Comparaison des thérapies anticoagulantes et antiplaquettaires pour l'athérosténose vasculaire intracrânienne - Imagerie par résonance magnétique (CAPTIVA-MRI)

CAPTIVA-MRI est une étude d'imagerie IRM multimodale observationnelle qui est accessoire à l'essai CAPTIVA [un essai de phase III à 3 bras et en double aveugle mené sur environ 115 sites StrokeNet randomisant des patients victimes d'un AVC attribué à 70 à 99 % de sténose athérosclérotique intracrânienne (ICAS ) à l'aspirine plus ticagrélor, clopidogrel ou rivaroxaban.]
L'objectif principal de cette étude auxiliaire est de déterminer si les biomarqueurs IRM peuvent potentiellement identifier les patients ICAS qui échouent à la meilleure prise en charge médicale.
L'étude CAPTIVA-MRI s'appuie sur la conception et la mise en œuvre de l'essai CAPTIVA pour recueillir des informations qui informeront et faciliteront la prochaine génération d'essais ICAS et la prise en charge des patients atteints d'ICAS.

Patients ICAS ayant récemment subi un AVC et inscrits à CAPTIVA.
La description
Critère d'intégration:
- Recrutement dans l'essai CAPTIVA : infarctus ischémique accident vasculaire cérébral symptomatique non invalidant dans les 30 jours secondaire à une sténose de 70 à 99 % de l'ICA, du MCA, du BA ou du VA intracrânien
- Capacité à obtenir l'IRM de l'étude de base dans les 14 jours suivant l'inscription à CAPTIVA
Critère d'exclusion:
- Incapable ou refusant de subir une IRM, y compris un stimulateur cardiaque ou d'autres contre-indications à l'IRM selon les directives de l'American College of Radiology62
- Allergie au produit de contraste au gadolinium ou maladie rénale aiguë ou chronique avec DFGe<30 ml/min/1,73 m2
