Justerbara Protetiska Socklar
Justerbara skaft för veteraner med amputation av nedre extremitet: Patientrapporterade utfall och tryck i skaftet
Varje år ansluter sig cirka 1500 veteraner till de 623 000 amerikaner som lever med en amputation av nedre extremitet. Många av dessa veteraner väljer den mödosamma vägen till rehabilitering för att förbli gångföra, en process som inkluderar utskrivning av en protes för nedre extremitet. Mycket beror på hur väl deras protes passar. En bra passform känns bekväm och möjliggör en mängd olika gångaktiviteter. En dålig passform resulterar i obehag, ofta åtföljd av kroniska hudproblem, och aktiviteter i det dagliga livet begränsas.
Konventionella protesfästen är specialbyggda av en skicklig protesist, noggrant formade för att passa veteranens amputerade lem, och bra passformer kan vanligtvis uppnås. Dessa fästen är stela, fasta strukturer med robustheten hos en bulldozer, byggda för ihållande, tunga arbetsuppgifter. Tyvärr kan formen på en persons amputerade lem förändras över tiden, så att en bra passform så småningom blir dålig. En uppskattning tyder på att ett nytt knäunderfäste behövs var tredje och ett halvt år. Kostnaden för att byta ut fästen som inte längre passar är inte obetydlig. Medicare-utgifter för ersättning av befintliga knäunderfästen och tillhörande komponenter uppskattades till 50 miljoner dollar 2017. Viktigt är att Medicare uppskattas utgöra endast 20 % av marknaden, vilket tyder på att de beräknade utgifterna för hela befolkningen med amputationer av nedre extremitet sannolikt är betydligt större. En välpassande protes som behåller sin passform över tid skulle tjäna veteranerna väl.
En potentiell lösning som skulle kunna göra en bra passform mer långvarig är ett justerbart protesfäste. Även om det finns lite bevis som stödjer förskrivningspraxis, har Centers for Medicare and Medicaid Services (CMS) nyligen godkänt ersättning för justerbara protesfästen, vilket tyder på ett starkt behov av en sådan produkt.
För att undersöka potentialen för skadliga effekter och hur veteraner kan använda justerbara protesfästen, är syftet med denna forskning att bestämma trycket som appliceras på den amputerade lemmen av ett justerbart protesfäste under aktiviteter med varierande intensitet och vilka aktiviteter som inducerar justeringar.
Ett laboratoriebaserat experiment med mänskliga försökspersoner kommer att genomföras med hjälp av en anpassad sensor placerad inuti den justerbara panelen på ett justerbart protesfäste som tillhandahålls av studien. Deltagarna kommer att vänja sig vid det justerbara protesfästet i två veckor i fält, för att sedan återvända till laboratoriet för att utföra sju aktiviteter. Trycket inuti protesen kommer att mätas under dessa aktiviteter. Deltagarna kommer också att rapportera hur tajt deras fäste känns efter varje aktivitet.

