ADAPT-HER2-IV abordera la question du traitement néoadjuvant optimal chez les patients atteints d'EBC -HER2+ moins avancé.

ADAPT-HER2-IV est prévu comme un essai de supériorité pour démontrer des taux de pCR plus élevés dans les deux sous-groupes cliniquement pertinents d'EBC HER2+ à risque faible à intermédiaire. De plus, il vise à démontrer une excellente survie chez les patients traités par T-DXd (avec l'utilisation d'une chimiothérapie standard à la décision de l'investigateur limitée uniquement aux patients présentant une charge tumorale résiduelle importante après le traitement par T-DXd).

Description détaillée

Comme les essais ADAPT l'ont clairement montré, la pCR après 12 semaines de traitement, indépendamment du régime néoadjuvant de désescalade spécifique et indépendamment de l'utilisation ultérieure de la chimiothérapie systémique, est un prédicteur indépendant d'un excellent pronostic4,19, également chez les patients traités par un conjugué anticorps-médicament seul (T-DM1), ou chez ceux recevant pertuzumab+trastuzumab+/-paclitaxel hebdomadaire.

Contrairement au contexte adjuvant, aucun des essais néoadjuvants jusqu'à présent ne s'est concentré sur les patientes HER2+ présentant un profil de risque faible à intermédiaire (par exemple, les patientes sans envahissement ganglionnaire avec des tumeurs cT1-2). L'essai ADAPT-HER2-IV vise à combler ce manque de preuves.

Étant donné qu'il existe une certaine incertitude quant à la durée optimale du traitement dans les EBC HER2+ à risque intermédiaire à élevé (par exemple, taille de la tumeur> 3 cm), nous recommandons d'utiliser un traitement à base de taxane plus long de 18 semaines (+/- carboplatine, chez l'investigateur. s) en raison d'un grand nombre de preuves pour les combinaisons taxane + carboplatine chez les patients à des stades localement avancés.

Les conjugués anticorps-médicament semblent être des candidats-médicaments idéaux pour un traitement "désamorcé" en raison de leur innocuité favorable (réduction des taux d'alopécie, de polyneuropathie, etc.) et d'un profil d'efficacité élevé (par exemple, taux de pCR comparables après 18 semaines de T- DM1 et taxane+pertuzumab+trastuzumab dans l'essai PREDIX HER220). Comme dans le paysage de la chimiothérapie classique, la durée optimale du traitement néoadjuvant à base de conjugué anticorps-médicament reste incertaine. Des taux de pCR d'environ 40 % à 60 % ont été observés après 12 et 18 semaines de traitement par T-DM1 (+/-pertuzumab) dans les essais ADAPT TP, KRISTINE et PREDIX HER2 dans la maladie HR+/HER2+21,22. De plus, la survie à long terme semble être comparable entre T-DM1 + ​​pertuzumab et les schémas thérapeutiques plus anciens contenant une chimiothérapie (docétaxel + carboplatine + trastuzumab + pertuzumab) malgré des taux de progression locale plus élevés et une pCR plus faible dans une étude22.

Le trastuzumab-deruxtecan (T-DXd) a montré une activité prometteuse dans une petite cohorte de patients métastatiques, comprenant à la fois HER2+ et HER2-low BC, prétraités avec plusieurs lignes de traitement. Doi et al. ont rapporté des taux de réponse globale (ORR) de 58 % et un taux de contrôle de la maladie de 100 % avec une survie globale à 12 mois dans la maladie HER2+ prétraitée par T-DM1+/-pertuzumab en traitement tardif23. La thérapie T-DXd était associée à un profil d'innocuité gérable. Récemment, une efficacité nettement supérieure du T-DXd par rapport au T-DM1 a été démontrée dans le cancer du sein métastatique (CSM) de deuxième ligne dans l'essai DESTINY-0324. La médiane de survie sans progression n'a pas été atteinte dans le bras T-DM1 contre 6,8 mois dans le bras T-DXd. Cet effet était indépendant du statut des récepteurs hormonaux, du traitement antérieur par le pertuzumab, des métastases viscérales, du nombre de lignes de traitement antérieures et de la présence de métastases cérébrales. Le TRG a été doublé (34,2 contre 79,7 %), favorisant le bras T-DXd.

Critère d'intégration:

Les patients éligibles à l'inclusion dans cette étude doivent répondre à tous les critères suivants :

  1. Patientes atteintes d'un cancer du sein HER2+ invasif et non traité (tel qu'évalué par la pathologie locale) maximum 6 semaines avant l'enregistrement (biopsie diagnostique standard selon les directives actuelles de l'AGO)
  2. Âge ≥18 ans 3a. Cohorte 1 : risque de récidive faible à intermédiaire selon la décision de l'investigateur (recommandation : cT1c - cT2 (1 - ≤ 3 cm), cN0 ; cT1a/b exclu), OU 3b. Cohorte 2 : risque de récidive intermédiaire à élevé selon la décision de l'investigateur (recommandation : cT2 (>3 - ≤5cm), cN0) 3c. Les patients âgés (≥ 65 ans) peuvent être affectés à n'importe quelle cohorte selon la décision de l'investigateur

