Säädettävät pistokkeet alaraaja-amputaation saaneille veteraaneille: Potilasarvioinnit ja pistokepaineet
Vuosittain noin 1500 veteraania liittyy 623 000 amerikkalaisen joukkoon, jotka elävät alaraajan amputaation kanssa. Monet näistä veteraaneista valitsevat vaativan kuntoutumisen tien pysyäksekseen liikkuvina, mikä sisältää alaraajaproteesin määräyksen. Paljon riippuu heidän proteesinsa sopivuudesta. Hyvä istuvuus tuntuu mukavalta ja mahdollistaa monenlaisia liikuntatoimia. Huono istuvuus johtaa epämukavuuteen, usein kroonisten ihovaivojen kera, ja päivittäiset toiminnot rajoittuvat.
Tavanomaiset proteesisokkelit valmistetaan räätälöitynä taitavan proteesiteknikon toimesta, huolellisesti muotoiltuna sopimaan veteraanin jäännösraajaan, ja hyvät istuvuudet saadaan yleensä. Nämä sokkelit ovat jäykkiä, kiinteän muotoisia rakenteita, joiden kestävyys on kuin puskuauraa, rakennettu jatkuvaan, raskaaseen käyttöön. Valitettavasti yksilön jäännösraajan muoto voi muuttua ajan myötä niin, että hyvä istuvuus muuttuu lopulta huonoksi. Yhden arvion mukaan uusi polven alapuolinen sokkel tarvitaan joka kolmas ja puolivuosi. Sopimattomien sokkelien vaihtaminen ei ole vähäpätöinen kustannus. Medicaren menot olemassa olevien polven alapuolisten sokkelien ja niihin liittyvien komponenttien vaihtamisesta arvioitiin olevan 50 miljoonaa dollaria vuonna 2017. Tärkeää on, että Medicare arvioidaan olevan vain 20 % markkinoista, mikä viittaa siihen, että koko alaraajan amputaatiosta kärsivän väestön arvioidut menot ovat todennäköisesti huomattavasti suuremmat. Hyvin istuva proteesi, joka säilyttää istuvuutensa ajan myötä, palvelisi veteraaneja hyvin.
Mahdollinen ratkaisu, joka voisi pitää hyvän istuvuuden kauemmin, on säädettävä proteesisokkel. Vaikka käytäntöä tukevia näyttöjä on vähän saatavilla, Centers for Medicare and Medicaid Services (CMS) hyväksyi äskettäin korvaukset säädettäville proteesisokkelleille, mikä viittaa vahvaan tarpeelle tällaiselle tuotteelle.
Tutkiakseen mahdollisia haitallisia vaikutuksia ja sitä, kuinka veteraanit saattaisivat käyttää säädettäviä proteesisokkeleita, tämän tutkimuksen tavoitteena on määrittää paine, jonka säädettävä proteesisokkel kohdistaa jäännösraajaan eri intensiteetin toimintojen aikana, sekä toiminnot, jotka aiheuttavat säätöjä.
Laboratorioon perustuva ihmiskokeilu suoritetaan käyttäen mukautettua anturia, joka asetetaan tutkimuksessa tarjotun säädettävän proteesisokkelin säädettävään paneeliin. Osallistujat totutetaan säädettävään proteesisokkelin kenttäolosuhteissa kahden viikon ajan, palaten sitten laboratorioon suorittamaan seitsemän toimintoa. Proteesin sisäinen paine mitataan näiden toimintojen aikana. Osallistujat raportoivat myös, kuinka tiukalta heidän sokkelinsa tuntuu jokaisen toiminnan jälkeen.

