Sockets ajustables pour les vétérans avec amputation du membre inférieur : Résultats rapportés par les patients et pressions intra-sockets
Chaque année, environ 1 500 anciens combattants rejoignent les 623 000 Américains qui vivent avec une amputation d'un membre inférieur. Beaucoup de ces anciens combattants choisissent la voie ardue de la réadaptation pour rester mobiles, un processus qui comprend la prescription d'une prothèse de membre inférieur. Beaucoup dépend de la qualité de l'ajustement de leur prothèse. Un bon ajustement est confortable et permet une variété d'activités ambulatoires. Un mauvais ajustement entraîne un inconfort, souvent accompagné de problèmes cutanés chroniques, et les activités de la vie quotidienne sont réduites.
Les emboîtures prothétiques conventionnelles sont fabriquées sur mesure par un prothésiste qualifié, soigneusement formées pour s'adapter au moignon résiduel de l'ancien combattant, et des ajustements corrects peuvent généralement être obtenus. Ces emboîtures sont des structures rigides de forme fixe avec la robustesse d'un bulldozer, conçues pour une action soutenue et intensive. Malheureusement, la forme du moignon résiduel d'un individu peut changer avec le temps, de sorte qu'un bon ajustement finit par devenir mauvais. Une estimation suggère qu'une nouvelle emboîture sous le genou est nécessaire tous les trois ans et demi. Le coût du remplacement des emboîtures qui ne s'adaptent plus n'est pas négligeable. Les dépenses de Medicare pour le remplacement des emboîtures sous le genou existantes et des composants associés ont été estimées à 50 millions de dollars en 2017. Il est important de noter que Medicare est estimé à seulement 20 % du marché, ce qui suggère que les dépenses estimées pour l'ensemble de la population avec des amputations des membres inférieurs sont probablement nettement plus élevées. Une prothèse bien ajustée qui maintient son ajustement dans le temps servirait bien les anciens combattants.
Une solution potentielle qui pourrait prolonger la durée d'un bon ajustement est une emboîture prothétique ajustable. Bien que peu de preuves soient disponibles pour soutenir la pratique de prescription, les Centers for Medicare and Medicaid Services (CMS) ont récemment autorisé le remboursement des emboîtures prothétiques ajustables, suggérant un besoin impérieux pour un tel produit.
Pour étudier les effets potentiellement délétères et la manière dont les anciens combattants pourraient utiliser les emboîtures prothétiques ajustables, l'objectif de cette recherche est de déterminer la pression exercée sur le moignon résiduel par une emboîture prothétique ajustable lors d'activités d'intensité variable et les activités qui induisent des ajustements.
Une expérience en laboratoire sur des sujets humains sera menée à l'aide d'un capteur personnalisé placé à l'intérieur du panneau ajustable d'une emboîture prothétique ajustable fournie par l'étude. Les participants s'adapteront à l'emboîture prothétique ajustable pendant deux semaines sur le terrain, puis retourneront au laboratoire pour effectuer sept activités. La pression à l'intérieur de la prothèse sera mesurée pendant ces activités. Les participants indiqueront également à quel point leur emboîture leur semble serrée après chaque activité.

