Essai TACKLE-IT - Traitez le rejet aigu des lymphocytes T avec des preuves et une confiance chez les receveurs de transplantation rénale (TACKLE-IT)
Un essai contrôlé randomisé multicentrique pour traiter le rejet aigu médié par les cellules T chez les receveurs de greffe de rein et de rein-pancréas
Après une transplantation rénale ou simultanée rein-pancréas, certains patients peuvent rencontrer des problèmes avec leurs nouveaux organes. Cela se produit parce que le corps fait parfois une erreur et essaie de se débarrasser de l'organe. Ce problème est appelé « rejet ». Un type de rejet est connu sous le nom de rejet aigu médié par les lymphocytes T (TCMR). Cela peut entraîner de nombreux problèmes, voire empêcher la greffe de fonctionner.
Les médecins donnent des stéroïdes puissants pour traiter ce problème, mais il n’existe aucune règle quant à la quantité de stéroïdes à administrer. Trop de stéroïdes peut causer des problèmes tels que des problèmes cardiaques et osseux, de graves infections et une prise de poids. C'est pourquoi nous devons trouver la bonne dose de stéroïdes pour chaque personne afin de traiter ce problème.
TACKLE-IT est une étude qui tentera de trouver la bonne dose de stéroïdes pour traiter le rejet.

Critère d'intégration:
- Les participants ou leur tuteur légal doivent être capables de comprendre et de fournir un consentement éclairé écrit ;
- Volonté déclarée de se conformer à toutes les procédures d'étude et disponibilité pour la durée de l'étude ;
- Tous les groupes ethniques et de genre auront un accès égal à l'étude ;
- Tous les enfants (âgés de 2 ans et plus) et les adultes ayant reçu une greffe de rein ou de SPK avec TCMR aigu prouvé par biopsie (≥ limite de Banff (score i1 minimum), qu'il soit clinique ou subclinique).
Critère d'exclusion:
- Rejet mitigé.
- ABMR actif ou chronique actif.
- TCMR actif chronique. *Les patients présentant simultanément un TCMR aigu et un TCMR actif chronique ne seront pas exclus de l'essai.
- V1 isolé sans inflammation.
- TCMR aigu de grade supérieur, ≥Banff 2A.
- Maladie rénale concomitante, telle qu'une glomérulonéphrite récurrente ou une néphropathie à polyomavirus.
- Tumeurs malignes actives ou infection active qui empêchent l'augmentation de l'immunosuppression.
- Utilisation d'autres agents immunomodulateurs, y compris, mais sans s'y limiter, le rituximab, l'anticorps monoclonal anti-TNF, le bélatacept, l'abatacept, les inhibiteurs de Janus kinase, l'éculizumab, le pegcetacoplan.
- Inscription à d'autres essais de médicaments interventionnels.
- Utilisation d'autres agents expérimentaux.
- Incapable de respecter le protocole de l'étude.

Histological resolution of biopsy-proven acute rejection is defined by the absence of any biopsy proven acute rejection (BPAR) on follow-up biopsy, including Banff Borderline (i1 t1), mixed rejection, ABMR and chronic active TCMR using Banff 2022 criteria.

Improvement in allograft function for clinical rejection; and defined by an average of at least 2 serum creatinine at baseline and at least 2 serum creatinine from 11-13 weeks

All cause death and death-censored graft loss have been identified as the core outcomes for kidney transplant recipients. However, death and death-censored graft loss are anticipated to have a very low incidence at the 12 weeks post-randomization primary outcome ascertainment and were therefore not included in the primary composite outcome. They will be reported as principal secondary endpoints. All-cause death also includes cause-specific death (infections, cancers, cardiovascular-related, and other causes of death) at 12 weeks after randomisation. |

QoL will be assessed using the EuroQol-5 Dimension-5 Level (EQ-5D-5L) for adult participants (aged ≥18 years). For paediatric participants (aged 2-17 years), age-appropriate versions of the EuroQol Youth version (EQ-5D-Y) will be used as follows:
Both the EQ-5D-5L and EQ-5D-Y report utility scores ranging from -0.281 to 1 (EQ-5D-5L UK value set) and -0.109 to 1 (EQ-5D-Y, proxy or self-report), where 1 represents perfect health, 0 represents a health state equivalent to death, and negative scores represent health states worse than death. Higher scores indicate better health-related quality of life. |

Des entretiens semi-structurés seront menés pour susciter les perspectives et les expériences des patients d'avoir un TCMR aigu, les effets néfastes des traitements.
Les professionnels de la santé seront également interrogés pour susciter leurs perspectives et leurs expériences concernant les obstacles, les défis et les catalyseurs pour la mise en œuvre de l'intervention, les avantages perçus et les méfaits de l'intervention et comment l'intervention doit être livrée de telle sorte que les interactions maximales de l'intervention sont atteintes pour les participants prévus.

Score de prédiction du risque de boîte intégrative (IBOX)

La prédiction de survie du greffon sera évaluée à l'aide du score de prédiction de risque de boîte intégrative (IBOX), un outil de prédiction de risque composite validé pour les résultats de la transplantation rénale.
Le score IBOX intègre des paramètres cliniques, fonctionnels et immunologiques, notamment la créatinine sérique, la protéinurie, les résultats de la biopsie et la présence d'anticorps spécifiques aux donneurs pour estimer le risque de défaillance de l'allogreffe.
Le score IBox attribue essentiellement une valeur numérique basée sur ces paramètres, indiquant le risque de perte d'allogreffe (échec de la transplantation rénale).

La participation à la vie sera évaluée à l'aide de la capacité du système d'information sur la mesure des résultats (Promis®) signalée par le patient (Promis®) à participer à des rôles et des activités sociales - Short Form 4A (SF4A). Cet instrument mesure la capacité perçue à jouer un rôle social et des activités habituels. Les scores sont signalés comme des scores T standardisés à la population générale américaine (moyenne = 50, ET = 10).
Des scores plus élevés indiquent une meilleure capacité à participer aux rôles et activités sociales (c'est-à-dire un résultat plus favorable). |