4. Consentement éclairé écrit 5. FEVG ≥ 50 % dans les 28 jours précédant la randomisation 6. Indice de performance Eastern Cooperative Oncology Group (ECOG PS) 0-1 7. Fonction adéquate des organes et de la moelle osseuse dans les 14 jours précédant la randomisation 8. Critère d'exclusion:

Les patients éligibles à l'inclusion dans cette étude ne doivent répondre à aucun des critères suivants :

  1. Cancer du sein non opérable dont cancer du sein inflammatoire
  2. cancer du sein cT1a/b
  3. Tout antécédent de cancer du sein invasif
  4. Tumeurs malignes primaires dans les 5 ans, à l'exception du cancer de la peau autre que le mélanome correctement réséqué, maladie in situ traitée curativement
  5. Toute preuve de maladie métastatique existante (confirmée par CT Thorax/Abdomen, scintigraphie osseuse ou autres méthodes selon la pratique clinique
  6. Traitement antérieur ou concomitant avec des agents cytotoxiques pour quelque raison que ce soit (sauf raisons non oncologiques)
  7. Traitement concomitant avec d'autres médicaments expérimentaux et participation à un autre essai clinique avec tout médicament expérimental dans les 30 jours précédant l'entrée à l'étude
  8. Co-morbidité grave et pertinente qui interagirait avec l'application d'agents cytotoxiques ou la participation à l'étude/fonctionnement inadéquat des organes
  9. Raisons indiquant un risque de mauvaise conformité
  10. Femme en âge de procréer définie comme une femme physiologiquement capable de devenir enceinte, et n'utilisant pas de méthodes de contraception très efficaces pendant le traitement à l'étude et pendant 3 mois après l'arrêt du traitement.
  11. Utilisation de méthodes contraceptives hormonales orales (œstrogènes et progestérone), transdermiques, injectées ou implantées ainsi que d'un traitement hormonal substitutif.
  12. A une toxicomanie ou toute autre condition médicale telle que des conditions cardiaques ou psychologiques cliniquement significatives, qui peuvent, de l'avis de l'investigateur, interférer avec la participation du sujet à l'étude clinique ou à l'évaluation des résultats de l'étude clinique.
  13. Patients ayant des antécédents médicaux d'infarctus du myocarde (IM) dans les 6 mois précédant la randomisation, insuffisance cardiaque congestive (ICC) symptomatique (New York Heart Association Class II à IV), sujets présentant des taux de troponine supérieurs à la LSN lors du dépistage (tel que défini par le fabricant) , et sans aucun symptôme lié au myocarde, doit avoir une consultation cardiologique avant l'inscription pour exclure un IM.
  14. Allongement de l'intervalle QT corrigé (QTcF) à > 470 msec (femmes) sur la base de la moyenne de l'ECG de dépistage en triple exemplaire à 12 dérivations.
  15. Antécédents de PID / pneumonie (non infectieuse) nécessitant des stéroïdes, présente une PID / pneumonie actuelle ou une suspicion de PID / pneumonie ne peut être exclue par imagerie lors du dépistage.
  16. Critères pulmonaires : maladies intercurrentes cliniquement significatives spécifiques aux poumons, y compris, mais sans s'y limiter, tout trouble pulmonaire sous-jacent ; Tout trouble auto-immun, du tissu conjonctif ou inflammatoire (par exemple, la polyarthrite rhumatoïde, la maladie de Sjogren, la sarcoïdose, etc.) lorsqu'il est documenté, ou une suspicion d'atteinte pulmonaire au moment de la randomisation ; Pneumectomie antérieure (complète); Infection non contrôlée nécessitant des antibiotiques IV, des antiviraux ou des antifongiques
  17. Immunodéficience primaire active, infection connue par le virus de l'immunodéficience humaine (VIH) ou infection active par l'hépatite B ou C. Les patients positifs pour les anticorps anti-hépatite C (VHC) ne sont éligibles que si la réaction en chaîne par polymérase est négative pour l'ARN du VHC. Les patients doivent subir un test de dépistage du VIH avant la randomisation si la réglementation locale ou le comité d'éthique (CE) l'exige.
  18. Réception d'un vaccin vivant atténué (les vaccins adénoviraux à ARNm et à réplication déficiente ne sont pas considérés comme des vaccins vivants atténués) dans les 30 jours précédant la première dose de trastuzumab deruxtecan.

    Remarque : Les patients, s'ils sont inscrits, ne doivent pas recevoir de vaccin vivant pendant l'étude et jusqu'à 30 jours après la dernière dose d'IMP.

  19. Allergie ou hypersensibilité connue au traitement de l'étude (T-DXd) ou à l'un des excipients du médicament à l'étude.
  20. Antécédents de réactions d'hypersensibilité sévères à d'autres anticorps monoclonaux.
  21. Patientes enceintes ou allaitantes, ou patientes qui envisagent de devenir enceintes.
