Beschrijving
Inclusiecriteria:
- Mannen ≥18 jaar oud
- Serumprostaatspecifiek antigeen (PSA) 4-15 ng/mL, minstens één keer herhaald
- Prostaat mpMRI uitgevoerd binnen 6 maanden
MRI prostaat met PI-RADS (11) scores van:
- PI-RADS score 2 met rode vlag (PSA-dichtheid >0,2 OF Prostaatgezondheidsindex, PHI>35),
- PI-RADS 3, OF
- PI-RADS 4 met lagere csPCa-risico's (PSAd <0,1 OF PHI<35)
- Normaal digitaal rectaal onderzoek
- Patiënt stemt in met gerichte en systematische prostaatbiopsie
Uitsluitingscriteria:
- Prostaatbiopsie in de afgelopen 5 jaar
- Voorgeschiedenis of huidige voorgeschiedenis van prostaatkanker
- Contra-indicatie voor het ondergaan van PSMA PET-CT-scan
- Contra-indicatie voor prostaatbiopsie: actieve urineweginfectie, mislukte inbrenging van transrectale ultrasone sonde in het rectum (abdominale perineale resectie, anusstenose), niet in staat om in lithotomiepositie geplaatst te worden, niet-corrigeerbare stollingsstoornis, antibloedplaatjes- of antistollingsmiddel dat niet kan worden gestopt (aspirine voor en na biopsie is toegestaan)

Experimenteel: COMBINED arm
In de COMBINED-arm wordt binnen 6 maanden na mpMRI en vóór biopsie een aanvullende alleen-pelvis 18F-PSMA-1007 PET-CT uitgevoerd.
De nucleair geneeskundespecialist die de PET-CT beoordeelt, is geblindeerd voor klinische of MRI-informatie.
