Terapia NeoAdjuvante com Trastuzumabe-deruxtecano Versus Quimioterapia+Trastuzumabe+Pertuzumabe no Câncer de Mama HER2+ Inicial (ADAPTHER2-IV)
Marcador Dinâmico NeoAdjuvant - Terapia Personalizada Ajustada Comparando Trastuzumabe-deruxtecano Versus Pacli-/Docetaxel+Carboplatina+Trastuzumabe+Pertuzumabe em Câncer de Mama HER2+ Inicial
ADAPT-HER2-IV abordará a questão da terapia neoadjuvante ideal em pacientes com EBC -HER2+ menos avançado.
O ADAPT-HER2-IV está planejado como um estudo de superioridade para demonstrar taxas mais altas de PCR em ambos os subgrupos clinicamente relevantes de HER2+ EBC de risco intermediário baixo. Além disso, visa demonstrar excelente sobrevida em pacientes tratados com T-DXd (com o uso de quimioterapia padrão por decisão do investigador restrito apenas a pacientes com carga tumoral residual substancial após o tratamento com T-DXd).

Descrição detalhada
Como os estudos ADAPT mostraram claramente, pCR após 12 semanas de terapia, independente do regime neoadjuvante desescalado específico e independente do uso adicional de quimioterapia sistêmica, é um preditor independente de excelente prognóstico4,19, também em pacientes tratados por um conjugado anticorpo-droga sozinho (T-DM1), ou naqueles recebendo pertuzumabe+trastuzumabe+/-paclitaxel semanal.
Em contraste com o cenário adjuvante, nenhum dos estudos neoadjuvantes até agora se concentrou em pacientes HER2+ com um perfil de risco intermediário baixo (por exemplo, pacientes com nódulo negativo com tumores cT1-2). O estudo ADAPT-HER2-IV visa preencher essa lacuna de evidências.
Como há alguma incerteza sobre a duração ideal do tratamento em EBC HER2+ de risco intermediário a alto (por exemplo, tamanho do tumor > 3 cm), recomendamos o uso de um tratamento mais longo de 18 semanas à base de taxano (+/- carboplatina, no investigador s decisão) devido a um grande corpo de evidências para combinações de taxano + carboplatina em pacientes em estágios localmente avançados.
Conjugados anticorpo-droga parecem ser drogas candidatas ideais para um tratamento "desescalado" devido à sua segurança favorável (alopecia reduzida, taxas de polineuropatia, etc.) e um perfil de alta eficácia (por exemplo, taxas de PCR comparáveis após 18 semanas de T- DM1 e taxano+pertuzumab+trastuzumab no estudo PREDIX HER220). Da mesma forma que no cenário clássico da quimioterapia, a duração ideal da terapia neoadjuvante baseada em conjugado anticorpo-droga permanece incerta. Taxas de pCR de cerca de 40% a 60% foram observadas após 12 e 18 semanas de tratamento com T-DM1 (+/-pertuzumabe) nos estudos ADAPT TP, KRISTINE e PREDIX HER2 na doença HR+/HER2+21,22. Além disso, a sobrevida em longo prazo parece ser comparável entre T-DM1+pertuzumabe e regimes mais antigos contendo quimioterapia (docetaxel+carboplatina+trastuzumabe+pertuzumabe), apesar das taxas de progressão local mais altas e menor pCR em um estudo22.
O trastuzumabe-deruxtecano (T-DXd) mostrou atividade promissora em uma pequena coorte de pacientes metastáticos, incluindo HER2+ e HER2-baixo BC, pré-tratados com várias linhas de terapia. Doi et al. relataram taxas de resposta geral (ORR) de 58% e uma taxa de controle da doença de 100% com sobrevida global em 12 meses na doença HER2+ pré-tratada por T-DM1+/-pertuzumabe em um cenário de linha tardia23. A terapia com T-DXd foi associada a um perfil de segurança administrável. Recentemente, uma eficácia claramente superior de T-DXd vs. T-DM1 foi demonstrada em câncer de mama metastático (MBC) de segunda linha no estudo DESTINY-0324. A sobrevida livre de progressão mediana não foi alcançada no braço T-DM1 versus 6,8 meses no braço T-DXd. Este efeito foi independente do status do receptor hormonal, tratamento anterior com pertuzumabe, metástases viscerais, número de linhas de terapia anteriores e presença de metástases cerebrais. ORR foi duplicado (34,2 vs. 79,7%), favorecendo o braço T-DXd.

