TACKLE-IT-onderzoek - Behandel acute T-celafstoting met bewijs en vertrouwen bij ontvangers van niertransplantaten (TACKLE-IT)
Een gerandomiseerd gecontroleerd onderzoek in meerdere centra voor de behandeling van acute t-celgemedieerde afstoting bij ontvangers van nier- en nier-pancreastransplantaties
Na een nier- of een gelijktijdige nier-pancreastransplantatie kunnen sommige patiënten problemen krijgen met hun nieuwe organen. Dit gebeurt omdat het lichaam soms een fout maakt en probeert het orgaan kwijt te raken. Dit probleem wordt "afwijzing" genoemd. Eén type afstoting staat bekend als acute T-cel-gemedieerde afstoting (TCMR). Dit kan tot veel problemen leiden of zelfs ervoor zorgen dat de transplantatie niet meer werkt.
Artsen geven sterke steroïden om dit probleem te behandelen, maar er zijn geen regels voor hoeveel steroïden je moet geven. Te veel steroïden kunnen problemen veroorzaken zoals hart- en botproblemen, ernstige infecties en gewichtstoename. Daarom moeten we voor elke persoon de juiste dosis steroïden vinden om dit te behandelen.
TACKLE-IT is een onderzoek dat probeert de juiste dosis steroïden te vinden voor de behandeling van afstoting.

Inclusiecriteria:
- Deelnemers of hun wettelijke voogd moeten schriftelijke geïnformeerde toestemming kunnen begrijpen en geven;
- Verklaarde bereidheid om te voldoen aan alle studieprocedures en beschikbaarheid voor de duur van de studie;
- Alle etnische en gendergroepen hebben gelijke toegang tot het onderzoek;
- Alle kinderen (2+ jaar) en volwassenen die een nier- of SPK-transplantatie hebben ondergaan met door biopsie bewezen acute TCMR (≥ Banff borderline (minimale i1-score), zowel klinisch als subklinisch).
Uitsluitingscriteria:
- Gemengde afwijzing.
- Actieve of chronisch actieve ABMR.
- Chronisch actieve TCMR. *Patiënten met gelijktijdige acute TCMR en chronisch actieve TCMR worden niet uitgesloten van het onderzoek.
- Geïsoleerde v1 zonder ontsteking.
- Hogere graad acute TCMR, ≥Banff 2A.
- Gelijktijdige nierziekte, zoals recidiverende glomerulonefritis of polyomavirus-nefropathie.
- Actieve maligniteiten of actieve infecties die vergroting van de immunosuppressie uitsluiten.
- Gebruik van andere immunomodulerende middelen, waaronder, maar niet beperkt tot, rituximab, monoklonaal antilichaam tegen TNF, belatacept, abatacept, Janus-kinaseremmers, eculizumab en pegcetacoplan.
- Deelname aan andere interventionele geneesmiddelenonderzoeken.
- Gebruik van andere onderzoeksagentia.
- Kan zich niet aan het onderzoeksprotocol houden.

All cause death and death-censored graft loss have been identified as the core outcomes for kidney transplant recipients. However, death and death-censored graft loss are anticipated to have a very low incidence at the 12 weeks post-randomization primary outcome ascertainment and were therefore not included in the primary composite outcome. QoL will be assessed using the EuroQol-5 Dimension-5 Level (EQ-5D-5L) for adult participants (aged ≥18 years). For paediatric participants (aged 2-17 years), age-appropriate versions of the EuroQol Youth version (EQ-5D-Y) will be used as follows:
Both the EQ-5D-5L and EQ-5D-Y report utility scores ranging from -0.281 to 1 (EQ-5D-5L UK value set) and -0.109 to 1 (EQ-5D-Y, proxy or self-report), where 1 represents perfect health, 0 represents a health state equivalent to death, and negative scores represent health states worse than death. Higher scores indicate better health-related quality of life. |

Semi-gestructureerde interviews zullen worden afgenomen om de perspectieven van patiënten en ervaringen van het hebben van acute TCMR, de nadelige effecten van de behandelingen op te wekken.
Gezondheidswerkers zullen ook worden geïnterviewd om hun perspectieven en ervaringen op te wekken met betrekking tot de barrières, uitdagingen en enablers voor het implementeren van de interventie, de waargenomen voordelen en schade van de interventie en hoe de interventie moet worden geleverd zodat maximale interacties van de interventie worden bereikt voor de beoogde deelnemers.

Integrative Box Risk Prediction Score (iBox)

Graft Survival Prediction zal worden beoordeeld met behulp van de Integrative Box Risk Prediction Score (IBOX), een gevalideerde composietrisicvoorspellingstool voor niertransplantatie -resultaten.
De Ibox-score integreert klinische, functionele en immunologische parameters, waaronder serumcreatinine, proteïnurie, biopsiebevindingen en de aanwezigheid van donorspecifieke antilichamen om het risico op allograftfalen te schatten.
De Ibox -score kent in wezen een numerieke waarde toe op basis van deze parameters, wat het risico van allograftverlies (niertransplantatie -falen) aangeeft.

De deelname van het leven zal worden beoordeeld met behulp van het Patiënt -gerapporteerde uitkomsten Measurement Information System (Promis®) vermogen om deel te nemen aan sociale rollen en activiteiten - korte vorm 4A (SF4A). Dit instrument meet het vermogen om gebruikelijke sociale rollen en activiteiten uit te voeren. Scores worden gerapporteerd als T-scores gestandaardiseerd naar de algemene Amerikaanse bevolking (gemiddelde = 50, SD = 10).
