Terapia neoadyuvante con trastuzumab-deruxtecan versus quimioterapia+trastuzumab+pertuzumab en cáncer de mama temprano HER2+ (ADAPTHER2-IV)
Marcador dinámico neoadyuvante: terapia personalizada ajustada que compara trastuzumab-deruxtecan versus pacli-/docetaxel+carboplatino+trastuzumab+pertuzumab en cáncer de mama temprano HER2+
ADAPT-HER2-IV abordará la cuestión de la terapia neoadyuvante óptima en pacientes con EBC -HER2+ menos avanzado.
ADAPT-HER2-IV está planificado como un ensayo de superioridad para demostrar tasas más altas de PCR en ambos subgrupos clínicamente relevantes de EBC HER2+ de riesgo bajo-intermedio. Además, tiene como objetivo demostrar una excelente supervivencia en pacientes tratados con T-DXd (con el uso de quimioterapia estándar a decisión del investigador restringida solo a pacientes con una carga tumoral residual sustancial después del tratamiento con T-DXd).

Como han demostrado claramente los ensayos ADAPT, la PCR después de 12 semanas de tratamiento, independientemente del régimen neoadyuvante específico desescalado e independiente del uso posterior de quimioterapia sistémica, es un predictor independiente de excelente pronóstico4,19, también en pacientes tratados con un conjugado anticuerpo-fármaco solo (T-DM1), o en aquellos que reciben pertuzumab+trastuzumab+/- paclitaxel semanal.
A diferencia del entorno adyuvante, ninguno de los ensayos neoadyuvantes hasta el momento se ha centrado en pacientes HER2+ con un perfil de riesgo intermedio bajo (p. ej., pacientes con ganglios negativos con tumores cT1-2). El ensayo ADAPT-HER2-IV tiene como objetivo cerrar esta brecha de evidencia.
Debido a que existe cierta incertidumbre acerca de la duración óptima del tratamiento en el EBC HER2+ de riesgo intermedio a alto (p. ej., tamaño del tumor >3 cm), recomendamos utilizar un tratamiento más prolongado basado en taxanos de 18 semanas (+/- carboplatino, a criterio del investigador). decisión de s) debido a una gran cantidad de evidencia para las combinaciones de taxano + carboplatino en pacientes en estadios localmente avanzados.
Los conjugados de fármaco-anticuerpo parecen ser fármacos candidatos ideales para un tratamiento "reducido" debido a su seguridad favorable (alopecia reducida, tasas de polineuropatía, etc.) y un alto perfil de eficacia (p. ej., tasas de PCR comparables después de 18 semanas de T- DM1 y taxano+pertuzumab+trastuzumab en el ensayo PREDIX HER220). De manera similar al panorama de la quimioterapia clásica, la duración óptima de la terapia neoadyuvante basada en conjugados de anticuerpos y fármacos sigue sin estar clara. Se observaron tasas de PCR de alrededor del 40 % al 60 % después de 12 y 18 semanas de tratamiento con T-DM1 (+/- pertuzumab) en los ensayos ADAPT TP, KRISTINE y PREDIX HER2 en enfermedad HR+/HER2+21,22. Además, la supervivencia a largo plazo parece ser comparable entre T-DM1+pertuzumab y regímenes más antiguos que contienen quimioterapia (docetaxel+carboplatino+trastuzumab+pertuzumab) a pesar de las tasas de progresión local más altas y la PCR más baja en un estudio22.
Trastuzumab-deruxtecan (T-DXd) ha mostrado una actividad prometedora en una pequeña cohorte de pacientes metastásicos, incluidos HER2+ y HER2-bajo BC, tratados previamente con varias líneas de terapia. Doi et al. informaron tasas de respuesta general (ORR) del 58 % y una tasa de control de la enfermedad del 100 % con una supervivencia general a los 12 meses en la enfermedad HER2+ pretratada con T-DM1+/-pertuzumab en un entorno de línea tardía23. La terapia con T-DXd se asoció con un perfil de seguridad manejable. Recientemente, se demostró una eficacia claramente superior de T-DXd frente a T-DM1 en el cáncer de mama metastásico (CMM) de segunda línea en el ensayo DESTINY-0324. La mediana de supervivencia libre de progresión no se alcanzó en el brazo T-DM1 frente a 6,8 meses en el brazo T-DXd. Este efecto fue independiente del estado de los receptores hormonales, el tratamiento previo con pertuzumab, las metástasis viscerales, el número de líneas de tratamiento anteriores y la presencia de metástasis cerebrales. La TRO se duplicó (34,2 frente a 79,7 %), favoreciendo el brazo T-DXd.

