Un coronavirus humano (HCoV-19) ha causado el nuevo brote de enfermedad por coronavirus (COVID-19) en todo el mundo. Los síntomas comunes de COVID-19 incluyen fiebre, tos y dificultad para respirar. La mayoría de los casos resultan en síntomas leves, pero algunos pueden progresar a neumonía y falla multiorgánica. Según la gravedad se divide en leve, normal, grave y críticamente enfermo, lo que se asocia con ingreso en UCI y mortalidad. En la actualidad, el tratamiento estándar de los pacientes con COVD-19 es la oxigenoterapia, la ventilación mecánica y los medicamentos para mantener la presión arterial. A día de hoy, no existe una terapia antiviral específica disponible para pacientes con COVID-19. La activación inmunitaria en algunos pacientes y la aparición del síndrome de tormenta de citoquinas (CSS) es una de las causas importantes del daño severo a los pulmones y otros órganos, que puede conducir a la muerte. Existe una necesidad urgente de desarrollar nuevas intervenciones para suprimir la respuesta inmunitaria excesiva de manera oportuna durante el curso de la enfermedad, proteger la función alveolar y reducir el daño pulmonar y de órganos sistémicos.
Zofin es un producto acelular, mínimamente manipulado, derivado del líquido amniótico humano (HAF). Este producto contiene más de 300 factores de crecimiento, citocinas y quimiocinas, así como otras vesículas/nanopartículas extracelulares derivadas de células madre amnióticas y epiteliales. El producto contiene una concentración media de 5,24x10^11 partículas/mL con un tamaño de modo medio de 125,2 nm. El análisis de marcadores de superficie confirmó la presencia de proteínas asociadas a exosomas CD63, CD81 y CD9, además de una alta expresión de CD133. La secuenciación completa reveló 102 miARN comúnmente expresados (con un mínimo de expresión de 100 copias). El análisis bioinformático vinculó 63 miARN con 1216 objetivos de ARN. Los principales actores en la cascada de citocinas proinflamatorias que se encontró que son el objetivo de miARN se descubrieron en el producto de Organicell, que incluyen TNF, IL-6 e IL-8. Además, una gama más amplia de citoquinas proinflamatorias también está dirigida por la colección de miARN como FGF2, IFNB1, IGF1, IL36a, IL37, TGF-B2, VEGFA, CCL8 y CXCL12. Se ha sugerido en investigaciones publicadas que la inhibición o supresión de esta cascada de citocinas proinflamatorias puede reducir la gravedad de los síntomas asociados con una respuesta inmunitaria elevada. Además, se descubrió que el miARN se dirige a 148 genes asociados con la respuesta inmunitaria.
La propiedad de Zofin demuestra el potencial terapéutico como supresor de la activación de citoquinas para la reducción de la gravedad de la infección por COVID-19. Este es un protocolo de acceso ampliado de etiqueta abierta para tratar sujetos que usan ZofinTM (OrganicellTM Flow) para el tratamiento de COVID-19 de leve a moderado debido al SARS-Cov-2.

Descripción
Criterios de inclusión:
- Proporcionar consentimiento informado por escrito
- Sujetos mayores de 18 años al momento de firmar el Consentimiento Informado.
- Masculino o femenino
- Debe tener un diagnóstico clínico de COVID-19 mediante la reacción en cadena de la polimerasa de transcripción inversa cualitativa (RT-PCR), con al menos uno de los síntomas clínicos leves o moderados de COVID-19 que se definen a continuación en el n.° 5
Individuos con síntomas leves a moderados de COVID-19 para la población de pacientes ambulatorios y hospitalizados.
Los principales síntomas de una enfermedad leve son:
- fiebre baja <38°C (37,5 a 37,9)
- una tos seca
- cansancio
- sintiéndose ligeramente sin aliento
- dolor muscular
- dolor de cabeza
- dolor de garganta
- diarrea
Los principales síntomas de la enfermedad moderada son:
Además de los síntomas que los pacientes pueden tener con una enfermedad leve, también pueden experimentar:
- fiebre ≥38°
- una tos seca y más persistente varias veces por hora
- cansancio y necesidad de quedarse en cama
- sentirse sin aliento al hacer ejercicio moderado (como subir escaleras)
- dolor muscular y dolores y necesidad de permanecer en cama
- dolor de cabeza, especialmente si se siente caliente
- dolor de garganta, dolor al toser, pero sin dolor
- diarrea
- una boca seca
- Acceso venoso adecuado
Solo para mujeres en edad fértil (WOCBP), voluntad de usar el control de la natalidad recomendado por la FDA hasta 6 meses después del tratamiento. Los métodos anticonceptivos aprobados y autorizados por la FDA se enumeran a continuación:
- Esterilización Permanente
- Anticonceptivos reversibles de acción prolongada (LARC)

- Cualquier sujeto masculino debe aceptar usar anticonceptivos y no donar esperma durante el estudio.
- Debe aceptar cumplir con todos los requisitos del protocolo y estar dispuesto a completar todas las visitas del estudio
Criterio de exclusión:
- Pacientes que tienen síndrome de dificultad respiratoria de moderado a grave debido a COVID-19
- Ser una mujer que esté embarazada, amamantando o en edad fértil y que no practique métodos anticonceptivos efectivos. Las mujeres deben someterse a una prueba de embarazo en sangre en la selección, que se realizará dentro de las 72 horas posteriores a la infusión IP.
- Incapacidad para realizar cualquiera de las evaluaciones requeridas.
- Listado activo (o listado futuro esperado) para trasplante de cualquier órgano.
- Ser receptor de un trasplante de órgano sólido. Esto no incluye terapia celular previa (>12 meses antes de la inscripción), hueso, piel, ligamentos, tendones o injertos de córnea. Tiene antecedentes de rechazo de trasplante de órganos o células.
- Antecedentes de abuso de drogas (drogas "callejeras" ilegales, excepto marihuana (si es un uso legal en los estados donde reside el paciente), o medicamentos recetados que no se usan adecuadamente para una afección médica preexistente o abuso de alcohol (≥ 5 tragos/día por ˃ 3 meses), o problemas médicos, ocupacionales o legales documentados que surjan del uso de alcohol o drogas en los últimos 24 meses
- Pacientes con infección por VIH no tratada. Sin embargo, los pacientes pueden inscribirse si han recibido tratamiento para el VIH y la prueba de carga viral del VIH es negativa, pero aún así la prueba de anticuerpos es positiva.