Les emboîtures prothétiques permettant la mobilité ambulatoire des vétérans ayant subi une amputation d'un membre inférieur sont généralement des éléments structurels rigides, trop solides et de forme fixe qui servent d'interface entre le corps et l'environnement. L'emboîture prothétique d'un vétéran est sur mesure ; elle est conçue pour s'adapter à une seule et unique personne.
Les changements d'ajustement après la fourniture d'une prothèse sont plus fréquents que l'inverse. Les changements liés à la masse corporelle, à l'atrophie musculaire ou à la condition physique personnelle peuvent survenir sur plusieurs mois, tandis que ceux dus à l'alimentation ou aux complications diabétiques peuvent survenir en quelques heures. Si l'écart de forme entre le moignon résiduel et l'emboîture est faible, le vétéran peut gérer la différence en enfilant ou en retirant des chaussettes pour moignon. Cette approche n'est pas idéale, car certains patients sont plus enclins à adopter cette pratique que d'autres. Si l'écart de forme est important, une nouvelle emboîture prothétique est généralement nécessaire. Que l'écart soit faible ou important, le fait de ne pas gérer les changements peut affecter négativement l'utilisation de la prothèse et la santé du moignon résiduel.
Malgré ses limites, l'emboîture prothétique de forme fixe est la norme de soins depuis des décennies. Cependant, un événement potentiellement important s'est produit en avril 2024 lorsque les Centers for Medicare and Medicaid Services (CMS) ont autorisé le remboursement des emboîtures prothétiques ajustables. Bien que la nouvelle autorisation permette le paiement de leur fourniture, il existe peu de preuves pour aider les patients, les prestataires et les payeurs à comprendre les résultats potentiels de la prescription d'une emboîture prothétique ajustable.
L'objectif de cette recherche est de découvrir comment les vétérans utilisent les emboîtures prothétiques ajustables en mesurant la pression exercée sur le moignon résiduel par une emboîture prothétique ajustable lors d'activités d'intensité variable et d'identifier quelles activités pourraient induire un ajustement. Les investigateurs recruteront des vétérans ambulatoires ayant subi des amputations transtibiales unilatérales (n = 25) et les équiperont d'une emboîture prothétique ajustable.
Les participants porteront une emboîture prothétique ajustable instrumentée après avoir porté une copie non instrumentée de l'emboîture prothétique ajustable pendant deux semaines. Les participants pourront marcher pendant 5 à 10 minutes pendant que le prothésiste de recherche vérifiera l'ajustement et l'alignement de la prothèse d'étude. Les mesures de pression seront ensuite collectées en position assise, debout et pendant la marche. Les activités debout et de marche seront effectuées dans un ordre aléatoire, entrecoupées de pauses assises d'une minute pendant lesquelles le participant relâchera la pression des panneaux ajustables. Les essais de marche seront effectués sur un tapis roulant.

- Marche en montée de 10 % à vitesse auto-sélectionnée 1 min
- Marche en descente de 10 % à vitesse auto-sélectionnée 1 min
- Marche 15 % plus rapide que la vitesse auto-sélectionnée 1 min
- Marche à vitesse auto-sélectionnée 10 min Juste avant le début de chaque activité, le participant sera informé de la prochaine activité et invité à ajuster son emboîture en conséquence. Les participants pourront également effectuer des ajustements pendant une activité. Dans cette situation, la durée de l'activité sera prolongée de manière à ce qu'il reste 1 minute d'activité avec le nouveau réglage d'ajustement. À la fin de chaque activité, la sensation de serrage sera évaluée à l'aide d'une question de sondage. Plus précisément, après chaque activité, les investigateurs demanderont à chaque participant d'évaluer la sensation de serrage de son emboîture prothétique ajustable à ce moment-là sur une échelle de 0 à 10.
En général, les investigateurs émettent l'hypothèse que la pression exercée sur le moignon résiduel dépendra de l'activité, tout comme la sensation de serrage.

Critères d'inclusion :
- Vétérans avec amputation des membres inférieurs
- ont une amputation transtibiale unilatérale
- portent une prothèse au moins 4 heures par jour selon leur propre déclaration
- au moins un an après l'amputation
- ont une prothèse modulaire comprenant une fixation d'emboîture à quatre boulons
- peuvent marcher sur un tapis roulant
Critères d'exclusion :
- ont un ajustement et une suspension inappropriés avec leur prothèse actuelle, et un bon ajustement ne peut être obtenu avec les ressources cliniques
- ont une irritation cutanée ou une blessure actuelle au membre résiduel, de l'arthrose, une blessure ou une douleur qui interfère avec la capacité de marche

Expérimental: Embase prothétique ajustable
Les participants porteront une emboîture prothétique ajustable.
|
Les investigateurs fabriqueront et assembleront une emboîture prothétique ajustable pour chaque participant, dont la forme sera une copie de leur emboîture prothétique actuelle, telle que prescrite.
Nous ajouterons trois panneaux ajustables à l'emboîture prothétique, qui seront situés au-dessus de la région tibiale proximale médiale et latérale et de la région postérieure du gastrocnémien.
La position des panneaux sera ajustable vers l'intérieur et l'extérieur à l'aide d'un mécanisme à câble.