Descrição
Critério de inclusão:
Os pacientes elegíveis para inclusão neste estudo devem atender a todos os seguintes critérios:
- Pacientes do sexo feminino com câncer de mama HER2+ invasivo e não tratado (conforme avaliado pela patologia local) no máximo 6 semanas antes do registro (biópsia diagnóstica padrão de atendimento de acordo com as diretrizes atuais da AGO)
- Idade ≥18 anos 3a. Coorte 1: risco baixo a intermediário de recorrência de acordo com a decisão do investigador (recomendação: cT1c - cT2 (1 - ≤3cm), cN0; cT1a/b excluídos), OU 3b. Coorte 2: risco intermediário a alto de recorrência conforme decisão do investigador (recomendação: cT2 (>3 - ≤5cm), cN0) 3c. Pacientes idosos (≥ 65 anos) podem ser designados para qualquer coorte conforme decisão do investigador
4. Termo de consentimento informado por escrito 5. FEVE ≥ 50% dentro de 28 dias antes da randomização 6. Status de desempenho do Eastern Cooperative Oncology Group (ECOG PS) 0-1 7. Função adequada de órgão e medula óssea dentro de 14 dias antes da randomização 8. Período de tratamento adequado antes da randomização (consulte o protocolo para obter informações detalhadas) 9. Evidência de estado pós-menopausa ou teste de gravidez sérico negativo para mulheres com potencial para engravidar (consulte o protocolo para obter informações detalhadas) 10. Indivíduos do sexo feminino não devem doar ou recuperar para uso próprio óvulos desde o momento da randomização e durante todo o período de tratamento do estudo e por pelo menos 7 meses após a administração final do medicamento do estudo. Critério de exclusão:
Os pacientes elegíveis para inclusão neste estudo não devem atender a nenhum dos seguintes critérios:
- Câncer de mama não operável, incluindo câncer de mama inflamatório
- câncer de mama cT1a/b
- Qualquer história anterior de câncer de mama invasivo
- Malignidades primárias dentro de 5 anos, com exceção de câncer de pele não melanoma adequadamente ressecado, doença in situ tratada curativamente
- Qualquer evidência de doença metastática existente (confirmada por TC de tórax/abdômen, cintilografia óssea ou outros métodos de acordo com a prática clínica
- Tratamento prévio ou concomitante com agentes citotóxicos por qualquer motivo (exceto motivos não oncológicos)
- Tratamento concomitante com outras drogas experimentais e participação em outro ensaio clínico com qualquer droga experimental dentro de 30 dias antes da entrada no estudo
- Comorbidade grave e relevante que interagiria com a aplicação de agentes citotóxicos ou a participação no estudo/função inadequada do órgão
- Razões que indicam risco de cumprimento insatisfatório
- Mulher com potencial para engravidar definida como uma mulher fisiologicamente capaz de engravidar e que não usa métodos contraceptivos altamente eficazes durante o tratamento do estudo e por 3 meses após a interrupção do tratamento.
- Uso de métodos contraceptivos hormonais orais (estrogênio e progesterona), transdérmicos, injetados ou implantados, bem como terapia de reposição hormonal.
- Tem abuso de substâncias ou qualquer outra condição médica, como condições cardíacas ou psicológicas clinicamente significativas, que podem, na opinião do investigador, interferir na participação do sujeito no estudo clínico ou na avaliação dos resultados do estudo clínico.
- Pacientes com histórico médico de infarto do miocárdio (IM) dentro de 6 meses antes da randomização, insuficiência cardíaca congestiva (ICC) sintomática (Classe II a IV da New York Heart Association), Indivíduos com níveis de troponina acima do LSN na triagem (conforme definido pelo fabricante) , e sem quaisquer sintomas relacionados ao miocárdio, devem ter uma consulta cardiológica antes da inscrição para descartar infarto do miocárdio.
- Prolongamento do intervalo QT (QTcF) corrigido para > 470 ms (mulheres) com base na média do ECG triplicado de 12 derivações.
- Histórico de DPI/pneumonite (não infecciosa) que exigiu esteróides, tem DPI/pneumonite atual ou quando a suspeita de DPI/pneumonite não pode ser descartada por imagem na triagem.
- Critérios pulmonares: Doenças clinicamente significativas intercorrentes específicas do pulmão, incluindo, mas não se limitando a, qualquer distúrbio pulmonar subjacente; Quaisquer distúrbios autoimunes, do tecido conjuntivo ou inflamatórios (por exemplo, artrite reumatóide, Sjögren, sarcoidose, etc.) documentados ou suspeita de envolvimento pulmonar no momento da randomização; Pneumonectomia prévia (completa); Infecção descontrolada que requer antibióticos IV, antivirais ou antifúngicos
- Imunodeficiência primária ativa, infecção conhecida pelo vírus da imunodeficiência humana (HIV) ou infecção ativa por hepatite B ou C. Pacientes positivos para anticorpo de hepatite C (HCV) são elegíveis apenas se a reação em cadeia da polimerase for negativa para RNA de HCV. Os pacientes devem ser testados para HIV antes da randomização, se exigido pelos regulamentos locais ou pelo comitê de ética (CE).
Recebimento de vacina viva atenuada (mRNA e vacinas adenovirais deficientes na replicação não são consideradas vacinas vivas atenuadas) dentro de 30 dias antes da primeira dose de trastuzumabe deruxtecano.
Nota: Os pacientes, se inscritos, não devem receber vacina viva durante o estudo e até 30 dias após a última dose de IMP.
- Alergia ou hipersensibilidade conhecida ao tratamento do estudo (T-DXd) ou a qualquer um dos excipientes do medicamento do estudo.
- História de reações graves de hipersensibilidade a outros anticorpos monoclonais.
- Pacientes do sexo feminino grávidas ou amamentando, ou pacientes que planejam engravidar.