Proteettiset pistorasiat, jotka mahdollistavat alaraajojen amputaation saaneiden veteraanien liikkumisen, ovat tyypillisesti jäykkiä, liian vahvoja, kiinteän muotoisia rakenteellisia elementtejä, jotka toimivat rajapintana kehon ja ympäristön välillä. Veteraanin proteettinen pistorasia on räätälöity; se on tarkoitettu sopimaan vain yhdelle henkilölle.
Muutokset istuvuudessa proteesin saamisen jälkeen ovat yleisempiä kuin harvinaisia. Muutokset, jotka liittyvät kehon painoon, lihasten surkastumiseen tai henkilökohtaiseen kuntoon, voivat tapahtua kuukausien aikana, kun taas ruokavaliosta tai diabeteksen komplikaatioista johtuvat muutokset voivat tapahtua tuntien sisällä. Jos jäännösraajan ja pistorasian muodon välinen epäsopivuus on pieni, veteraani voi hallita eroa laittamalla tai ottamalla pois jäännösraajasukkia. Lähestymistapa ei ole ihanteellinen, sillä jotkut potilaat ovat taipuvaisempia omaksumaan käytännön kuin toiset. Jos muodon epäsopivuus on suuri, yleensä tarvitaan uusi proteettinen pistorasia. Olipa se pieni tai suuri, muutosten hallitsematta jättäminen voi vaikuttaa negatiivisesti proteesin käyttöön ja jäännösraajan terveyteen.
Huolimatta rajoituksistaan, kiinteän muotoinen proteettinen pistorasia on ollut hoitostandardi vuosikymmeniä. Kuitenkin merkittävä tapahtuma tapahtui huhtikuussa 2024, kun Centers for Medicare and Medicaid Services (CMS) hyväksyi korvattavuuden säädettävistä proteettisista pistorasioista. Vaikka uusi valtuutus mahdollistaa maksun sen tarjoamisesta, on vähän näyttöä, joka auttaisi potilaita, palveluntarjoajia ja maksajia ymmärtämään säädettävän proteettisen pistorasian määräämisen mahdollisia tuloksia.
Tämän tutkimuksen tarkoituksena on selvittää, miten veteraanit käyttävät säädettäviä proteettisia pistorasioita mittaamalla painetta, jonka säädettävä proteettinen pistorasia kohdistaa jäännösraajaan eri intensiteetin toimintojen aikana, ja tunnistaa, mitkä toiminnot saattavat aiheuttaa säätöä. Tutkijat rekrytoivat liikkuvia veteraaneja, joilla on yksipuolinen sääriluun amputaatio (n = 25), ja asentavat heille säädettävän proteettisen pistorasian.
Osallistujat käyttävät instrumentoitua säädettävää proteettista pistorasiaa kahden viikon ajan käytettyään instrumentoimatonta kaksoiskappaletta säädettävästä proteettisesta pistorasiasta. Osallistujat saavat kävellä noin 5–10 minuuttia, kun tutkimusproteetikko tarkistaa tutkimusproteesin istuvuuden ja linjauksen. Paineen mittaukset kerätään sitten istuessa, seistessä ja kävellessä. Seisomis- ja kävelytoteutukset suoritetaan satunnaisessa järjestyksessä, ja niiden välissä on 1 minuutin istumataukoja, jolloin osallistuja vapauttaa säädettävän paneelin paineen. Kävelykokeet suoritetaan juoksumatolla.

- Kävely omavalintaisella nopeudella 10 min Juuri ennen jokaisen toiminnan alkua osallistujalle kerrotaan, mikä seuraava toiminta on, ja häntä kehotetaan säätämään pistorasiaansa vastaavasti. Osallistujat voivat myös vapaasti tehdä säätöjä toiminnan aikana. Tässä tilanteessa toiminnan kestoa jatketaan siten, että 1 minuutti toimintaa pysyy uudessa säätöasetuksessa. Jokaisen toiminnan lopussa tiukkuutta arvioidaan kyselykysymyksen avulla. Tarkemmin sanottuna tutkijat kysyvät jokaisen toiminnan jälkeen jokaiselta osallistujalta arvioimaan säädettävän proteettisen pistorasiansa tiukkuuden sillä hetkellä asteikolla 0–10.

- modulaarinen proteesi, joka sisältää neljällä pultilla kiinnitettävän pesän

Poissulkemiskriteerit:
- nykyisen proteesin sopimattomuus ja kiinnitysongelmat, joita ei voida ratkaista käytettävissä olevilla kliinisillä resursseilla

Osallistujat käyttävät säädettävää proteesisokkelia.

Tutkijat valmistavat ja kokoavat jokaiselle osallistujalle säädettävän proteesisokkelin, jonka muoto on kopio heidän nykyisestä, lääkärin määräämästä proteesisokkelistaan.
Lisäämme proteesisokkelin kolme säädettävää paneelia, jotka sijoitetaan mediaalisen ja lateraalisen lähitibian alueen sekä posteriorisen gastrokniuksen alueen päälle.
