Altérations à long terme des interactions hôte-microbiome et progression des maladies cardiovasculaires et respiratoires après une pneumonie - le projet HOMI-LUNG - Pneumonie nosocomiale

L'étude HOMI-LUNG - HAP fait partie du projet HOMI-LUNG, financé par le programme Horizon Europe. Le projet « HOMI-LUNG » est un projet international et interdisciplinaire qui vise à mieux comprendre les liens de causalité entre les infections des voies respiratoires (c.-à-d. pneumonie) et la progression des maladies cardiovasculaires. Plus spécifiquement, le projet vise à quantifier le fardeau des maladies cardiovasculaires après une pneumonie et à évaluer l'acceptabilité par les patients des altérations de santé à long terme, ainsi qu'à définir des endotypes de pneumonie avec des mécanismes pathobiologiques distincts associés à l'exacerbation des maladies cardiovasculaires.

Groupe A (patients atteints d'une maladie cardiaque aiguë)

  • Mâle ou femelle
  • Âge ≥ 40 ans
  • Hospitalisé pour syndrome coronarien aigu depuis moins de 7 jours.
  • Consentement éclairé du patient
  • Personne assurée par un régime d'assurance maladie

Groupe B (patients atteints de maladies cardiovasculaires chroniques)

  • Mâle ou femelle,
  • Âge ≥ 40 ans
  • Subir un pontage aorto-coronarien
  • Hospitalisé en unité de soins intensifs pendant > 12 heures
  • Consentement éclairé du patient
  • Personne assurée par un régime d'assurance maladie

Groupe C (patients à risque de maladie cardiovasculaire sans maladie cardiovasculaire chronique)

  • Mâle ou femelle,
  • Âge ≥ 40 ans
  • Taux familiaux élevés de cholestérol ou de triglycérides
  • Sans antécédent personnel de maladie cardiovasculaire, avec un test d'effort cardiaque récent négatif (dernier test inférieur à 12 mois)
  • Suivi des anomalies lipidiques à haut risque d'événements de maladie cardiovasculaire
  • Consentement éclairé du patient
  • Personne assurée par un régime d'assurance maladie

Groupe D (patients atteints de HAP)

  • Mâle ou femelle
  • Âge ≥ 40 ans
  • Avec un ou plusieurs facteurs de risque de maladie cardiovasculaire parmi :

tabagisme, taux lipidiques anormaux, hypertension artérielle, obésité, diabète sucré, maladie rénale chronique

  • Guéri d'un HAP sous ventilation mécanique lors de l'hospitalisation en cours
  • Consentement éclairé du patient ou de ses proches
  • Personne assurée par un régime d'assurance maladie

Critères d'exclusion :

  • o Groupes A, B, C et D
  • Âge >80 ans
  • Immunosuppression préexistante à l'hospitalisation de référence, définie par une lymphopénie < 500 éléments/mm3, un cancer hématologique, une aplasie, une chimiothérapie/radiothérapie pour cancer dans les 3 mois précédant l'inclusion, ou un médicament anti-rejet de greffon.
  • Femmes enceintes, femmes allaitantes.
  • Adultes sous tutelle ou curatelle.
  • Faible probabilité de survie au jour 28.

    o Groupes A, B, C

  • Pneumonie nosocomiale ou pneumonie nosocomiale au cours de la dernière année.
