de COVID-19. L'activation immunitaire chez certains patients et l'apparition du syndrome de tempête de cytokines (CSS) sont l'une des principales causes de lésions graves des poumons et d'autres organes, pouvant entraîner la mort. Il est urgent de développer de nouvelles interventions pour supprimer la réponse immunitaire excessive en temps opportun au cours de l'évolution de la maladie, protéger la fonction alvéolaire et réduire les dommages aux poumons et aux organes systémiques.
Zofin est un produit acellulaire, peu manipulé, dérivé du liquide amniotique humain (HAF). Ce produit contient plus de 300 facteurs de croissance, cytokines et chimiokines ainsi que d'autres vésicules/nanoparticules extracellulaires dérivées de cellules souches et épithéliales amniotiques. Le produit contient une concentration moyenne de 5,24x10^11 particules/mL avec une taille de mode moyenne de 125,2nm. L'analyse des marqueurs de surface a confirmé la présence de protéines associées aux exosomes CD63, CD81 et CD9 en plus de l'expression élevée de CD133. Le séquençage terminé a révélé 102 miARN couramment exprimés (avec un minimum d'expression de 100 copies). L'analyse bioinformatique a lié 63 miARN à 1216 cibles d'ARN. Les principaux acteurs de la cascade de cytokines pro-inflammatoires ciblées par les miARN ont été découverts dans le produit d'Organicell, notamment le TNF, l'IL-6 et l'IL-8. De plus, un éventail plus large de cytokines pro-inflammatoires est également ciblé par la collecte de miARN tels que FGF2, IFNB1, IGF1, IL36a, IL37, TGF-B2, VEGFA, CCL8 et CXCL12. Il a été suggéré dans des recherches publiées que l'inhibition ou la suppression de cette cascade de cytokines pro-inflammatoires peut réduire la gravité des symptômes associés à une réponse immunitaire élevée. De plus, le miARN s'est avéré cibler 148 gènes associés à la réponse immunitaire.
La propriété de Zofin démontre le potentiel thérapeutique en tant que suppresseur de l'activation des cytokines pour la réduction de la gravité de l'infection au COVID-19. Il s'agit d'un protocole d'accès élargi ouvert pour traiter les sujets utilisant ZofinTM (OrganicellTM Flow) pour le traitement du COVID-19 léger à modéré dû au SRAS-Cov-2.

Critère d'intégration:
- Fournir un consentement éclairé écrit
- Les sujets sont âgés de plus de 18 ans au moment de la signature du formulaire de consentement éclairé.
- Masculin ou féminin
- Doit avoir un diagnostic clinique de COVID-19 par la réaction qualitative en chaîne par polymérase de transcription inverse (RT-PCR), avec au moins un des symptômes cliniques légers ou modérés de COVID-19 qui sont définis ci-dessous au n ° 5
Personnes présentant des symptômes légers à modérés de la COVID-19 pour la population ambulatoire et hospitalière.
Les principaux symptômes d'une maladie bénigne sont :
- fièvre légère <38°C (37,5 à 37,9)
- une toux sèche
- fatigue
- se sentir légèrement essoufflé
- douleur musculaire
- mal de tête
- mal de gorge
- diarrhée
Les principaux symptômes d'une maladie modérée sont :
En plus des symptômes que les patients peuvent ressentir avec une maladie bénigne, ils peuvent également ressentir :
- fièvre ≥38°
- une toux sèche et plus persistante plusieurs fois par heure
- fatigue et besoin de rester au lit
- se sentir essoufflé lors d'exercices modérés (comme monter les escaliers)
- douleurs musculaires et courbatures et besoin de rester au lit
- mal de tête surtout si vous vous sentez chaud
- mal de gorge, douleur due à la toux, mais pas de douleur
- diarrhée
- une bouche sèche
- Accès veineux adéquat
Pour les femmes en âge de procréer (WOCBP) uniquement, volonté d'utiliser le contrôle des naissances recommandé par la FDA jusqu'à 6 mois après le traitement. Les méthodes de contraception approuvées et autorisées par la FDA sont énumérées ci-dessous :

- Tout sujet masculin doit accepter d'utiliser des contraceptifs et de ne pas donner de sperme pendant l'étude.
- Doit accepter de se conformer à toutes les exigences du protocole et être prêt à effectuer toutes les visites d'étude
Critère d'exclusion:
- Patients présentant un syndrome de détresse respiratoire modéré à sévère dû au COVID-19
- Être une femme enceinte, allaitante ou en âge de procréer sans pratiquer de méthodes contraceptives efficaces. Les sujets féminins doivent subir un test de grossesse sanguin lors du dépistage qui aura lieu dans les 72 heures suivant la perfusion IP.
- Incapacité à effectuer l'une des évaluations requises.
- Inscription active (ou inscription future prévue) pour la transplantation de tout organe.
- Être un receveur d'une greffe d'organe solide. Cela n'inclut pas la thérapie cellulaire antérieure (> 12 mois avant l'inscription), la greffe osseuse, cutanée, ligamentaire, tendineuse ou cornéenne. Avoir des antécédents de rejet de greffe d'organe ou de cellule.
- Antécédents d'abus de drogues (drogues "de rue" illégales à l'exception de la marijuana (s'il s'agit d'une utilisation légale dans les États où réside le patient) ou de médicaments sur ordonnance non utilisés de manière appropriée pour une condition médicale préexistante ou un abus d'alcool (≥ 5 verres/jour pendant ˃ 3 mois), ou des problèmes médicaux, professionnels ou juridiques documentés résultant de la consommation d'alcool ou de drogues au cours des 24 derniers mois
- Patients infectés par le VIH non traités. Cependant, les patients peuvent être inscrits s'ils ont été traités pour le VIH et que le test de charge virale du VIH est négatif, mais qu'ils sont toujours positifs pour les anticorps.
