Verstelbare Prothesekokers voor Veteranen met Onderbeenamputatie: Patiëntgerapporteerde Uitkomsten en Drukken Binnen de Koker

Elk jaar sluiten ongeveer 1500 veteranen zich aan bij de 623.000 Amerikanen die leven met een amputatie van een onderste ledemaat. Veel van deze veteranen kiezen voor het zware pad van revalidatie om mobiel te blijven, een proces dat de voorschrijving van een prothese voor een onderste ledemaat omvat. Er hangt veel af van de juiste pasvorm van hun prothese. Een goede pasvorm voelt comfortabel en maakt een verscheidenheid aan mobiele activiteiten mogelijk. Conventionele prothesekokers worden op maat gemaakt door een vaardige prothesemaker, zorgvuldig gevormd om te passen bij het resterende ledemaat van de veteraan, en meestal kan een goede pasvorm worden bereikt. Deze kokers zijn rigide, vaste structuren met de robuustheid van een bulldozer, gebouwd voor aanhoudende, zware actie. Helaas kan de vorm van het resterende ledemaat van een persoon in de loop van de tijd veranderen, waardoor een goede pasvorm uiteindelijk slecht wordt. Er zal een laboratoriumgebaseerd experiment met menselijke proefpersonen worden uitgevoerd met behulp van een aangepaste sensor die in het verstelbare paneel van een voor het onderzoek verstrekte verstelbare prothesekoker wordt geplaatst. Prothesekappen die ambulante mobiliteit van veteranen met amputatie van de onderste ledematen mogelijk maken, zijn typisch starre, te sterke structurele elementen met een vaste vorm die dienen als interface tussen het lichaam en de omgeving. Een prothesekap van een veteraan is op maat gemaakt; hij is bedoeld voor één persoon en slechts één persoon.

Veranderingen in pasvorm na het verstrekken van een prothese komen vaker wel dan niet voor. Veranderingen gerelateerd aan lichaamsmassa, spieratrofie of persoonlijke conditie kunnen over maanden optreden, terwijl veranderingen door dieet of diabetische complicaties binnen uren kunnen plaatsvinden. Als het verschil in vorm tussen de stomp en de kap klein is, kan de veteraan het verschil beheren door stompsokken aan of uit te trekken. De aanpak is niet ideaal, aangezien sommige patiënten meer geneigd zijn deze praktijk toe te passen dan anderen. Als het verschil in vorm groot is, is meestal een nieuwe prothesekap vereist. Of het nu klein of groot is, het niet beheren van veranderingen kan hun gebruik van de prothese en de gezondheid van hun stomp negatief beïnvloeden.

Ondanks zijn beperkingen is de prothesekap met vaste vorm decennialang de standaardzorg geweest. Echter, een potentieel significante gebeurtenis vond plaats in april 2024 toen de Centers for Medicare and Medicaid Services (CMS) vergoeding voor verstelbare prothesekappen autoriseerden. Hoewel de nieuwe autorisatie betaling voor het verstrekken ervan mogelijk maakt, is er weinig bewijs om patiënten, zorgverleners en betalers te helpen de potentiële uitkomsten van het voorschrijven van een verstelbare prothesekap te begrijpen.

Het doel van dit onderzoek is om te ontdekken hoe veteranen verstelbare prothesekappen gebruiken door de druk die een verstelbare prothesekap op de stomp uitoefent tijdens activiteiten van verschillende intensiteit te meten en te identificeren welke activiteiten een aanpassing zouden kunnen veroorzaken. De onderzoekers zullen ambulante veteranen met unilaterale transtibiale amputaties (n = 25) werven en hen voorzien van een verstelbare prothesekap.

Deelnemers zullen een geïnstrumenteerde verstelbare prothesekap dragen na het dragen van een niet-geïnstrumenteerde duplicaat verstelbare prothesekap gedurende twee weken. Deelnemers mogen 5 tot 10 minuten rondlopen terwijl de onderzoeksprotheticus de pasvorm en uitlijning van de studieprothese controleert. Vervolgens worden drukmetingen verzameld tijdens zitten, staan en lopen. Deelnemers zullen een verstelbare prothetische koker dragen.
De onderzoekers zullen voor elke deelnemer een verstelbare prothesehuls vervaardigen en assembleren, waarvan de vorm een kopie is van hun bestaande, zoals voorgeschreven prothesehuls. We zullen drie verstelbare panelen aan de prothesehuls toevoegen die zich zullen bevinden over het mediale en laterale proximale tibia-gebied en het achterste gastrocnemius-gebied. De positie van de panelen zal verstelbaar zijn naar binnen en buiten via een kabelmechanisme.

Onze methoden zullen worden geverifieerd door de VAPSHCS Privacy Officer.

Open toegang via PubMed Central, SimTK, PhysioNet of andere vergelijkbare open-source gegevensopslagwebsites. Het onderzoeksprotocol, het statistisch analyseplan en het geïnformeerde toestemmingsformulier worden gedeeld op clinicaltrials.gov.
