La description
Critères d'inclusion :
- Hommes âgés de ≥18 ans
- Antigène prostatique spécifique (PSA) sérique 4-15 ng/mL, répété au moins une fois
- IRM multiparamétrique de la prostate réalisée dans les 6 mois
IRM de la prostate avec scores PI-RADS (11) de :
- Score PI-RADS 2 avec signal d'alarme (densité de PSA >0,2 OU indice de santé prostatique, PHI>35),
- PI-RADS 3, OU
- PI-RADS 4 avec risques plus faibles de csPCa (PSAd <0,1 OU PHI<35)
- Examen rectal digital normal
- Patient acceptant une biopsie prostatique ciblée et systématique
Critères d'exclusion :
- Biopsie de la prostate dans les 5 dernières années
- Antécédents passés ou actuels de cancer de la prostate
- Contre-indication à la réalisation d'un TEP-scanner PSMA
- Contre-indication à la biopsie prostatique : infection urinaire active, échec d'insertion de la sonde d'échographie transrectale dans le rectum (résection abdominale périnéale, sténose anale), impossibilité de placement en position de lithotomie, coagulopathie non corrigeable, antiplaquettaire ou anticoagulant qui ne peut être arrêté (la poursuite de l'aspirine avant et après la biopsie est autorisée)

Dans le groupe COMBINÉ, une TEP-TDM supplémentaire du pelvis uniquement avec 18F-PSMA-1007 est réalisée dans les 6 mois suivant l'IRMmp et avant la biopsie.
Le spécialiste en médecine nucléaire interprétant la TEP-TDM est en aveugle des informations cliniques ou de l'IRM. |
