Toxicités tardives des radiations dans le cancer du col de l'utérus et de l'endomètre : Une étude postopératoire de radiothérapie conformationnelle avec modulation d'intensité (IMRT)/curiethérapie
ÉVALUATION DES TOXICITÉS GASTRO-INTESTINALES & GÉNITO-URINAIRES TARDIVES DANS LES CANCERS DU COL DE L'UTÉRUS & DE L'ENDOMÈTRE NÉCESSITANT UNE RCMI POST-OPÉRATOIRE ET/OU UNE CURITHÉRAPIE
Le cancer du col de l'utérus est le 4e cancer le plus fréquent chez les femmes à l'échelle mondiale et le 2e le plus fréquent en Inde. En Inde, entre 2018 et 2020, le cancer du col de l'utérus a connu une augmentation de 26 985 cas de 2018 à 2020. Le traitement du cancer du col de l'utérus dépend du stade clinique. Le traitement du cancer du col de l'utérus à un stade précoce (stade IB1-IIA) comprend la chimioradiothérapie ou la chirurgie +/- radiothérapie adjuvante (CT) et la curiethérapie vaginale (VBT) si indiqué. Le choix du traitement adjuvant repose sur l'identification de facteurs de risque spécifiques. Les patients répondant aux critères de risque intermédiaire de Sedlis nécessitent une radiothérapie pelvienne seule, et les patients présentant les critères de risque élevé de Peters nécessitent une chimioradiothérapie adjuvante. Cette approche basée sur le risque permet d'adapter les thérapies adjuvantes à chaque patient.
L'Inde a signalé 16 413 nouveaux cas et 6 385 décès dus au cancer de l'endomètre, avec un taux de mortalité de 0,73 %. Le traitement principal du carcinome de l'endomètre est l'hystérectomie abdominale totale avec salpingo-ovariectomie bilatérale (TAH-BSO). Le traitement adjuvant dépend du groupe de stratification du risque selon les directives ESGO/ESTRO/ESP, déterminé par une stratification du risque basée sur des critères moléculaires. Le traitement adjuvant comprend la radiothérapie, la chimiothérapie et la curiethérapie.
Pour réduire la charge de toxicité aiguë et tardive, des techniques avancées de radiothérapie externe comme la radiothérapie conformationnelle avec modulation d'intensité guidée par l'image (IG IMRT) sont utilisées. L'IG IMRT a démontré son potentiel à réduire la toxicité tardive chez les survivants à long terme par rapport à la technique 3DCRT. Depuis janvier 2020, notre institution (TATA Memorial Centre, Mumbai) a intégré l'IG-IMRT (Radiothérapie avec Modulation d'Intensité Guidée par l'Image) de routine pour le traitement du cancer du col de l'utérus et de l'endomètre. Cependant, aucune évaluation post-implantation des résultats du traitement et de la toxicité potentielle n'a été réalisée. Cette étude observationnelle rétrospective vise à évaluer l'application clinique de l'IG-IMRT.
L'objectif principal de cette étude est d'auditer l'incidence sur 3 ans des toxicités gastro-intestinales et génito-urinaires de grade ≥ II chez les femmes recevant une IMRT adjuvante (avec ou sans chimiothérapie) entre janvier 2020 et juin 2023.

Critères d'inclusion :
- Patients de plus de 18 ans au moment du diagnostic.
- Tous les patients avec un diagnostic histologique confirmé de cancer du col de l'utérus ou de l'endomètre.
- Seuls les patients ayant reçu une (chimio)radiothérapie adjuvante avec IMRT et/ou curiethérapie au TMH/ACTREC seront inclus.
- Patients dont les informations de suivi sont disponibles.
Critères d'exclusion :
- Patients avec des détails de traitement incomplets.
- Patient présentant une maladie résiduelle post-chirurgicale.
- Patient traité pour des récidives post-opératoires.
- Patient perdu de vue.
- Patients avec un trouble immunosuppresseur.
