Um coronavírus humano (HCoV-19) causou o surto da doença do novo coronavírus (COVID-19) em todo o mundo. Os sintomas comuns do COVID-19 incluem febre, tosse e falta de ar. A maioria dos casos resulta em sintomas leves, mas alguns podem progredir para pneumonia e falência de múltiplos órgãos. De acordo com a gravidade, divide-se em leve, normal, grave e grave, o que está associado à internação em UTI e mortalidade. Atualmente, o tratamento padrão de pacientes com COVD-19 é oxigenoterapia, ventilação mecânica e medicamentos para manter a pressão arterial. Até o momento, nenhuma terapia antiviral específica está disponível para pacientes com COVID-19. A ativação imune em alguns pacientes e o aparecimento da síndrome da tempestade de citocinas (CSS) é uma das causas importantes de danos graves aos pulmões e outros órgãos, que podem levar à morte. Há uma necessidade urgente de desenvolver novas intervenções para suprimir a resposta imune excessiva em tempo hábil durante o curso da doença, proteger a função alveolar e reduzir danos pulmonares e de órgãos sistêmicos.
Zofin é um produto acelular, minimamente manipulado, derivado do líquido amniótico humano (HAF). Este produto contém mais de 300 fatores de crescimento, citocinas e quimiocinas, bem como outras vesículas/nanopartículas extracelulares derivadas de células epiteliais e estaminais amnióticas. O produto contém uma concentração média de 5,24x10^11 partículas/mL com um tamanho de modo médio de 125,2 nm. A análise do marcador de superfície confirmou a presença de proteínas associadas ao exossoma CD63, CD81 e CD9, além da alta expressão de CD133. O sequenciamento completo revelou 102 miRNA comumente expressos (com um mínimo de expressão de 100 cópias). A análise de bioinformática ligou 63 miRNAs a 1216 alvos de RNA. Os principais atores na cascata de citocinas pró-inflamatórias que são alvo de miRNA foram descobertos no produto da Organicell, incluindo TNF, IL-6 e IL-8. Além disso, uma gama mais ampla de citocinas pró-inflamatórias também é alvo da coleção de miRNA, como FGF2, IFNB1, IGF1, IL36a, IL37, TGF-B2, VEGFA, CCL8 e CXCL12. Foi sugerido em pesquisas publicadas que a inibição ou supressão dessa cascata de citocinas pró-inflamatórias pode reduzir a gravidade dos sintomas associados à resposta imune elevada. Além disso, descobriu-se que o miRNA tem como alvo 148 genes associados à resposta imune.
A propriedade de Zofin demonstra o potencial terapêutico como supressor da ativação de citocinas para a redução da gravidade da infecção por COVID-19. Este é um protocolo de acesso expandido de rótulo aberto para tratar indivíduos usando ZofinTM (OrganicellTM Flow) para tratamento de COVID-19 leve a moderado devido ao SARS-Cov-2.

Descrição
Critério de inclusão:
- Fornecer consentimento informado por escrito
- Sujeitos com idade > 18 anos no momento da assinatura do Termo de Consentimento Livre e Esclarecido.
- Macho ou fêmea
- Deve ter um diagnóstico clínico de COVID-19 pela reação em cadeia da polimerase com transcrição reversa qualitativa (RT-PCR), com pelo menos um dos sintomas clínicos leves ou moderados de COVID-19 definidos abaixo no nº 5
Indivíduos com sintomas leves a moderados de COVID-19 para pacientes ambulatoriais e internados.
Os principais sintomas da doença leve são:
- febre baixa <38°C (37,5 a 37,9)
- uma tosse seca
- cansaço
- sentindo-se um pouco sem fôlego
- dor muscular
- dor de cabeça
- dor de garganta
- diarréia
Os principais sintomas da doença moderada são:
Além dos sintomas que os pacientes podem apresentar com uma doença leve, eles também podem apresentar:
- febre ≥38°
- uma tosse seca e mais persistente várias vezes por hora
- cansaço e necessidade de ficar na cama
- sentir falta de ar ao fazer exercícios moderados (como subir escadas)
- dores musculares e dores e necessidade de ficar na cama
- dor de cabeça, especialmente se você estiver se sentindo quente
- dor de garganta, dor de tosse, mas sem dor
- diarréia
- uma boca seca
- Acesso venoso adequado
Apenas para mulheres com potencial para engravidar (WOCBP), vontade de usar o controle de natalidade recomendado pela FDA até 6 meses após o tratamento. Os métodos aprovados e liberados pela FDA para controle de natalidade estão listados abaixo:

- Qualquer indivíduo do sexo masculino deve concordar em usar contraceptivos e não doar esperma durante o estudo.
- Deve concordar em cumprir todos os requisitos do protocolo e estar disposto a concluir todas as visitas do estudo
Critério de exclusão:
- Pacientes com síndrome do desconforto respiratório moderado a grave devido à COVID-19
- Ser uma mulher grávida, amamentando ou com potencial para engravidar sem praticar métodos contraceptivos eficazes. Indivíduos do sexo feminino devem ser submetidos a um teste de gravidez de sangue na triagem, que será feito dentro de 72 horas após a infusão IP.
- Incapacidade de realizar qualquer uma das avaliações necessárias.
- Listagem ativa (ou listagem futura esperada) para transplante de qualquer órgão.
- Ser um receptor de transplante de órgão sólido. Isso não inclui terapia celular anterior (>12 meses antes da inscrição), osso, pele, ligamento, tendão ou enxerto de córnea. Ter um histórico de rejeição de transplante de órgãos ou células.
- História de abuso de drogas (drogas "de rua" ilegais, exceto maconha (se for uso legal nos estados onde o paciente reside), ou medicamentos prescritos não sendo usados adequadamente para uma condição médica pré-existente ou abuso de álcool (≥ 5 drinques/dia por ˃ 3 meses), ou problemas médicos, ocupacionais ou legais documentados decorrentes do uso de álcool ou drogas nos últimos 24 meses
- Pacientes com infecção por HIV não tratada. No entanto, os pacientes podem ser inscritos se tiverem sido tratados para o HIV e o teste for negativo para a carga viral do HIV, mas ainda assim o teste for positivo para anticorpos.