Proteshylsor som möjliggör gående rörlighet för veteraner med amputation av nedre extremitet är vanligtvis styva, överdrivet starka strukturdelar med fast form som fungerar som gränssnitt mellan kroppen och omgivningen. En veterans proteshylsa är skräddarsydd; den är avsedd att passa en person och endast en person.
Förändringar i passform efter att en protes har tillhandahållits är vanligare än inte. Förändringar relaterade till kroppsvikt, muskelatrofi eller personlig kondition kan ske över månader medan förändringar på grund av kost eller diabetiska komplikationer kan ske över timmar. Om skillnaden i form mellan amputeringsstumpen och hylsan är liten kan veteranen hantera skillnaden genom att ta på eller ta av amputeringsstumpstrumpor. Metoden är inte idealisk eftersom vissa patienter är mer benägna att använda metoden än andra. Om skillnaden i form är stor krävs vanligtvis en ny proteshylsa. Oavsett om den är liten eller stor kan misslyckandet att hantera förändringar negativt påverka deras användning av protesen och hälsan hos deras amputeringsstump.
Trots dess begränsningar har den fastformade proteshylsan varit standardvården i årtionden. Men en potentiellt betydelsefull händelse inträffade i april 2024 när Centers for Medicare and Medicaid Services (CMS) auktoriserade ersättning för justerbara proteshylsor. Medan den nya auktoriseringen möjliggör betalning för dess tillhandahållande finns det lite bevis för att hjälpa patienter, vårdgivare och betalare att förstå de potentiella resultaten av att ordinera en justerbar proteshylsa.
Syftet med denna forskning är att upptäcka hur veteraner använder justerbara proteshylsor genom att mäta trycket som appliceras på amputeringsstumpen av en justerbar proteshylsa under aktiviteter av varierande intensitet och identifiera vilka aktiviteter som kan inducera en justering. Forskarna kommer att rekrytera gående veteraner med enkel sidig transtibial amputation (n = 25) och förse dem med en justerbar proteshylsa.
Deltagarna kommer att bära en instrumenterad justerbar proteshylsa efter att ha burit en icke-instrumenterad duplikatjusterbar proteshylsa i två veckor. Deltagarna kommer att få gå omkring i 5 till 10 minuter medan forskningsprotesisten kontrollerar passformen och inriktningen av studieprotesen. Tryckmätningar kommer sedan att samlas in medan man sitter, står och går. Stående- och gåaktiviteter kommer att utföras i slumpmässig ordning, avbrutna med 1-minuters sittande vila där deltagaren kommer att släppa trycket på den justerbara panelen. Gåförsöken kommer att utföras på ett löpband.

Slumpmässig ordning med 1 min sittande vila mellan varje:
- Stå 1 min
- Gå i självvald hastighet 1 min
- Gå uppför 10% lutning i självvald hastighet 1 min
- Gå nedför 10% lutning i självvald hastighet 1 min
- Gå 15% snabbare än självvald hastighet 1 min
- Gå i självvald hastighet 10 min Strax före början av varje aktivitet kommer deltagaren att informeras om vad nästa aktivitet är och uppmanas att justera sin hylsa därefter. Deltagarna kommer också att vara fria att göra justeringar under en aktivitet. I denna situation kommer aktivitetens varaktighet att förlängas så att 1 minut av aktivitet återstår vid den nya justeringsinställningen. I slutet av varje aktivitet kommer trångheten att bedömas med hjälp av en enkätfråga. Specifikt, efter varje aktivitet, kommer forskarna att be varje deltagare att betygsätta trångheten i sin justerbara proteshylsa i ögonblicket på en skala från 0 till 10.
I allmänhet antar forskarna att trycket som appliceras på amputeringsstumpen kommer att bero på aktiviteten, liksom trångheten.

Inklusionskriterier:
- Veteraner med amputation i nedre extremitet
- ha en ensidig transtibial amputation
- bära en protes minst 4 timmar per dag enligt egen uppgift
- minst ett år efter amputation
- ha en modulär protes som inkluderar en fyrbultad sockelfäste
- kunna gå på löpband
Exklusionskriterier:
- ha en olämplig passform och upphängning med sin nuvarande protes, och en god passform inte kan uppnås med kliniska resurser
- ha aktuell hudirritation eller skada på den kvarvarande lemmen, artros, skada eller smärta som hindrar gångförmågan

Experimentell: Justerbar protesmottagare
Deltagarna kommer att bära en justerbar protesfäste.
|
Forskarna kommer att tillverka och montera en justerbar protesfästeplats för varje deltagare vars form är en kopia av deras befintliga, förskrivna protesfästeplats.
Vi kommer att lägga till tre justerbara paneler på protesfästeplatsen som kommer att placeras över den mediala och laterala proximala tibiaregionen samt den posteriora gastrocnemiusregionen.
Panelernas position kommer att vara justerbar inåt och utåt med hjälp av en kabelmekanism.
