Un ensayo controlado aleatorio multicéntrico para tratar el rechazo agudo mediado por células T en receptores de trasplantes de riñón y páncreas
Después de un trasplante de riñón o de un trasplante simultáneo de riñón y páncreas, algunos pacientes pueden enfrentar problemas con sus nuevos órganos. Esto sucede porque el cuerpo a veces comete un error e intenta deshacerse del órgano. Este problema se llama "rechazo". Un tipo de rechazo se conoce como rechazo agudo mediado por células T (TCMR). Esto puede provocar muchos problemas o incluso impedir que el trasplante funcione.
Los médicos recetan esteroides potentes para tratar este problema, pero no existen reglas sobre la cantidad de esteroides que se debe administrar. Demasiados esteroides pueden causar problemas como problemas cardíacos y óseos, infecciones graves y aumento de peso. Por eso necesitamos encontrar la dosis adecuada de esteroides para cada persona para tratar esto.
TACKLE-IT es un estudio que intentará encontrar la dosis de esteroides adecuada para tratar el rechazo.

Criterios de inclusión:
- Los participantes o su tutor legal deben poder comprender y proporcionar su consentimiento informado por escrito;
- Declaración de voluntad de cumplir con todos los procedimientos del estudio y disponibilidad durante la duración del estudio;
- Todos los grupos étnicos y de género tendrán igual acceso al estudio;
- Todos los niños (mayores de 2 años) y adultos que hayan recibido un trasplante de riñón o SPK con TCMR agudo comprobado por biopsia (≥ límite de Banff (puntuación i1 mínima) ya sea clínico o subclínico).
Criterio de exclusión:
- Rechazo mixto.
- ABMR activo o crónico activo.
- TCMR activo crónico. *Los pacientes con TCMR aguda concomitante y TCMR activa crónica no serán excluidos del ensayo.
- V1 aislado sin inflamación.
- TCMR aguda de grado superior, ≥Banff 2A.
- Enfermedad renal concurrente, como glomerulonefritis recurrente o nefropatía por poliomavirus.
- Neoplasias malignas activas o infección activa que impiden el aumento de la inmunosupresión.
- Uso de otros agentes inmunomoduladores, incluidos, entre otros, rituximab, anticuerpo monoclonal anti-TNF, belatacept, abatacept, inhibidores de la Janus quinasa, eculizumab, pegcetacoplan.
- Inscripción en otros ensayos de medicamentos intervencionistas.
- Uso de otros agentes en investigación.
- No se puede cumplir con el protocolo del estudio.

Histological resolution of biopsy-proven acute rejection is defined by the absence of any biopsy proven acute rejection (BPAR) on follow-up biopsy, including Banff Borderline (i1 t1), mixed rejection, ABMR and chronic active TCMR using Banff 2022 criteria.

All cause death and death-censored graft loss have been identified as the core outcomes for kidney transplant recipients. However, death and death-censored graft loss are anticipated to have a very low incidence at the 12 weeks post-randomization primary outcome ascertainment and were therefore not included in the primary composite outcome. They will be reported as principal secondary endpoints. All-cause death also includes cause-specific death (infections, cancers, cardiovascular-related, and other causes of death) at 12 weeks after randomisation. |

QoL will be assessed using the EuroQol-5 Dimension-5 Level (EQ-5D-5L) for adult participants (aged ≥18 years). For paediatric participants (aged 2-17 years), age-appropriate versions of the EuroQol Youth version (EQ-5D-Y) will be used as follows:
Both the EQ-5D-5L and EQ-5D-Y report utility scores ranging from -0.281 to 1 (EQ-5D-5L UK value set) and -0.109 to 1 (EQ-5D-Y, proxy or self-report), where 1 represents perfect health, 0 represents a health state equivalent to death, and negative scores represent health states worse than death. Higher scores indicate better health-related quality of life. |
