Justerbare protesefester
Justerbare sokker for veteraner med nedre ekstremitetsamputasjon: Pasientrapporterte resultater og trykk inne i sokken
Hvert år blir rundt 1500 veteraner en del av de 623 000 amerikanerne som lever med amputasjon av nedre ekstremitet. Mange av disse veteranene velger den krevende veien med rehabilitering for å forbli gående, en prosess som inkluderer resepsjon av protese for nedre ekstremitet. Mye avhenger av hvor godt deres protese passer. En god passform føles behagelig og muliggjør en rekke gående aktiviteter. En dårlig passform fører til ubehag, ofte ledsaget av kroniske hudproblemer, og daglige aktiviteter blir begrenset.
Konvensjonelle protesefester er spesialbygget av en dyktig protesemaker, nøye formet for å passe veteranens amputasjonsstump, og gode passformer kan vanligvis oppnås. Disse festene er stive, fastformede strukturer med robustheten til en bulldoser, bygget for vedvarende, tung bruk. Dessverre kan formen på en persons amputasjonsstump endre seg over tid, slik at en god passform til slutt blir dårlig. En estimering antyder at en ny under-kné-feste trengs hvert tredje og et halvt år. Kostnadene ved å erstatte fester som ikke lenger passer er ikke ubetydelige. Medicare-utgifter for erstatning av eksisterende under-kné-fester og tilhørende komponenter ble estimert til 50 millioner dollar i 2017. En godt passende protese som opprettholder sin passform over tid ville tjene veteranene godt.
En potensiell løsning som kan gjøre en god passform varig lengre er en justerbar protesefeste. Mens det er lite bevis tilgjengelig for å støtte resepsjonspraksis, har Centers for Medicare and Medicaid Services (CMS) nylig autorisert refusjon for justerbare protesefester, noe som antyder et presserende behov for et slikt produkt.
For å undersøke potensialet for skadelige effekter og hvordan veteraner kan bruke justerbare protesefester, er målet med denne forskningen å fastslå trykket som påføres amputasjonsstumpen av en justerbar protesefeste under aktiviteter med varierende intensitet og aktivitetene som forårsaker justeringer.
Et laboratoriebasert eksperiment med menneskelige forsøkspersoner vil bli utført ved bruk av en tilpasset sensor plassert inne i den justerbare panelen på en studie-levert justerbar protesefeste. Deltakere vil bli tilpasset den justerbare protesefesten i to uker i feltet, deretter returnere til laboratoriet for å utføre syv aktiviteter. Protesesokler som muliggjør ambulant mobilitet for veteraner med amputasjon av nedre ekstremitet er vanligvis stive, overdrevent sterke strukturelementer med fast form som fungerer som grensesnitt mellom kroppen og omgivelsene. En veterans protesesokkel er skreddersydd; den er ment å passe én person og bare én person.
Endringer i passform etter utlevering av en protese er mer vanlig enn ikke. Endringer knyttet til kroppsmasse, muskelatrofi eller personlig form kan skje over måneder, mens endringer på grunn av kosthold eller diabetiske komplikasjoner kan skje over timer. Hvis uoverensstemmelsen i form mellom amputasjonsstumpen og sokkel er liten, kan veteranen håndtere forskjellen ved å ta på eller av seg sokker til amputasjonsstumpen. Tilnærmingen er ikke ideell, da noen pasienter er mer tilbøyelige til å ta i bruk praksisen enn andre. Hvis uoverensstemmelsen i form er stor, kreves det vanligvis en ny protesesokkel. Uansett om den er liten eller stor, kan manglende håndtering av endringer negativt påvirke bruken av protesen og helsen til amputasjonsstumpen.
Til tross for begrensningene har den fastformede protesesokkelen vært standardbehandlingen i flere tiår. Imidlertid skjedde en potensielt betydningsfull hendelse i april 2024 da Centers for Medicare and Medicaid Services (CMS) autoriserte refusjon for justerbare protesesokler. Selv om den nye autorisasjonen muliggjør betaling for dens utlevering, er det lite bevis for å hjelpe pasienter, behandlere og betalere med å forstå de potensielle resultatene av å foreskrive en justerbar protesesokkel.
Formålet med denne forskningen er å oppdage hvordan veteraner bruker justerbare protesesokler ved å måle trykket som påføres amputasjonsstumpen av en justerbar protesesokkel under aktiviteter med varierende intensitet og identifisere hvilke aktiviteter som kan føre til en justering. Forskerne vil rekruttere ambulante veteraner med ensidig transtibial amputasjon (n = 25) og tilpasse dem en justerbar protesesokkel.
Deltakerne vil bruke en instrumentert justerbar protesesokkel etter å ha brukt en ikke-instrumentert duplikat justerbar protesesokkel i to uker. Deltakerne vil få lov til å gå rundt i 5 til 10 minutter mens forskningsprotesemakeren sjekker passformen og justeringen av studieprotesen. Trykkmålinger vil deretter bli samlet inn mens man sitter, står og går. Stå- og gåaktiviteter vil bli utført i tilfeldig rekkefølge, avbrutt av 1-minutters sittende hvileperioder hvor deltakeren vil løsne trykket på det justerbare panelet. Gåforsøkene vil bli utført på et løpeband.

- Gå med selvvalgt hastighet 1 min
- Gå opp 10 % stigning med selvvalgt hastighet 1 min
- Gå ned 10 % stigning med selvvalgt hastighet 1 min
- Gå 15 % raskere enn selvvalgt hastighet 1 min
- Gå med selvvalgt hastighet 10 min Like før starten av hver aktivitet vil deltakeren bli informert om hva den neste aktiviteten er og bedt om å justere sokkelens trykk deretter. Deltakerne vil også være fri til å gjøre justeringer under en aktivitet. I denne situasjonen vil varigheten av aktiviteten bli forlenget slik at 1 minutts aktivitet gjenstår ved den nye justeringsinnstillingen. På slutten av hver aktivitet vil stramheten bli vurdert ved hjelp av et spørreskjema. Spesifikt vil forskerne etter hver aktivitet be hver deltaker om å vurdere stramheten i sin justerbare protesesokkel i øyeblikket på en skala fra 0 til 10.
Generelt antar forskerne at trykket som påføres amputasjonsstumpen vil avhenge av aktiviteten, det samme vil stramheten.

- Veteraner med amputation i underkroppen
- har en ensidig transtibial amputasjon
- bruker protese i minst 4 timer per dag ifølge egenrapportering
- minst ett år etter amputasjon
- har en modulær protese som inkluderer en fire-bolt sokkel-feste
- kan gå på tredemølle
Ekskluderingskriterier:
- har dårlig passform og feste med sin nåværende protese, og en god passform kan ikke oppnås med kliniske ressurser

Eksperimentell: Justerbar protesehylse
Deltakerne vil bære en justerbar protesefeste.
|
Forskerne vil lage og montere en justerbar protesesokk for hver deltaker, hvis form er en kopi av deres eksisterende, foreskrevne protesesokk.
Vi vil legge til tre justerbare paneler på protesesokken som vil være plassert over det mediale og laterale proksimale tibia-området og det posteriore gastrocnemius-området.
Posisjonen til panelene vil være justerbar inn og ut ved hjelp av en kabelmekanisme.