Criterios de inclusión:
Los pacientes elegibles para su inclusión en este estudio deben cumplir con todos los siguientes criterios:
- Pacientes mujeres con cáncer de mama HER2+ invasivo y no tratado (evaluado por patología local) máximo 6 semanas antes del registro (biopsia diagnóstica estándar de atención según las pautas actuales de AGO)
- Edad ≥18 años 3a. Cohorte 1: riesgo bajo a intermedio de recurrencia según decisión del investigador (recomendación: cT1c - cT2 (1 - ≤3cm), cN0; excluida cT1a/b), O 3b. Cohorte 2: riesgo intermedio a alto de recurrencia según decisión del investigador (recomendación: cT2 (>3 - ≤5cm), cN0) 3c. Los pacientes de edad avanzada (≥ 65 años) pueden asignarse a cualquier cohorte según la decisión del investigador
4. Consentimiento informado por escrito 5. FEVI ≥ 50 % dentro de los 28 días previos a la aleatorización 6. Estado funcional del Grupo Oncológico Cooperativo del Este (ECOG PS) 0-1 7. Función adecuada de órganos y médula ósea dentro de los 14 días previos a la aleatorización 8. Criterio de exclusión:
Los pacientes elegibles para su inclusión en este estudio no deben cumplir ninguno de los siguientes criterios:
- Cáncer de mama no operable, incluido el cáncer de mama inflamatorio
- cáncer de mama cT1a/b
- Cualquier historial previo de cáncer de mama invasivo
- Neoplasias malignas primarias dentro de los 5 años, con la excepción de cáncer de piel no melanoma resecado adecuadamente, enfermedad in situ tratada curativamente
- Cualquier evidencia de enfermedad metastásica existente (confirmada por TC de tórax/abdomen, gammagrafía ósea u otros métodos según la práctica clínica)
- Tratamiento previo o concurrente con agentes citotóxicos por cualquier motivo (excepto motivos no oncológicos)
- Tratamiento simultáneo con otros medicamentos experimentales y participación en otro ensayo clínico con cualquier medicamento en investigación dentro de los 30 días anteriores al ingreso al estudio
- Comorbilidad grave y relevante que interactuaría con la aplicación de agentes citotóxicos o la participación en el estudio/función orgánica inadecuada
- Razones que indican riesgo de cumplimiento deficiente
- Mujer en edad fértil definida como una mujer fisiológicamente capaz de quedar embarazada y que no utiliza métodos anticonceptivos altamente efectivos durante el tratamiento del estudio y durante los 3 meses posteriores a la finalización del tratamiento.
- Uso de métodos anticonceptivos hormonales orales (estrógenos y progesterona), transdérmicos, inyectados o implantados, así como terapia de reemplazo hormonal.
- Tiene abuso de sustancias o cualquier otra afección médica, como afecciones cardíacas o psicológicas clínicamente significativas, que pueden, en opinión del investigador, interferir con la participación del sujeto en el estudio clínico o la evaluación de los resultados del estudio clínico.
- Pacientes con antecedentes médicos de infarto de miocardio (IM) dentro de los 6 meses anteriores a la aleatorización, insuficiencia cardíaca congestiva (ICC) sintomática (Clase II a IV de la New York Heart Association), Sujetos con niveles de troponina por encima del LSN en la selección (como lo define el fabricante) , y sin ningún síntoma relacionado con el miocardio, debe tener una consulta cardiológica antes de la inscripción para descartar un infarto de miocardio.
- Prolongación del intervalo QT corregido (QTcF) a > 470 mseg (mujeres) basado en el promedio del ECG de 12 derivaciones por triplicado de detección.
- Historial de ILD/neumonitis (no infecciosa) que requirió esteroides, tiene ILD/neumonitis actual, o donde la sospecha de ILD/neumonitis no se puede descartar mediante imágenes en el examen de detección.
- Criterios pulmonares: Enfermedades clínicamente significativas intercurrentes específicas de los pulmones que incluyen, entre otras, cualquier trastorno pulmonar subyacente; Cualquier trastorno autoinmunitario, del tejido conjuntivo o inflamatorio (p. ej., artritis reumatoide, enfermedad de Sjogren, sarcoidosis, etc.) en el que se documente o se sospeche afectación pulmonar en el momento de la aleatorización; Neumonectomía previa (completa); Infección no controlada que requiere antibióticos, antivirales o antifúngicos por vía intravenosa
- Inmunodeficiencia primaria activa, infección conocida por el virus de la inmunodeficiencia humana (VIH) o infección activa por hepatitis B o C. Los pacientes positivos para anticuerpos contra la hepatitis C (VHC) son elegibles solo si la reacción en cadena de la polimerasa es negativa para el ARN del VHC. Los pacientes deben someterse a la prueba del VIH antes de la aleatorización si así lo exigen las normativas locales o el comité de ética (CE).
Recepción de vacuna viva atenuada (las vacunas de ARNm y adenovirales de replicación deficiente no se consideran vacunas vivas atenuadas) dentro de los 30 días anteriores a la primera dosis de trastuzumab deruxtecan.
Nota: Los pacientes, si se inscriben, no deben recibir la vacuna viva durante el estudio y hasta 30 días después de la última dosis de IMP.
- Alergia conocida o hipersensibilidad al tratamiento del estudio (T-DXd) o a cualquiera de los excipientes del fármaco del estudio.
- Antecedentes de reacciones de hipersensibilidad graves a otros anticuerpos monoclonales.
- Pacientes mujeres embarazadas o en período de lactancia, o pacientes que planean quedar embarazadas.
